Evaluation of autonomic nervous system by heart rate variability and differential count of leukocytes in athletes

Top Japanese sprinters were evaluated for their physical condition, autonomic function, blood chemistry, differential leukocyte count and blood lactate level before and after short, maximal exercise to explore methods of quantifying their conditioning level. Statistical analysis of data obtained before and 10 min after exercise were used to estimate the athletes’ autonomic capacity during recovery. Pre and post exercise variances in differential leukocyte count revealed strong correlations between neutrophil and sympathetic activity, and lymphocyte and parasympathetic activity. The results of the study demonstrated significant alterations in autonomic parameters and differential white blood cell count in response to maximal exercise.

Diffuse malignant pleural mesothelioma （DMPM） is a rare thoracic malignancy, but its incidence isdramatically increasing worldwide as a result ot widespread use of asbestos. The World Health Organization classifies DMPM into three types： epithelioid, sarcomatoid, and biphasic types. DMPM remains suffering poor prognosis and the diagnosis should always be based on adequate, representative tissue samples. There still remains a considerable number of patients with DMPM who are misdiagnosed after a complete investigation including thoracoscopic biopsies.

Association of depressive symptoms with dietary habits among Japanese physicians

The aim of this study was to determine the association of depressive symptoms with dietary habits among physicians working in hospitals in Japan. We mailed an anonymous questionnaire with the Quick Inventory of Depressive Symptomatology and items about dietary habits to 10,000 randomly selected physicians. Logistic regression analysis was used to explore the association of depressive symptoms with dietary habits. The participants comprised 3862 physicians (3025 men and 837 women). Among the respondents, 252 (8.3%) men and 88 (10.5%) women were determined to be in depressive symptoms. For men, “never eat a balanced diet” (Odds ratio;95% confidence interval, 2.54;1.70 - 3.80), and for women “eat quite small meals” (3.81;1.14 - 13.1), “always eat until full” (4.40;1.48 - 13.1), and “never eat a balanced diet” (3.05;1.49 - 6.23) were associated with depressive symptoms. For men, “do not eat too much” (0.57;0.39 - 0.83) was associated with prevention of depression. Physicians should recognize the important role of healthy dietary habits in good mental health. Hospitals should intervene to help physicians address depressive symptoms and dietary habits.

Salmonella enterica serovar Enteritidis (SE)-induced diarrhea in humans is the typical non-typhoid diarrhea. It develops acutely or subacutely and may be fatal. This SE infectious disease suddenly became a major public health issue worldwide in the 1980s. The main causative food material of SE food poisoning is chicken eggs, and many outbreaks of food poisoning caused by chicken eggs occurred throughout the world. SE epidemics occurred in layer farms, and this was the main cause of SE-induced food poisoning in humans. The major subject of our epidemiological study described in this report is why SE-contaminated eggs became the main causative food. In this study, we focused on difference of molecular expression for farm-isolated SEs. That is because recent studies have demonstrated that O-antigen enlargement may be related to pathogenicity in mice as well as 22-kDa polypeptide-expression (SEp22). We have discovered that many SE strains isolated from chicken farms do not express SEp22, and a deficiency or decreased level of cellular antigen 0-12 in SE strains isolated from chicken farms was clarified in a report. Additionally, SEp22 was deficient in SE strains passaged through chickens, whereas SEp22 was expressed at a high level in SE strains passaged through mice. These findings suggest that SE infection and retention more effectively occur in layer farms than in other animal maintenance environments, which may be a basis of the epidemiological hypothesis to explain the high-levelproduction of SE-contaminated eggs (the presence of mice may be the basis of the retention of SE infection in layer farms, and this may also be the mechanism causing the high-level production of SE-contaminated eggs).

To investigate the chemical structure of cell wall mannan of pathogenic yeast, Candida catenulata IFO 0745 strain, which possess the epitopes of antigenic factors 1, 9, and 34 to genus Candida, we previously performed the two-dimensional nuclear magnetic resonance (NMR) analysis of this mannan, Fr. C, without the need for harsh procedures. In this study, three oligosaccharides, biose, triose, and tetraose, and mannose were isolated from Fr. C by acetolysis. The results of NMR analysis indicate that the chemical structures of these oligosaccharides were identified to Manα1-2Man, Manα1-2Manα1-2Man, and Manα1-3Manα1-2Manα1-2Man. The most of resultant mannose seems to be originated from the α-1,6-linked mannan backbone which is recognized by antiserum to factor 9. The inhibition assay of slide agglutination reaction between Fr. C and antigenic antibodies using three oligosaccharides indicate that the Manα1-2Manα1-2Man and Manα1-3Manα1-2Manα1-2Man possess domains corresponding to immunodominants of antigenic factors 1 and 34, respectively.

Nasolabial cysts are uncommonly diagnosed non-odontogenic soft tissue lesions occurring close to the nasal alar region of the face. Patients usually present with a slowly enlarging asymptomatic swelling. Diagnosis is usually made in the early stages because of the esthetic effects. Histologically, the lesion is lined with non-keratinized squamous epithelium or, more frequently, pseudostratified columnar epithelium with goblet cells. These cysts are most often diagnosed in the fourth decade of life. However, we report a case of nasolabial cyst in an 80-year-old woman, and discuss the diagnosis, differential diagnosis, and treatment with reference to the literature.

Structural Investigation of Cell Wall Mannan Antigen Obtained from Pathogenic Yeast Candida zeylanoides

Antigenicity of pathogenic yeast Candida zeylanoides NBRC 10326 was found to have similar to that of Candida guilliermondii, but it was slightly different, i.e. NBRC 10326 strain cells react strongly with the factor antibodies 1, 4, and 9, and react relatively weakly with factor antibody 5. We have investigated the structural characteristic of cell wall mannan of this strain using a non-degradable method, nuclear magnetic resonance analysis. The results obtained from NMR analysis, the two-dimensional homonuclear Hartmann-Hahn, indicated that C. zeylanoides mannan contains α-1, 3-linked and β-1,2-linked oligomannosyl side chains corresponding to C. guilliermondii specific antigenic factor 9. In addition, it was found that the mannan contains β-1,2-linked oligomannosyl side chains attached to phosphate groups corresponding to anitigenic factor 5, which were not found in C. guilliermondii mannan.

Comparison of Clinical Problems in Thoracoscopic Esophagectomy between Prone Position with Pneumothorax Procedure and Lateral Position

Study Objective: We compared the effects of intraoperative body position on blood gas and fluid balance in patients undergoing a thoracoscopic esophagectomy as well as during operation and postoperative laboratory data. Design: Prospective study. Setting: Operating room and intensive care unit. Patients: ASA physical status 1 and 2 patients (n = 26), scheduled for elective thoracoscopic esophagectomy and immediate reconstruction under general anesthesia with one-lung ventilation were enrolled. Interventions: Patients were assigned to either the lateral (n = 16) or prone (n = 10) position groups based on the planned intraoperative body position. A pneumothorax procedure was concomitantly performed only in the prone position group. Measurements: Fluid balance, PaO2/FIO2 ratio (P/F ratio), and maximum PaCO2 during the operation and postoperative laboratory data were analyzed. Main Results: There were no significant differences between the groups for amount of blood loss, blood transfusion, fluid infusion, or urine output. The P/F ratio during one-lung ventilation was significantly higher in the prone than the lateral position group (379 ± 122 vs. 297 ± 67 mmHg, p = 0.017), as was maximum intraoperative PaCO2 (72.2 ± 15.6 vs. 48.3 ± 6.3 mmHg, p Conclusions: A thoracoscopic esophagectomy in the prone position performed concomitantly with a CO2 pneumothorax procedure resulted in a significantly better P/F ratio during one-lung ventilation as compared to the lateral position, indicating that the prone position is more advantageous for oxygenation.

Perinatal Morbidity, Mortality, and Neurodevelopmental Outcomes of Neonates with Fetal Growth Restriction

Objective: This study aimed to assess perinatal morbidity, mortality rates, and neurodevelopmental outcomes in the management of fetal growth restriction (FGR) at a single tertiary institute. Methods: Among 2465 deliveries between 2013 and 2019, 109 cases of FGR were reviewed retrospectively for causes, indications for pregnancy termination, perinatal death, overall neonatal outcomes, and long-term prognosis. Results: Excluding FGR due to congenital anomalies (n = 17), the mortality rate was 3.3% (3/92). One neonate delivered at 23 weeks developed cerebral palsy (1.1%). Retinopathy of prematurity occurred in four neonates (4.3%). Neurodevelopmental disorders were present in six neonates (6.5%), all of whom were delivered at 32 - 38 weeks. Significantly lower gestational age at delivery, lower birth weight, and higher umbilical artery resistance indices were observed in neonates with neurodevelopmental disorders. Conclusions: Intact survival before 27 weeks of gestation at delivery with FGR is uncommon. Neurodevelopmental disorders may still develop after delivery at 32 - 38 weeks;consideration should be given to the timing of delivery usingfetal ductus venosus Doppler waveforms measurements to reduce neurodevelopmental disorders.
