Pleomorphic Adenoma Consisting of Multiple Cysts with Squamous Epithelial Lining: Findings on MRI, FNAC, and Histopathological Examination

The incidence of pleomorphic adenoma (PA) of the minor salivary glands is reported to be 10%, and while the histological findings in PA can be diverse, keratin-filled cysts lined by squamous epithelium are rarely reported. The condition can, however, present with cyst formation in some cases. We review a rare case of pleomorphic adenoma in the buccal mucosa that involved the formation of multiple squamous epithelium-lined cysts in a 69-year-old woman. Magnetic resonance imaging (MRI), fine needle aspiration cytology, and histopathological examination were performed. Physical examination revealed a painless, mobile, elastic hard mass in the right buccal mucosa, measuring 2.5 × 1.0 cm. The MRI revealed a well-defined lesion with different signal intensities in the medial and distal regions of the right cheek. The medial side of the lesion showed a low signal intensity on T1-weighted imaging (T1WI) and T2-weighted imaging (T2WI), while the distal side showed a low signal intensity on T1WI, and a high signal intensity on T2WI and short tau inversion recovery (STIR) imaging. Fine needle aspiration of the lesion was performed under local anesthesia and a cytological diagnosis of an epidermoid or dermoid cyst was made. The tumor was completely resected under local anesthesia combined with intravenous sedation. The histopathological examination demonstrated the proliferation of atypical tumor cells with poor atypia and the formation of glandular, alveolar, large, and small cysts. The cysts were lined by keratinized squamous epithelial cells, their cavities were filled with keratinous material, and foreign body reaction was observed after rupture. Histopathological evaluation led to the diagnosis of pleomorphic adenoma. The patient had no evidence of recurrence, 2 years and 3 months after the surgery. In conclusion, the presence of multiple cysts lined by squamous epithelium can pose a significant diagnostic challenge in patients with PA. It is important to make the correct

Salivary gland tumors represent 2% - 6.5% of all head and neck tumors. Since salivary gland tumors have various clinical features and histological types, it is often difficult to diagnose and treat them. The aim of this study was to conduct a retrospective clinical-statistical analysis of 37 minor salivary gland tumors (MSGTs) treated in the Department of Oral Surgery at Nihon University School of Dentistry at Matsudo over a 16-year period. The frequencies and distributions of sex, age, occurrence site, preoperative examination (fine needle cytology and/or biopsy), treatment, and prognosis of the tumors were analyzed and compared with previous reports. The average age at diagnosis was 58.1 years (range 22 - 91 years). The peak occurrence of tumors was in the sixties (10 cases, 27.0%), followed by the forties (8 cases, 21.6%) and the seventies (7 cases, 18.9%). The average age of patients with benign tumors was 56.8 years (range 22 - 91 years). For malignant tumors, the average age was 61.7 years (range 42 - 81 years). The male-female ratio was 1:2.1 for all minor salivary gland tumors, 1:2.4 for benign tumors, and 1.5:1 for malignant tumors. There were 27 (73.0%) benign and 10 (27.0%) malignant tumors. Pleomorphic adenoma (PA) was the most common tumor (24 (64.9%) cases). Most salivary gland tumors originated from the palate (21 cases, 56.8%);the second most common site was the buccal mucosa (14 cases, 37.8%). For most patients, an intraoral mass was the primary presentation, and the second most common symptom was swelling. Surgical treatment was performed for all cases, both benign and malignant tumors. There was one case of local recurrence of PA, as well as one of multiple lung metastases after surgery for adenoid cystic carcinoma. Diagnosis and treatment of MSGTs are often delayed because the patients have few symptoms, and it is difficult to distinguish benign from malignant tumors. Purpose: The genus Pseudomonas is a ubiquitous microorganism frequently detected from immunocompromised patients. The inherent resistance to numerous antimicrobial agents contributes to the opportunistic character of this pathogen exhaustive monitoring of this pathogen is considered of critical importance to public health organizations. The reliable identification method able to distinguish genetic close Pseudomonas species is needed, because these organisms are difficult to differentiate by phenotypic or biochemical methods. The purpose of the present study was to design species-specific primers in order to identify and detect four Pseudomonas species which are frequently detected from the human oral cavities, and to investigate the distribution of these organisms in the living environment using a multiplex PCR. Methods: Polymerase chain reaction (PCR) primers were designed based on partial sequences of the rpoD gene of four Pseudomonas species. Swab samples were collected from fifty washstands, and the distribution of Pseudomonas species was investigated using a conventional PCR at genus level and a multiplex PCR at species level. Results: Multiplex PCR method developed in this study was able to distinguish four Pseudomonas species clearly. The genus Pseudomonas was detected from all samples (100%), whereas P. putida, P, aeruginosa, P. stutzeri and P. fluorescens were detected at 44%, 8%, 4% and 2% in fifty swab samples, respectively. Conclusion: Our developed one-step multiplex PCR method is accurate, specific, cost-effective, time-saving, and works without requiring DNA extraction. It was indicated that washstands were the uninhabitable environment for P. putida, P, aeruginosa, P. stutzeri and P. fluorescens.

Purpose: Recently, Candida haemulonii complex (Candida haemulonii, Candida duobushaemulonii and Candida haemulonii var. vulnera) and two genetically close species (Candida pseudohaemulonii and Candida auris) have emerged as an opportunistic fungal pathogen associated with various infectious diseases of humans, and most of those isolates have displayed antifungal resistance. Because it is difficult to differentiate these microorganisms by a current technique, unfortunately, it is important to establish a method for identifying them accurately. The purpose of the present study was to design species-specific primers in order to identify and detect C. auris, C. pseudohaemulonii, and C. haemulonii complex, i.e. , C. haemulonii, C. duobushaemulonii and C. haemulonii var. vulnera using a multiplex PCR. Methods: Polymerase chain reaction (PCR) primers were designed based on partial sequences of the 26S rRNA, 18S rRNA, and RPB1 genes and ITS region of five Candida species. Results: The multiplex PCR method developed in this study was able to distinguish five Candida species clearly. Conclusion: Our developed one-step multiplex PCR method is accurate, specific, cost-effective, time-saving, and works without requiring DNA extraction.

Study on the optical property and biocompatibility of a tissue engineering cornea

AIM: To study the optical property and biocompatibility of a tissue engineering cornea. METHODS: The cross-linker of N- (3-Dimethylaminoropyl)-N'ethylcarbodiimide hydrochloride (EDC)/ N-Hydroxysuccinimide (NHS) was mixed with Type I collagen at 10% (weight/volume). The final solution was molded to the shape of a corneal contact lens. The collagen concentrations of 10%, 12.5%, 15%, 17.5% and 20% artificial corneas were tested by UV/vis-spectroscopy for their transparency compared with normal rat cornea. 10-0 sutures were knotted on the edges of substitute to measure the corneal buttons's mechanical properties. Normal rat corneal tissue primary culture on the collagen scaffold was observed in 4 weeks. Histopathologic examinations were performed after 4 weeks of in vitro culturing. RESULTS: The collagen scaffold appearance was similar to that of soft contact lens. With the increase of collagen concentration, the transparency of artificial corneal buttons was diminished, but the toughness of the scaffold was enhanced. The scaffold transparency in the 10% concentration collagen group resembled normal rat cornea. To knot and embed the scaffold under the microscope, 20% concentration collagen group was more effective during implantation than lower concentrations of collagen group. In the first 3 weeks, corneal cell proliferation was highly active. The shapes of cells that grew on the substitute had no significant difference when compared with the cells before they were moved to the scaffold. However, on the fortieth day, most cells detached from the scaffold and died. Histopathologic examination of the primary culture scaffold revealed well grown corneal cells tightly attached to the scaffold in the former culturing. CONCLUSION: Collagen scaffold can be molded to the shape of soft contact corneal lens with NHS/EDC. The biological stability and biocompatibility of collagen from animal species may be used as material in preparing to engineer artificial corneal scaffold.

Controllable multi-domain semantic artwork synthesis

We present a novel framework for the multidomain synthesis of artworks from semantic *** of the main limitations of this challenging task is the lack of publicly available segmentation datasets for art *** address this problem,we propose a dataset called ArtSem that contains 40,000 images of artwork from four different domains,with their corresponding semantic label *** first extracted semantic maps from landscape photography and used a conditional generative adversarial network(GAN)-based approach for generating high-quality artwork from semantic maps without requiring paired training ***,we propose an artwork-synthesis model using domain-dependent variational encoders for high-quality multi-domain ***,the model was improved and complemented with a simple but effective normalization method based on jointly normalizing semantics and style,which we call spatially style-adaptive normalization(SSTAN).Compared to the previous methods,which only take semantic layout as the input,our model jointly learns style and semantic information representation,improving the generation quality of artistic *** results indicate that our model learned to separate the domains in the latent ***,we can perform fine-grained control of the synthesized artwork by identifying hyperplanes that separate the different ***,by combining the proposed dataset and approach,we generated user-controllable artworks of higher quality than that of existing approaches,as corroborated by quantitative metrics and a user study.

Efficacy of cytapheresis in patients with ulcerative colitis showing insufficient or lost response to biologic therapy

For the optimal management of refractory ulcerative colitis(UC),secondary loss of response(LOR)and primary non-response to biologics is a critical *** article aimed to summarize the current literature on the use of cytapheresis(CAP)in patients with UC showing a poor response or LOR to biologics and discuss its advantages and ***,we summarized the efficacy of CAP in patients with UC showing insufficient response to thiopurines or immunomodulators(IM).Eight studies evaluated the efficacy of CAP in patients with UC with inadequate responses to thiopurines or *** were no significant differences in the rate of remission and steroid-free remission between patients exposed or not exposed to thiopurines or *** studies evaluated the efficacy of CAP in patients with UC showing an insufficient response to biologic *** remission rates of biologics exposed or unexposed patients were 29.4%and 44.2%,*** studies evaluated the efficacy of CAP in combination with biologics in patients with inflammatory bowel disease showing a poor response or LOR to *** rates of remission/response and steroid-free remission in patients with UC ranged 32%-69%(mean:48.0%,median:42.9%)and 9%-75%(mean:40.7%,median:38%),*** had the same effectiveness for remission induction with or without prior failure on thiopurines or IM but showed little benefit in patients with UC refractory to *** heterogeneity existed in the efficacy of the combination therapy with CAP and biologics,these combination therapies induced clinical remission/response and steroid-free remission in more than 40%of patients with UC refractory to biologics on *** the excellent safety profile of CAP,this combination therapy can be an alternative therapeutic strategy for UC refractory to *** prospective studies are needed to understand the efficacy of combination therapy with CAP and biologics.

Heterogeneity in supraglacial debris thickness and its role in glacier mass changes of the Mount Gongga

In the Tibetan Plateau, many glaciers have extensive covers of supraglacial debris in their ablation zones, which affects glacier response to climate change by altering ice melting and spatial patterns of mass loss. Insufficient debris thickness data make it difficult to analyze regional debris-cover effects. Maritime glaciers of the Mount Gongga have been characterized by a substantial reduction in glacier area and ice mass in recent decades. The thermal property of the debris layer estimated from remotely sensed data reveals that debris-covered glaciers are dominant in this region, on which the proportion of debris cover to total glacier area varies from 1.74% to 53.0%. Using a physically-based debris-cover effect assessment model, we found that although the presence of supraglacial debris has a significant insulating effect on heavily debris-covered glaciers, il accelerates ice melting on -10.2% of total ablation zone and produces rapid wastage of -25% of the debris-covered glaciers, leading to the similar mass losses between the debris-covered and debris-free glaciers. Widespread debris cover also facilitates the development of active terminus regions. Regional differences in debris-cover effects are apparent, highlighting the im- portance of debris cover for understanding glacier mass changes in the Tibetan Plateau and other mountain ranges around the world.

Representative heights for assessing whole-tree values of cell-type proportions in Eucalyptus camaldulensis and E. globulus

Eucalyptus camaldulensis Dehnh. and E. globulus Labill. are economically important species for wood and pulpwood materials. Representative heights for assessing whole-tree values of cell-type proportions(vessels, fibers, ray and axial parenchyma percentages) using increment cores were examined by analysis of within-tree variations. Pattern differences were evaluated between trees and species in both radial and axial directions by statistical data analysis(Moses test). In E. camaldulensis,within-tree variation of vessel percentage was generally higher in the upper and outer regions of the trunk. In contrast, E. globulus within-tree variation was unclear. In both species, although no clear pattern of fiber percentages was observed, within-tree variations of ray and axial parenchyma levels were higher in the lower and inner *** differences in patterns were observed in the axial variation between species for vessel percentages and in the radial variation between trees of E. camaldulensis for ray parenchyma percentages. The representative heights for assessing whole-tree cell-type proportions were 0.8 mabove the ground for E. camaldulensis and 2.8 m for ***, regardless of differences in tree height and pattern of within-tree variation of cell-type proportions.
