AIM： To evaluate the safety of unsedated transnasal small-caliber esophagogastroduodenoscopy （EGD） for elderly and critically ill bedridden patients. METHODS： One prospective randomized comparative study and one crossover comparative study between transnasal small-caliber EGD and transoral conventional EGD was done （Study 1）. For the comparative study, we enrolled 240 elderly patients aged 〉 65 years old. For the crossover analysis, we enrolled 30 bedridden patients with percutaneous endoscopic gastrostomy （PEG）（Study 2）, We evaluated cardiopulmonary effects by measuring arterial oxygen saturation （SpO2） and calculating the rate-pressure product （RPP）（pulse rate x systolic blood pressure/100） at baseline, 2 and 5 min after endoscopic intubation in Study 1. To assess the risk for endoscopy-related aspiration pneumonia during EGD, we also measured blood leukocyte counts and serum C-reactive protein （CRP） levels before and 3 d after EGD in Study 2. RESULTS： In Study 1, we observed significant decreases in SpO2 during conventional transoral EGD, but not during transnasal small-caliber EGD （0.24% vs -0.24% after 2 min, and 0.18% vs -0.29% after 5 min, P = 0.034, P = 0.044）. Significant differences of the RPP were not found between conventional transoral and transnasal small-caliber EGD. In Study 2, crossover analysis showed statistically significant increases of the RPP at 2 min after intubation and the end of endoscopy （26.8 and 34.6 vs 3.1 and 15.2, P = 0.044, P = 0.046）, and decreases of SpO2 （-0.8% vs -0.1%, P = 0.042） during EGD with transoral conventional in comparison with transnasal small-caliber endoscopy. Thus, for bedridden patients with PEG feeding, who were examined in the supine position, transoral conventional EGD more severely suppressed cardiopulmonary function than transnasal small-caliber EGD. Genome-wide Association Study Reveals that the Aquaporin NIP1;1 Contributes to Variation in Hydrogen Peroxide Sensitivity in Arabidopsis thaiiana

Hydrogen peroxide （H202） is a reactive oxygen species that affects cell signaling in various plant defense responses and induces programmed cell death. To identify the new components associated with H202 signaling and tolerance, we conducted a genome-wide association study （GWAS） on the root growth of 133 Arabidopsis thaliana accessions grown in the presence of toxic H2O2 levels. The most significant SNPs were associated with a cluster of chromosome 4 genes encoding an aquaporin NODULIN 26-LIKE INTRINSIC PROTEIN 1; 1 （NIP1;1）, an NB-ARC domain-containing disease resistance protein （AT4G19050）, and a putative membrane lipoprotein （AT4G19070）. The expression level of NIP1;1 was relatively high in A. thaliana accessions sensitive to H2O2. Additionally, overexpression of NIP1;1 in a tolerant accession （e.g., Col-0） increased the sensitivity of transgenic plants to H2O2. An in planta β-glucuronidase reporter assay revealed that variations in the NIP1;1 promoter were responsible for the differences of its expression level in H2O2-tolerant and -sensitive accessions. Cell death was extensive and H2O2 levels were high in the roots of H2O2-sensitive and NIP1;1-overexpressing accessions. Together, our results indicate that the aquaporin NIP1;1 is a key determinant of the sensitivity ofA. thaliana to H2O2, and contributes to the phenotypic variations detected by our GWAS.

Plasma brain natriuretic peptide level in older outpatients with heart failure is associated with physical frailty, especially with the slowness domain

Objective To determine the association between plasma brain natriuretic peptide （BNP） level in patients with heart failure （HF） and physical frailty as well as with each domain of physical frailty. Methods Two hundred and six outpatients of cardiovascular medicine aged 60 years and older who had been hospitalized for HF or had been given a prescription medication for HF were included. Physical frailty was assessed using the following five domains： slowness, weakness, exhaustion, low activity, and shrinking, according to the Cardiovascular Health Study. Patients were divided into nonfi-ailty and frailty groups according to frailty scores. Plasma BNP level was measured. The 6-min walk test was performed to measure endurance. Results Plasma BNP was significantly different between the two groups （frailty group： 158.0 i 214.7 pg/mL, nonfrailty group： 65.2 ~ 88.0 pg/mL, P 〈 0.01）. Multivariate logistic regression analysis revealed log-transformed plasma BNP （Log BNP） was significantly associated with physical frailty （OR： 1.68, 95% CI： 1.11-2.56）, and Log BNP was significantly associated with the slowness domain （walking speed 〈 1.0 m/s） of physical frailty （OR： 1.75, 95% Ch 1.15-2.67）. Additionally, Log BNP was negatively correlated to the 6-minute walk distance （6MWD）（p=0.37, P 〈 0.01）, while 6MWD was positively correlated to walking speed （p = 0.66, P 〈 0.01）. Conclusions Plasma BNP level was related to physical frailty, especially in the slowness domain. Endurance may intervene in the associations between plasma BNP level and walking speed.

Endoscopic submucosal dissection for early esophageal neoplasms using the stag beetle knife

AIM To determine short-and long-term outcomes of endoscopic submucosal dissection(ESD) using the stag beetle(SB) knife, a scissor-shaped *** Seventy consecutive patients with 96 early esophageal neoplasms, who underwent ESD using a SB knife at Kure Medical Center and Chugoku Cancer Center, Japan, between April 2010 and August 2016, were retrospectively evaluated. Clinicopathological characteristics of lesions and procedural adverse events were assessed. Therapeutic success was evaluated on the basis of en bloc, histologically complete, and curative or non-curative resection rates. Overall and tumor-specific survival, local or distant recurrence, and 3-and 5-year cumulative overall metachronous cancer rates were also *** Eligible patients had dysplasia/intraepithelial neoplasia(22%) or early cancers(squamous cell carcinoma, 78%). The median procedural time was 60 min and on average, the lesions measured 24 mm in diameter, yielding 33-mm tissue defects. The en bloc resection rate was 100%, with 95% and 81% of dissections deemed histologically complete and curative, respectively. All procedures were completed without accidental incisions/perforations or delayed bleeding. During follow-up(mean, 35 ± 23 mo), no local recurrences or metastases were observed. The 3-and 5-year survival rates were 83% and 70%, respectively, with corresponding rates of 85% and 75% for curative resections and 74% and 49% for noncurative resections. The 3-and 5-year cumulative rates of metachronous cancer in the patients with curative resections were 14% and 26%, *** ESD procedures using the SB knife are feasible, safe, and effective for treating early esophageal neoplasms, yielding favorable short-and long-term outcomes.

Leukocyte function-associated antigen-1 deficiency impairs responses to polymicrobial sepsis

AIM: To determine the role of leukocyte functionassociated antigen-1(LFA-1) in polymicrobial sepsis model in ***: Cecal ligation and puncture model was used to study polymicrobial sepsis in wild type and LFA-1 knockout(KO)(= CD11 a KO) mice. Their survivals were examined. Neutrophil recruitment to the abdominal cavity, bacterial tissue load and bacterial killing by neutrophils, tissue cytokine profiles, and serum cytokines were examined. Apoptosis of tissues was assessed using cleaved-caspase 3 and TUNNEL staining. The recruitment of neutrophils to various tissues was assessed using myeloperoxidase staining or measuring myeloperoxidase activity. RESULTS: LFA-1 deficiency significantly decreased survival(P = 0.0024) with the reduction of neutrophil recruitment to the abdominal cavity and higher bacterial load in blood. It was also associated with increased apoptosis in spleen and more organ injuries probed by interleukin-6 m RNA level. However, the deficiency of LFA-1 did not prevent neutrophil recruitment to lung, liver, spleen or kidney, which suggested the existence of LFA-1 independent recruitment mechanism in these organs. CONCLUSION: LFA-1 deficiency did not attenuate neutrophil recruitment to various organs to adequately mitigate secondary tissue injury in sepsis. It was associated with decreased neutrophil recruitment to the abdominal cavity, higher bacterial load, leading to increased mortality in an abdominal, polymicrobial sepsis.

Steering performance of an inverted pendulum vehicle with pedals as a personal mobility vehicle

From the viewpoints of environmental protection, support for the aged and ensuring the right to mobility, there is a need to develop a new type of mobility vehicle that provides more effective transportation. The authors propose an inverted pendulum vehicle with pedals as one of the forms of personal mobility vehicles （PMVs）. In this paper, the steering performance of the inverted pendulum vehicle with pedals is discussed based on experiments on a prototype. From the experimental results, it was confirmed that the errors from the five subjects for the target trajectory and the five-grade evaluation of the maneuverability were similar. Finally, we created an inverted pendulum vehicle with pedals to which was added a reaction actuator for the steering system. From the experimental results, it was found that setting appropriate feedback gains for the handle steering angle and its rate of rotation, which control the right and left wheel driving torques, resulted in greatly improved maneuverability. Particle selectivity of filtering by C. elegans

A nematode Caenorhabditis elegans(C. elegans) is a filter feeder, which draws a suspension of bacteria and separates bacteria from the solvent by using pharyngeal pumping motions and specific mouth parts. This mechanism has not been fully understood. We investigated the mechanism of filtering of bacteria in the pharynx of C. elegans by visualization by fluorescent particles and dyed E. coli. We succeeded in quantifying the selectivity of bacteria-sized particles by C. elegans. The most accumulated particles were those of 0.5 μm in diameter. The quantity of accumulated particles of 0.2 μm or 1.0 μm in diameter was about one third of that of particles of 0.5μm in diameter. The least accumulated particles were those of 0.05 μm in diameter. These results suggest that the pharyngeal structures of C. elegans would be suitable for eating bacteria because the size of bacteria ingested by C. elegans worms is about 0.5 μm in diameter. We also succeeded in visualizing pharyngeal structures and pumping motions and flow in the pharynx. We found that there were phase differences in the motions among procorpus, metacorpus and isthmus. This result suggests filtering would occur at the two tips of procorpus and isthmus by the phase differences. We found that bacteria-sized particles and bacteria were flowed and trapped in the channels, which existed along the central lumen from tip of procorpus to isthmus. From our results, we proposed the novel mechanism of filtering of bacteria through the channels for flowing and trapping. In future, this selective filtering mechanism of C. elegans would be applied to development of microfluidic filtration devices for medical and biological equipment.

Sphingosine kinase 1 is upregulated with lysophosphatidic acid receptor 2 in human colorectal cancer

AIM: To examine the expression of SphK1, an oncogenic kinase that produces sphingosine 1-phosphate (S1P), and its correlation with the expression of LPAR2, a major lysophosphatidic acid (LPA) receptor overexpressed in various cancers, in human colorectal ***: Real-time reverse-transcription polymerase chain reaction was used to measure the mRNA expression of SphK1, LPAR2, and the three major S1P receptors in 27 colorectal cancer samples and corresponding normal tissue samples. We also examined the correlation between the expression of SphK1 and ***: Colorectal cancer tissue in 22 of 27 patients had higher levels of SphK1 mRNA than in normal tissue. In two-thirds of the samples, SphK1 mRNA expression was more than two-fold higher than in normal tissue. Consistent with previous reports, LPAR2 mRNA expression in 20 of 27 colorectal cancer tissue samples was higher compared to normal tissue samples. Expression profiles of all three major S1P receptors, S1PR1, S1PR2, and S1PR3, varied without any trend, with no significant difference in expression between cancer and normal tissues. A highly significant positive correlation was found between SphK1 and LPAR2 expression [Pearson’s correlation coefficient (r) = 0.784 and P < 0.01]. The mRNA levels of SphK1 and LPAR2 did not correlate with TNM ***: Our findings suggest that S1P and LPA may play important roles in the development of colorectal cancer via the upregulation of SphK1 and LPAR2, both of which could serve as new therapeutic targets in the treatment of colorectal cancer.
