Development of a validation technique for road surface profile applicable to point cloud data

In recent years,various information and communication technology(ICT)devices measuring three-dimensional(3D)point cloud data have been developed and widely used for the application of pavement surface ***,ICT devices have generally been developed not only for measuring road surface profiles but for various geo-reference point *** this background,the validation of surface profiles acquired with ICT devices fulfils an important role in proving the reliability of measurement result composed by point *** study proposes a wavelet transform agreement(WTA)which involves a normalization factor of profile amplitude for further improvement in the wavelet-based coherence *** WTA analysis allows evaluating similarity/dissimilarity of two profiles considering both the location and wavelength *** this purpose,a terrestrial laser scanner(TLS),a mobile mapping system(MMS),and an unmanned aerial vehicle(UAV)are employed to prove the advantage of WTA in practical *** a result,the advantages of WTA analysis are clearly recognized in the optimization for the measurement interval of TLS,the multi-line measurement of MMS for ride quality improvement of a pavement,and the efficient operation of UAV in terms of the flight *** paper also shows the identification of aging development for surface roughness over time in terms of locations and *** findings help not only to guarantee the accuracy of profile measurements but to realize the sophisticated way of using 3D point clouds acquired with ICT *** outcomes of this study contribute to the increase of productivity for pavement works with improving the quality of surface profile measurement.

Endoscopic submucosal dissection for early esophageal neoplasms using the stag beetle knife

AIM To determine short-and long-term outcomes of endoscopic submucosal dissection(ESD) using the stag beetle(SB) knife, a scissor-shaped *** Seventy consecutive patients with 96 early esophageal neoplasms, who underwent ESD using a SB knife at Kure Medical Center and Chugoku Cancer Center, Japan, between April 2010 and August 2016, were retrospectively evaluated. Clinicopathological characteristics of lesions and procedural adverse events were assessed. Therapeutic success was evaluated on the basis of en bloc, histologically complete, and curative or non-curative resection rates. Overall and tumor-specific survival, local or distant recurrence, and 3-and 5-year cumulative overall metachronous cancer rates were also *** Eligible patients had dysplasia/intraepithelial neoplasia(22%) or early cancers(squamous cell carcinoma, 78%). The median procedural time was 60 min and on average, the lesions measured 24 mm in diameter, yielding 33-mm tissue defects. The en bloc resection rate was 100%, with 95% and 81% of dissections deemed histologically complete and curative, respectively. All procedures were completed without accidental incisions/perforations or delayed bleeding. During follow-up(mean, 35 ± 23 mo), no local recurrences or metastases were observed. The 3-and 5-year survival rates were 83% and 70%, respectively, with corresponding rates of 85% and 75% for curative resections and 74% and 49% for noncurative resections. The 3-and 5-year cumulative rates of metachronous cancer in the patients with curative resections were 14% and 26%, *** ESD procedures using the SB knife are feasible, safe, and effective for treating early esophageal neoplasms, yielding favorable short-and long-term outcomes.

Gastric and enteric anisakiasis successfully treated with Gastrografin therapy: A case report

We report a case of a 59-year-old woman who was diagnosed with gastric and small intestinal anisakiasis, which was successfully treated with endoscopic extraction and Gastrografin therapy. She was admitted to our hospital with epigastric pain and vomiting one day after eating raw fish. She exhibited tenderness in the epigastrium without obvious rebound tenderness or guarding. Computed tomography (CT) demonstrated segmental edema of the intestinal wall with proximal dilatation and a small number of ascites. Because enteric anisakiasis was suspected based on the patient's history of recent raw fish consumption and abdominal CT, we performed gastroscopy and confirmed that nine Anisakis larvae were attached to the gastric mucosa. All of the Anisakis larvae were extracted via endoscopy, and the patient was diagnosed with gastric and enteric anisakiasis. Additionally, in the hospital, we performed ileography twice using Gastrografin, which led to shortened hospital stay. Based on the clinical results of this case, we suggest that Gastrografin therapy is a safe, convenient, and useful method to extract enteric Anisakis larvae.

An Unexpected Complication by Misplacement of an External Ventricular Drain;Transient Oculomotor Palsy Due to Direct Compression by a Drain—A Case Report

Background: Insertion of an external ventricular drain (EVD) is an important neurosurgical technique. There is a minor risk of misplacement, and complications including infections or hemorrhages have been reported. Transient neurological complications are, however, very rare, especially when EVD is placed under endoscope assistance. We report a case of unilateral transient oculomotor palsy after an EVD procedure. Case presentation: An 11-year-old boy with past medical history of moyamoya disease suffered from intraventricular hemorrhage and acute hydrocephalus. Insertion of EVD and hematoma removal was performed with endoscope assistance. After the surgery, transient oculomotor palsy occurred by the direct compression of the brain stem by the drainage tube. Conclusion: Transient unilateral oculomotor palsy due to the direct compression of the midbrain by an EVD tube is a very rare, but possible complication, even under endoscopic assistance. Attention through the procedure is required until skin closure to avoid tube dislocation.

Correlations between Motor Ability with Gait Speed and Cognitive Functions in Rehabilitation Users

Purpose: To compare and examine motor and cognitive functions in people who require support, and in healthy elders. Method: The variables of age, BMI, educational background, blood pressure, grip strength, knee extension, gait speed, MMSE, and subjective health feeling for 36 rehabilitation users and 22 healthy elders were assessed. We compared and examined motor and cognitive functions in rehabilitation users who need support and healthy elders. Results: The percentage of MMSE scores with 27 points or higher and that of subjective health feeling with 3 points or higher were significantly higher in healthy elders than in rehabilitation users. Systolic and diastolic blood pressure were significantly higher in rehabilitation users than in healthy elders. Gait speed and MMSE scores were significantly higher in healthy elders than rehabilitation users. Conclusion: Gait speed could be used for physical fitness in elders. Gait speed was the best physical fitness measurement for elders. Gait speed of rehabilitation users was strongly related to MMSE scores. Gait speed could be a determinant of MMSE scores.

Evaluation of the Tokyo Cognitive Assessment for Predicting Cognitive Impairment in Rehabilitation Users

[Purpose] This study aimed to evaluation of the Tokyo cognitive assessment (Toca) for predicting cognitive impairment in rehabilitation users. [Method] The variables of age, BMI, educational background, Toca for 36 rehabilitation users and 22 healthy elder were assessed. We compared cognitive functions in rehabilitation users who need support and healthy elders. [Results] Of the Toca 1 to 11 trials, there was a significant difference other than 9, 10 rials. The Toca scores with 8 points higher were significantly higher in healthy elders than in rehabilitation users. The area under the receiver-operator curve (AUC) for predicting mild cognitive impairment (MCI) by the Toca was 0.874. Using a cut-off point of 19/20, the Toca demonstrated a sensitivity of 83.9% and a sensitivity of 85.0% in diagnosing MCI. [Conclusion] The Toca is a brief cognitive screening tool with high sensitivity and specificity for detecting MCI as currently conceptualized in rehabilitation users.

Feature-Tracking Strain Derived from Compressed Sensing Cine Cardiovascular Magnetic Resonance Imaging for Myocardial Infarct Detection: A Feasibility Study

Purpose: This study aimed to use gadolinium-enhanced cardiovascular magnetic resonance (LGE-CMR) scanning to examine the clinical feasibility of feature-tracking strain (FT-strain) analysis on compressed sensing (CS) cine cardiovascular magnetic resonance (CMR) imaging for detecting myocardial infarction (MI). Methods: We enrolled 37 patients who underwent conventional cine CMR, CS cine CMR, and LGE-CMR scanning to assess cardiovascular disease. FT-strain analysis was used to assess peak circumferential strain (p-CS) based on an 18-segment model in both cine CMR imaging modalities. Based on LGE-CMR imaging findings, myocardial segments were classified as remote, adjacent, subendocardial infarcted, and transmural infarcted. The diagnostic performance of p-CS for detecting MI was compared between CS cine CMR imaging and conventional cine CMR imaging using the receiver operating characteristic (ROC) curve analysis. Results: A total of 440 remote, 85 adjacent, 76 subendocardial infarcted, and 65 transmural infarcted segments were diagnosed on LGE-CMR imaging. There were significant between-group differences in p-CS on both conventional and CS cine CMR (p Conclusion: FT-strain analysis of CS cine CMR imaging may help identify MI;it may be used alongside or instead of conventional CMR imaging.

Case Report: Asymptomatic Osteosarcoma

Osteosarcomas are primary malignant tumors that are characterized by the production of osteoid or immature bone. However they are the most common primary malignant tumor of bone in adolescents and young adults, the diagnosis of osteosarcoma is often delayed. It is because of the rarity of population, the variety of findings, nonspecific symptoms and so on. A 46-year-old male patient had left knee pain for 3 months without any symptoms in his right knee. At a nearby clinic, X-ray of his healthy side was taken by chance. The irregular bone formation was however seen in the metaphysis of the distal femur with spreading into the soft tissues. No abnormal findings were pointed out at his left side knee. A diagnosis of pleomorphic sarcoma was made by incisional biopsy. Pulmonary CT showed multiple nodules which were considered as multiple lung metastases. He was diagnosed as right femoral sarcoma, clinical stage IV (T1N0M1a) by TNM classification. He underwent preoperative chemotherapy, followed by limb-saving operation. No preoperative complaints were found in his left knee. Final diagnosis of surgically resected specimen was osteoblastic osteosarcoma which showed neoplastic osteoid formation within pleomorphic malignant cells. However, osteosarcomas occurred in adult populations are sometimes occurred as a secondary neoplasms, he had a history of diabetes but no other medical history, such as radiation therapy for a solid cancer, Paget’s disease, other benign bone lesions or inherited conditions. As far as our knowledge, we believe that this is a first case report of asymptomatic incidental osteosarcoma.
