Does high altitude increase risks of the elderly patients with coronary artery disease？

Objective To assess the effect of altitude hypoxia on the elderly patients with coronary artery disease （CAD）. Methods Three subject groups were surveyed during their train trip on the highest railroad the Qinghai-Tibet Railway： 22 elderly individuals with documented CAD, 20 healthy elderly controls, and 20 healthy young controls, all of whom from Beijing near the sea level （76 m）, Survey questions addressed clinical features of their healthy conditions and aspects of their coronary disease. The baseline study was performed at Xining at an altitude of 2261m, and then during acute exposure to altitudes of 2808 m, 4768m, 5072 m and 4257 m by train for 24 hours. Resting pulse rate, blood pressure, oxygen saturation, electrocardiograph （ECG）, and cardiac work estimated by the heart rateblood pressure double product were obtained five times in each subject at different altitudes. Results On arrival to altitudes between 4768 m and 5072 m, the older passengers, especially those with preexisting coronary disease, had higher HR, higher BP, and lower SaO2, as well as more frequent abnormalities on ECG, as compared to the younger healthy subjects. As compared with the healthy elderly controls, incomplete right bundle branch block, left ventricular hypertrophy, and ST segment depression were more frequently seen in the elderly coronary patients （P〈0.01）. Cardiac work in group 1 was increased by 13% 12 hours after arrival to altitudes between 2808 m and 5072 m. Oxygen saturation decreased significantly with the altitude increasing by train ascent but improved after inhalation of oxygen. Most of the older subjects tolerated their sojourn at high altitude well except one who developed angina repeatedly with a significant ST segment depression. Conclusions Coronary events and ECG signs of myocardial ischemia are rare in elderly individuals with CAD who travel from sea level to moderate altitudes of 1500m to 2800 m. 目的探讨应用芯片检测高糖和葡萄糖胺诱导的基因表达评估肾系膜细胞氧化应激的预警意义和价值。方法选择120例糖尿病肾病(DN)患者为观察组,另选同时间段120例健康体检者为对照组。采用芯片检测高糖和葡萄糖胺诱导的基因表达,检测两组细胞氧化应激及肾功能指标。比较两组谷氨酰胺果糖6-磷酸氨基转移酶(GFAT)、细胞外基质(ECM)、转化生长因子α、泛素样结构域蛋白成员1、硫氧还蛋白(TRX)、谷胱甘肽过氧化酶1、代谢型谷氨酸受体1、70 kDa热休克蛋白5基因表达水平,细胞氧化应激[还原型谷胱甘肽(GSH)与氧化型谷胱甘肽(GSSG)的比值(GSH/GSSG)、活性氧自由基]及肾功能指标[尿微量白蛋白、肌酐(Cr)、视黄醇结合蛋白(RBP)、β_(2)微球蛋白(β_(2)-MG)、胱抑素C(Cys C)、尿素氮(BUN)];采用Pearson线性相关分析高糖和葡萄糖胺诱导的基因表达与肾系膜细胞氧化应激的相关性。结果观察组GFAT、ECM、转化生长因子α、泛素样结构域蛋白成员1、TRX、谷胱甘肽过氧化酶1、代谢型谷氨酸受体1、70 kDa热休克蛋白5基因表达水平分别为(1.57±0.32)、(1.61±0.35)、(1.43±0.29)、(1.44±0.32)、(1.63±0.37)、(1.32±0.24)、(1.54±0.27)、(1.58±0.33),高于对照组的(0.63±0.11)、(0.75±0.17)、(0.46±0.07)、(0.78±0.19)、(0.54±0.12)、(0.68±0.14)、(0.43±0.09)、(0.84±0.21)(P<0.05)。观察组GSH/GSSG(6.67±0.11)低于对照组的(9.02±0.07),活性氧自由基(2853.73±62.16)μmol/L、尿微量白蛋白(42.64±3.26)mg/24 h、Cr(213.85±89.41)μmol/L、RBP(114.29±16.25)mg/L、β_(2)-MG(3.05±0.21)mg/L、Cys C(5.31±0.61)mg/L、BUN(12.13±2.14)mmol/L高于对照组的(2325.91±55.79)μmol/L、(19.21±1.52)mg/24 h、(91.19±14.29)μmol/L、(40.24±8.51)mg/L、(1.32±0.06)mg/L、(1.03±0.23)mg/L、(4.39±0.84)mmol/L(P<0.05)。Pearson线性相关分析结果显示:DN患者GFAT、ECM、转化生长因子α、泛素样结构域蛋白成员1、TRX、谷胱甘肽过氧化酶1、代谢型谷氨酸受体1、70 kDa热休克蛋白5基因与活性氧自由基呈正相关,与GSH/GSSG呈负相关(P<0.05)。结论应用芯片能检测高糖和葡萄糖胺诱导的基因表达水平,且与肾系膜细胞氧化应激存在强相关性,能发挥良好的预警作用,为后续诊疗提供依据。

Cytokines have been implicated in pancreatic β-cell destruction leading to type 1 *** to interleuken-1β（IL-1β）of pancreatic β-cells induces expression of inducible nitric oxide syn-thase（iNOS）and cyclooxygenase-2（COX-2）.Subsequent formation of nitric oxide（NO）and prostaglandinE2（PGE2）may impair β-cell *** NOS inhibitor NG-monomethyl-L-arginine（L-NMMA）,wehave further investigated the relation between NO formation and COX-2 ***-1β stimulated theformation of NO and PGE2 by pancreatic β-cells.L-NMMA completely inhibited IL-1β-induced NO forma-tion and attenuated PGE2 ***-2 gene transcription level and protein expression were deter-mined by real-time PCR,Western blot and luciferase analysis.L-NMMA inhibited IL-1β-induced promoteractivity,gene transcription and protein expression of COX-2 in pancreatic β-***,we concludedthat NO-affected COX-2 activity is directly linked to COX-2 gene transcription and protein expression inpancreatic β-*** identification of a novel interaction of NO on the COX signaling pathway in β-cellsprovides a strategy of intervention for further evaluating the role of NO and PGE2 in autoimmune diabetes.

Development and validation of a nomogram prediction model for overall survival in patients with rectal cancer

BACKGROUND Rectal cancer is prevalent and associated with substantial morbidity and *** To develop a nomogram prediction model for overall survival(OS)in patients with rectal cancer by leveraging a comprehensive analysis of demographic,clinicopathological,haematological,and follow-up data to identify independent prognostic *** We conducted a prospective cohort study in China involving rectal cancer patients and applied Cox regression and least absolute shrinkage and selection operator regression to assess the significance of various variables as independent prognostic factors for *** identified factors were integrated into a nomogram model,which was evaluated for predictive accuracy via the C-index,area under the curve(AUC),calibration curve,and decision curve analysis(DCA).RESULTS Multivariate analysis revealed independent predictors of OS,including the Karnofsky performance status,age,sex,TNM stage,chemotherapy,surgery,targeted therapy,β2-microglobulin,lactate dehydrogenase,and the neutrophil-to-lymphocyte *** nomogram demonstrated a C-index of 0.80 for the training and validation cohorts,with AUC values indicating high predictive accuracy for 1-year,3-year,and 5-year *** calibration curves confirmed the model's excellent agreement with the observed survival rates,and DCA revealed the superior clinical utility of the nomogram over the TNM staging *** In this study,a novel prognostic model that accurately predicts the OS of rectal cancer patients was *** model exhibited excellent discriminatory and calibration capabilities,thus offering a reliable tool for health care professionals to estimate patient survival.

As an emerging environmental contaminant,antibiotic resistance genes(ARGs)in tap water have attracted great *** studies have provided ARG profiles in tap water,research on their abundance levels,composition characteristics,and potential threat is still ***,9 household tap water samples were collected from the Guangdong-Hong Kong-Macao Greater Bay Area(GBA)in ***,75 sets of environmental sample data(9 types)were downloaded from the public *** was then performed to explore the differences in the abundance and composition of ARGs.221 ARG subtypes consisting of 17 types were detected in tap *** the ARG abundance in tap water was not significantly different from that found in drinking water plants and reservoirs,their composition *** tap water samples,the three most abundant classes of resistance genes were multidrug,fosfomycin and MLS(macrolide-lincosamidestreptogramin)ARGs,and their corresponding subtypes ompR,fosX and macB were also the most abundant ARG *** the potential mobility,vanS had the highest abundance on plasmids and viruses,but the absence of key genes rendered resistance to vancomycin ***,the majority of ARGs present in tap water were those that have not been assessed and are currently not listed as high-threat level ARG families based on the World Health Organization *** the current potential threat to human health posed by ARGs in tap water is limited,with persistent transfer and accumulation,especially in pathogens,the potential danger to human health posed by ARGs should not be ignored.

Sodium-ion batteries have emerged as competitive substitutes for low-temperature applications due to severe capacity loss and safety concerns of lithium-ion batteries at−20°C or ***,the key capability of ultrafast charging at ultralow temperature for SIBs is rarely ***,a hybrid of Bi nanoparticles embedded in carbon nanorods is demonstrated as an ideal material to address this issue,which is synthesized via a high temperature shock *** a hybrid shows an unprecedented rate performance(237.9 mAh g^(−1) at 2 A g^(−1))at−60℃,outperforming all reported SIB anode *** with a Na_(3)V_(2)(PO_(4))_(3)cathode,the energy density of the full cell can reach to 181.9 Wh kg^(−1) at−40°*** on this work,a novel strategy of high-rate activation is proposed to enhance performances of Bi-based materials in cryogenic conditions by creating new active sites for interfacial reaction under large current.

四川盆地页岩气勘探已在川东南—川南海相下志留统、川东—川东北湖相下侏罗统泥页岩层系获得了商业发现和工业气流,展示了四川盆地页岩气具有多领域、多层系、多类型的特点。采用氩离子抛光—扫描电镜、压汞—吸附联合测定、脉冲渗透率和X-射线衍射等实验测试方法,通过对海相、湖相泥页岩沉积相、生烃条件、储集特征、可压裂性和富集类型多方面对比表明,海相龙马溪组优质页岩主要发育于克拉通台内凹陷深水陆棚沉积,具有分布广,厚度稳定,有机碳含量高,有机质孔、页理、有机硅均发育的良好的生、储、可压裂一体的页岩气物质基础,3类页岩气富集类型(盆内超高压页岩气型、盆内缘超压页岩气型和盆外常压页岩气型),其中盆内超高压页岩气型和盆内缘超压页岩气型为最有利的页岩气勘探类型;湖相大安寨段有利的页岩与灰岩薄互层岩性组合,主要发育于最大湖进期碳酸盐岩湖泊的湖坡风暴滩与湖坡泥交替沉积环境,具累计厚度大,有机碳含量较高,粒缘孔缝发育,渗透性较好,钙质脆性矿物含量高的较好的生、储、可压裂近配的页岩气物质基础,是湖相有利的页岩气勘探类型。针对海、湖相不同类型页岩气的形成特点,开展适应性勘探开发技术的攻关,是实现多领域页岩气经济有效开发的关键。

Residual symmetry and interactions between solitons and periodic waves of the Drinfeld-Sokolov-Satsuma-Hirota system

In this paper,the Drinfeld-Sokolov-Satsuma-Hirota(DSSH)system is studied by using residual symmetry and the consistent Riccati expansion(CRE)method,*** residual symmetry of the DSSH system is localized to Lie point symmetry in a properly prolonged system,based on which we get a new Bäcklund transformation for this *** symmetry reduction solutions of the DSSH system are obtained by applying the classical Lie group approach on the prolonged ***,the DSSH system proves to be CRE integrable and new interesting interaction solutions between solitons and periodic waves are generated and analyzed.
