BACKGROUND Rectal cancer ranks as the second leading cause of cancer-related mortality worldwide,necessitating surgical resection as the sole treatment *** the years,there has been a growing adoption of minimally invasive surgical techni-ques such as robotic and laparoscopic *** surgery represents an innovative modality that effectively addresses the limitations associated with traditional laparoscopic *** previous studies have reported favo-rable perioperative outcomes for robot-assisted radical resection in rectal cancer patients,further evidence regarding its oncological safety is still *** To conduct a comparative analysis of perioperative and oncological outcomes between robot-assisted and laparoscopic-assisted low anterior resection(LALAR)*** The clinical data of 125 patients who underwent robot-assisted low anterior resection(RALAR)and 279 patients who underwent LALAR resection at Shandong Provincial Hospital Affiliated to Shandong First Medical University from December 2019 to November 2022 were retrospectively *** performing a 1:1 propensity score matching,the patients were divided into two groups:The RALAR group and the LALAR group(111 cases in each group).Subsequently,a comparison was made between the short-term outcomes within 30 d after surgery and the 3-year survival outcomes of these two *** Compared to the LALAR group,the RALAR group exhibited a significantly earlier time to first flatus[2(2-2)d vs 3(3-3)d,P=0.000],as well as a shorter time to first fluid diet[4(3-4)d vs 5(4-6)d,P=0.001].Additionally,the RALAR group demonstrated reduced postoperative indwelling catheter time[2(1-3)d vs 4(3-5)d,P=0.000]and decreased length of hospital stay after surgery[5(5-7)d vs 7(6-8)d,P=0.009].Moreover,there was an observed increase in total cost of hospitalization for the RALAR group compared to the LALAR group[10777(10780-11850)dollars vs 10550(8766-11715)dollars,P=0.012].No signi

Effects of intensive control of blood glucose and blood pressure on microvascular complications in patients with type Ⅱ diabetes mellitus

AIM: To evaluate the effects of intensive control of blood glucose and blood pressure on microvascular complications in patients with type Ⅱ diabetes by comparing the therapeutic effects of intensive and standard treatment in patients with type Ⅱ diabetes. METHODS: A total of 107 patients with type Ⅱ diabetes were randomly assigned into intensive and standard treatment groups. Patients in the intensive treatment group received preterax (perindopril/ indapamide) to control blood pressure, and gliclazide (diamicron) MR to control blood glucose. Patients in the standard treatment group received routine medications or placebo. Urinary microalbumin (UMA), urinary creatinine (UCR), the UMA/ UCR ratio, and visual acuity were monitored according to the study design of the ADVANCE trial. Direct ophthalmoscopy and seven-field stereoscopic retinal photography were used to examine the fundi at baseline,and repeated after 5 years of treatment. RESULTS: The characteristics of patients in both groups were well balanced at baseline. After 5 years of treatment, visual acuity was found to be decreased in the standard group (P=0.04), but remained stable in the intensive group. The severity of diabetic retinopathy had not progressed in patients in the intensive group, but had deteriorated in the standard group (P=0.0006). The UMA/UCR ratio was not obviously changed in patients in the intensive group, whereas it was significantly increased in the standard group (P=0.00). CONCLUSION: Intensive control of blood glucose and blood pressure can decrease the incidence or slow the progression of microvascular complications in patients with type Ⅱ diabetes, and maintain stable vision.

Efficient and fair PPO-based integrated scheduling method for multiple tasks of SATech-01 satellite

SATech-01 is an experimental satellite for space science exploration and on-orbit demonstration of advanced *** satellite is equipped with 16 experimental payloads and supports multiple working modes to meet the observation requirements of various *** to the limitation of platform power supply and data storage systems,proposing reasonable mission planning schemes to improve scientific revenue of the payloads becomes a critical *** this article,we formulate the integrated task scheduling of SATech-01 as a multi-objective optimization problem and propose a novel Fair Integrated Scheduling with Proximal Policy Optimization(FIS-PPO)algorithm to solve *** use multiple decision heads to generate decisions for each task and design the action mask to ensure the schedule meeting the platform *** results show that FIS-PPO could push the capability of the platform to the limit and improve the overall observation efficiency by 31.5%compared to rule-based plans currently ***,fairness is considered in the reward design and our method achieves much better performance in terms of equal task *** of its low computational complexity,our task scheduling algorithm has the potential to be directly deployed on board for real-time task scheduling in future space projects.

Basic properties of trapezoidal channel plasmon polariton subwavelength waveguides

An air channel inside a silver metal film is usually used as a typical Channel Plasmon Polariton(CPP) waveguide. This paper presents research on the basic properties of this waveguide using the Finite Difference Time Domain(FDTD) method. The relationship between transmission power and structure of the waveguide is investigated. The simulation results show that the subwavelength trapezoidal CPP waveguide is superior to the rectangle CPP waveguide for controlling SPP radiation loss and enhancing the transmitted power. As a result, if the rectangle CPP waveguide is replaced by a trapezoidal CPP waveguide in certain integrated optical devices, their performance may be improved.

A novel domain decomposition-based model for efficient dynamic predictions of large composite machine tools

Feedback of Efficient Shock Acceleration on Magnetic-field Structure Inside Young Type Ia Supernova Remnants

Using an effective adiabatic index γ_(eff) to mimic the feedback of efficient shock acceleration,we simulate the temporal evolution of a young type Ia supernova remnant (SNR) with two different background magnetic field(BMF) topologies:a uniform and a turbulent *** density distribution and magnetic-field characteristics of our benchmark SNR are studied with two-dimensional cylindrical magnetohydrodynamic *** γ_(eff)is considered,we find that:(1) the two-shock structure shrinks and the downstream magnetic-field orientation is dominated by the Rayleigh–Taylor instability structures;(2) there exists more quasi-radial magnetic fields inside the shocked region;and (3) inside the intershock region,both the quasi-radial magnetic energy density and the total magnetic energy density are enhanced:in the radial direction,with γ_(eff)=1.1,they are amplified about 10–26 times more than those with γ_(eff)=5/*** in the angular direction,the total magnetic energy densities could be amplified about 350 times more than those with γ_(eff)=5/3,and there are more grid cells within the intershock region where the magnetic energy density is amplified by a factor greater than 100.

Virtual source approach for maximizing resolution in high-penetration gamma-ray imaging

High-energy gamma-ray radiography has exceptional penetration ability and has become an indispensable nondestructive testing(NDT)tool in various *** high-energy photons,point projection radiography is almost the only feasible imaging method,and its spatial resolution is primarily constrained by the size of the gamma-ray *** conventional industrial applications,gamma-ray sources are commonly based on electron beams driven by accelerators,utilizing the process of bremsstrahlung *** size of the gamma-ray source is dependent on the dimensional characteristics of the electron *** research has been conducted on various advanced accelerator technologies that have the potential to greatly improve spatial resolution in *** our investigation of laser-driven gamma-ray sources,a spatial resolution of about 90μm is achieved when the areal density of the penetrated object is 120 g/cm^(2).A virtual source approach is proposed to optimize the size of the gamma-ray source used for imaging,with the aim of maximizing spatial *** this virtual source approach,the gamma ray can be considered as being emitted from a virtual source within the convertor,where the equivalent gamma-ray source size in imaging is much smaller than the actual emission *** the basis of Monte Carlo simulations,we derive a set of evaluation formulas for virtual source scale and gamma-ray emission *** optimal conditions,the virtual source size can be as small as 15μm,which can significantly improve the spatial resolution of high-penetration imaging to less than 50μm.
