An early scoring system to predict mechanical ventilation for botulism: a single-center-based study

BACKGROUND: Early identification of patients requiring ventilator support will be beneficial for the outcomes of botulism. The present study aimed to establish a new scoring system to predict mechanical ventilation(MV) for botulism ***: A single-center retrospective study was conducted to identify risk factors associated with MV in botulism patients from 2007 to 2022. Univariate analysis and multivariate logistic regression analysis were used to screen out risk factors for constructing a prognostic scoring system. The area under the receiver operating characteristic(ROC) curve was ***: A total of 153 patients with botulism(66 males and 87 females, with an average age of 43 years) were included. Of these, 49 patients(32.0%) required MV, including 21(13.7%) with invasive ventilation and 28(18.3%) with non-invasive ventilation. Multivariate analysis revealed that botulinum toxin type, pneumonia, incubation period, degree of hypoxia, and severity of muscle involvement were independent risk factors for MV. These risk factors were incorporated into a multivariate logistic regression analysis to establish a prognostic scoring system. Each risk factor was scored by allocating a weight based on its regression coefficient and rounded to whole numbers for practical utilization([botulinum toxin type A: 1], [pneumonia: 2], [incubation period ≤1 day: 2], [hypoxia <90%: 2], [severity of muscle involvement: grade Ⅱ, 3;gradeⅢ, 7;grade IV, 11]). The scoring system achieved an area under the ROC curve of 0.82(95% CI 0.75–0.89, P<0.001). At the optimal threshold of 9, the scoring system achieved a sensitivity of 83.7% and a specificity of 70.2%.CONCLUSION: Our study identified botulinum toxin type, pneumonia, incubation period, degree of hypoxia, and severity of muscle involvement as independent risk factors for MV in botulism patients. The fiber Bragg grating （FBG） strain sensor is used in the oil and gas derricks for the health monitoring. The system consisted of the FBG strain sensors, optical interrogator and monitoring software. 64 sensors were installed on a derrick for loading and unloading tests. The results showed that the monitoring system had the advantages of the high accuracy, good repeatability, large capacity and real-time monitoring ability.

When the vehicle is in the process of moving,the GNSS signals may be unlocked or lost star which will make GNSS/INS integrated navigation system not reliable enough for *** solve this problem,I put forward a kind of vehicular integrated navigation scheme constructed by GNSS,INS and vehicle kinematics *** integrated navigation strategy depends on GNSS information to determine the condition:when GNSS information is available,it will use GNSS aided inertial navigation system;When the GNSS information is invalid,the kinematics model and constraint condition of the vehicle aided inertial navigation system will start to work;The results of simulation show that this method can provide high precision position information speed when GNSS information is valid,and the position error can be effectively suppressed when GNSS failure,which will ensure the precision,reliability and continuous of navigation information of vehicle navigation system.

The floods caused by the extreme precipitation in the Yangtze River basin(YRB)and Murray–Darling River basin(MDRB),the largest basins in China and Australia,have significant impacts on the society and regional *** on the spatial–temporal analysis of the daily precipitation extremes(DPEs)during 1982–2016,we found that for both basins,the whole-basin-type DPEs have the highest proportion and a synchronous DPE interannual variation characteristic exists in the two basins,with the 3-yr running correlation coefficient of the annual DPE days(DPEDs)reaching almost 0.7(significant at the 0.01 level).The El Ni?o–Southern Oscillation(ENSO),which is one of the most significant climate disturbance factors in the world,plays an important role in modulating the variability of the DPEs in the two *** value decomposition(SVD)analysis revealed that both the YRB and the MDRB’s whole-basin-type DPEs are closely coupled with the procedure that the preceding winter eastern Pacific(EP)-type El Ni?o faded to a central Pacific(CP)-type La *** means that the DPEs in the YRB and MDRB may synchronously occur more frequently when the above process *** to the atmosphere–ocean interaction from the east–west dipole sea surface temperature(SST)anomaly pattern,the atmospheric circulation disturbance exhibits a pattern in which the equatorial eastern Pacific region is a mass source anomaly with a higher pressure,drier air,and weaker convection,while the equatorial western Pacific region is a mass sink anomaly with a lower pressure,wetter air,and stronger ***,two wave trains that originated from the tropical western Pacific were found to extend to the YRB and *** interaction between the wave train’s interphase dynamics and water vapor transport disturbance results in the ascent conditions and enhanced water vapor transport,which leads to the synchronous occurrence of DPEs in the YRB and MDRB on an interannual scale.

Initial allocation modes and schemes are studied in this paper to provide guidelines for allocation of carbon emission permits in power system. We first introduce different allocation modes and the need to apply free allocation to assignability of emission permits. Then, we compare two different allocation schemes, which are based on historical emissions and generation performance standard. Further, a new allocation scheme based on Boltzmann distribution is proposed. Finally, a case study on Shanghai power grid in China is conducted to compare the allocation effects of these schemes respectively. The analytical results show that Boltzmann distribution based scheme has the best performance and should be adopted when developing initial allocation of carbon emission permits in Chinese power systems.
