Background:The supine position is the most common birth position adopted in China,but the World Health Organization recommends non-supine positions for *** handsand-knees position shows several advantages,such as wide pelvic diameter and easy fetal rotation during *** trials conducted in China in 2011 revealed that the handsand-knees position resulted in improved maternal and neonate outcomes than those in the supine ***,a comprehensive study must be conducted before the handsand-knees position can be introduced into clinical ***,we conducted this multicenter trial to comprehensively examine the benefits of the hands-and-knees position over the supine position during ***:Our clinical study was conducted in 11 hospitals in China from May to December 2012.A total of 446 pregnant women who gave birth in the hands-and-knees position were assigned into the experimental group,and 440 women who gave birth in the supine position were classified into the control *** rate was evaluated as the primary outcome,and perineum laceration degree was considered the secondary ***:Women in the experimental group achieved lower rates of episiotomy and higher rates of intact perineum and first-degree perineum lacerations compared with those in the *** bleeding amount,neonatal asphyxia,and APGAR scores at 1 and 5 min were not significantly different between the two ***:This study proves that women who delivered in the hands-and-knees position achieved low rates of episiotomy and intact ***,the rates of neonatal asphyxia and postpartum bleeding did not *** patients who prefer to adopt the hands-and-knees position should be assisted in assuming such position during delivery.

BACKGROUND Endoscopic rubber band ligation(ERBL)is a nonsurgical technique for the treatment of symptomatic internal hemorrhoids but is limited by recurrence and post-procedural *** To evaluate satisfaction,long-term recurrence,and post-procedural pain in managing internal hemorrhoids using a combination of polidocanol foam sclerotherapy and *** This was a prospective,multicenter,randomized study.A total of 195 consecutive patients diagnosed with grade II-III internal hemorrhoids were enrolled from four tertiary hospitals and randomly divided into a cap-assisted endoscopic polidocanol foam sclerobanding(EFSB)or an ERBL *** patients were followed-up for 12 ***-based severity and post-procedural pain were assessed using a hemorrhoid severity score(HSS)and a visual analog scale(VAS).Continuous variables were reported as medians and interquartile *** One hundred and ninety-five patients were enrolled,with 98 in the EFSB *** was lower in the EFSB group than in the ERBL group at 8 weeks[4.0(3.0-5.0)vs 5.0(4.0-6.0),P=0.003]and 12-month[2.0(1.0-3.0)vs 3.0(2.0-3.0),P<0.001]of *** prolapse recurrence rate was lower in the EFSB group at 12 months(11.2%vs 21.6%,P=0.038).Multiple linear regression analysis demonstrated that EFSB treatment[B=-0.915,95%confidence interval(CI):−1.301 to−0.530,P=0.001]and rubber band number(B=0.843,95%CI:0.595-1.092,P<0.001)were negatively and independently associated with the VAS score 24 hours *** median VAS was lower in the EFSB group than in the ERBL[2.0(1.0-3.0)vs 3.0(2.0-4.0),P<0.001].CONCLUSION Cap-assisted EFSB provided long-term satisfaction and effective relief from the recurrence of prolapse and pain 24 hours post-procedure.

Vena cava thrombosis after vena cava filter placement： Incidence and risk factors

Background The objective of this study was to assess the clinical safety and efficacy of vena cava filter （VCF） placement, with particular emphasis on the incidence and risk factors of inferior vena cava thrombosis （VCT） after VCF placement. Methods Clinical data of patients with venous thromboembolism （VTE）, with or without placement of VCF, were analyzed in a retrospective single-center audit of medical records from January 2005 to June 2009. The collected data included demographics, procedural details, filter type, indications, and complications. Results A total of 168 cases of VTE （82 with VCF; 86 without VCF） were examined. Over a median follow-up of 24.2 months, VCT occurred in 18 of 82 patients with VCFs （11 males, 7 females, mean age 55.4 years）. In 86 patients without VCFs, VCT occurred in only 6 individuals （4 males, 2 females） during the study period. VCT was observed more frequently in patients fitted with VCFs than in those without VCFs （22% vs. 7.0%）. Conclusions The incidence of VCT in patients with VTE after VCF implantation was 22% approximately. Anticoagulation therapy should be continued for all patients with VCF placement, unless there is a specific contraindication. Almost all instances of VCT in patients with VCF implants in our study occurred after stopping anticoagulation treatment. The use of VCFs is increasing, and more trials are needed to confirm their benefit and accurately assess their safety.

Nutrient transport following water transfer through the world's largest water diversion channel

Preparation and Characteristics of Hydroxyapatite Whiskers by Homogeneous Precipitation Method

Hydroxyapatite whilkers were prepared by the homogeneousprecipitation method. Soluble calci- um ion and phosphate ion wereused as initial materials, they were refluxed respectively at 85 deg.C, 90 deg. C and 95 deg. C for various lengths of time. A properprecipitation agent was selected to control the releasing speed ofions in the system; it induced the hydroxyapatite crystal to grow indesired way. The pH each solutions were mea- sured continuouslyduring the reaction.

Comparison and Evaluation of Several Dermatophagoides Pteronyssinus Allergen Extracts for Skin Prick Test

Objective To evaluate the significance of several Dermatophagoides pteronyssinus allergen extracts for skin prick test(SPT) in patients allergic to Dermatophagoides *** Two hundred and nineteen patients enrolled in Peking Union Medical College Hospital underwent SPT and serum specific IgE assay to detect the Dermatophagoides pteronyssinus *** kinds of house dust mite allergen extracts were used for SPT,including the Dermatophagoides pteronyssinus extract prepared by our laboratory(group A),standardized Dermatophagoides pteronyssinus extract(group B),and mixed extracts of Dermatophagoides pteronyssinus and Dermatophagoides farinae(group C).Human serum specific IgE result was regarded as the reference standard for diagnosis of Dermatophagoides pteronyssinus *** receiver operating characteristic(ROC) curve was used to evaluate the diagnostic performance of SPT with the extracts of three *** SPT results showed that the median wheal diameter of group A,group B,and group C was 0.43,0.35,and 0.28 cm,respectively,with significant difference among three groups(P0.05).There was no local urticaria or systemic allergic reactions following the procedure of *** reaction was observed in 5 patients and delayed reaction was in 2 patients of group *** for group B and C,local reaction occurred in 3 cases and delayed reaction in 2 cases in each *** area under ROC curve of SPT with extract in group A,group B,and group C was 0.765,0.801,and 0.782,*** on the detection results of serum specific IgE,the sensitivity of SPT in diagnosis of Dermato-phagoides pteronyssinus allergy with extract of group A,group B,and group C was 92.4%,87.0%,and 81.5%,and the specificity was 60.6%,73.2%,and 74.8%,*** The Dermatophagoides pteronyssinus extract for SPT prepared by our labor
