BACKGROUND:the accelerated diagnostic protocol(ADP)using the Emergency Department Assessment of Chest pain Score(EDACS-ADP),a tool to identify patients at low risk of a major adverse cardiac event(MACE)among patients presenting with chest pain to the emergency department,was developed using a contemporary troponin *** study was performed to validate and compare the performance of the EDACS-ADP incorporating high-sensitivity cardiac troponin I between patients who had a 30-day MACE with and without unstable angina(MACE I and II,respectively).MEtHODS:A single-center prospective observational study of adult patients presenting with chest pain suggestive of acute coronary syndrome was *** performance of EDACS-ADP in predicting MACE was assessed by calculating the sensitivity and negative predictive ***:Of the 1,304 patients prospectively enrolled,399(30.6%;95%confidence interval[95%CI]:27.7%–33.8%)were considered low-risk using the *** them,the rates of MACE I and II were 1.3%(5/399)and 1.0%(4/399),*** EDACS-ADP showed sensitivities and negative predictive values of 98.8%(95%CI:97.2%–99.6%)and 98.7%(95%CI:97.0%–99.5%)for MACE I and 98.7%(95%CI:96.8%–99.7%)and 99.0%(95%CI:97.4%–99.6%)for MACE II,***:EDACS-ADP could help identify patients as safe for early ***,when unstable angina was added to the outcome,the 30-day MACE rate among the designated lowrisk patients remained above the level acceptable for early discharge without further evaluation.

BACKGROUND Currently,there is no standard adjuvant therapy for patients with resected ampulla of Vater(AoV)*** to evaluate the effectiveness of adjuvant concurrent chemoradiotherapy(CCRt)in patients with advanced AoV cancer who underwent curative *** this single-centered,retrospective study included 29 patients with advanced AoV cancer who underwent pancreaticoduodenectomy between 2006 and *** impact of CCRt on advanced AoV cancer was *** the 1-,3-,and 5-yr recurrence-free survival(RFS)rates for patients with advanced AoV cancer were 82.8%,48.3%,and 40.8%,respectively,and the overall survival(OS)rates were 89.7%,62.1%,and 51.7%,***-cular invasion was found to be a significant risk factor for RFS and OS in patients with advanced AoV cancer in the univariate analysis,whereas t stage and lymph node metastasis were significantly associated with OS in the multivariate *** to the patients who did not receive adjuvant CCRt,those who received adjuvant CCRt did not show statistically significant improvements in the RFS and OS,although they had a significantly lower average age and significantly higher platelet-to-lymphocyte *** Adjuvant CCRt did not improve survival outcomes in patients with advanced AoV *** findings contribute to existing knowledge on the effectiveness of CCRt in this patient population and provide important insights for clinical decision-making.

As location data are widely available to portable devices, trajectory tracking of moving objects has become an essential technology for most location-based services. to maintain such streaming data of location updates from mobile clients, conventional approaches such as time-based regular location updating and distance-based location updating have been used. However, these methods suffer from the large amount of data, redundant location updates, and large trajectory estimation errors due to the varying speed of moving objects. In this paper, we propose a simple but efficient online trajectory data reduction method for portable devices. to solve the problems of redundancy and large estimation errors, the proposed algorithm computes trajectory errors and finds a recent location update that should be sent to the server to satisfy the user requirements. We evaluate the proposed algorithm with real GPS trajectory data consisting of 17 201 trajectories. the intensive simulation results prove that the proposed algorithm always meets the given user requirements and exhibits a data reduction ratio of greater than 87% when the acceptable trajectory error is greater than or equal to 10 meters.

the sliding wear behaviors of ZrO2-22 wt pct MgO （MZ） and ZrO2-8 wt pct Y2O3（YZ） coatings deposited on a cast aluminum alloy with bond layer （NiCrCoAlY） by plasma spray were investigated under dry test conditions at room temperature. Under all load conditions, the wear mechanisms of the MZ and YZ coatings were almost the same. the material transfer and pullout were involved in the wear process of the studied coatings under the test conditions. the wear rate of the MZ coating was less than that of the YZ coating. While increasing the normal load, the wear rates of the MZ and YZ coatings increased. SEM was used to examine the worn surfaces and to elucidate likely wear mechanisms. Energy dispersive X-ray spectroscopy （EDX） analysis of the worn surfaces indicated that material transfer occurred in the direction from the SiC ball to the disk. Fracture toughness had a significant influence on the wear performance of the coatings. It was suggested that the material transfer played an important role in the wear behavior.

the aim of this paper is to present an analytical expression for the streamwise velocity distribution in a non-uniform flow in the presence of waves; the correlation between the horizontal and vertical velocity components has been compreheusively examined. Different from previous researches which attributed the deviation of velocity from the classical log-law to the wave Reynolds stress, i.e. - ρ uv^- only, this study demonstrates that the momentum flux caused by mean velocities, i.e., u^- and v^-, is also responsible for the velocity deviation, and it is found that the streamwise velocity for a flow in the presence of non-zero wall-normal velocity does not follow the classical log-law, but the modified log-law proposed in this study based on simplified mixing-length theorem. the validity of the modified log-law has been verified by use of available experimental data from published sources for combined wave-current flows, and good agreement between the predicted and observed velocity profiles has been achieved.

BACKGROUND:Patients with suspected acute coronary syndrome (ACS) in whom myocardial infarction has been ruled out are still at risk of having obstructive coronary artery disease (CAD).this rate is higher among patients with intermediate high-sensitivity troponin I (hstnI) concentrations (5 ng/L to 99th percentile) than low concentrations (therefore,an intermediate concentration has been suggested as a candidate for downstream investigation with computed tomography coronary angiography(CtCA).We tried to compare the HEARt score-guided ***-guided approach for identifying obstructive ***:From a prospective cohort study of patients presenting to the emergency department with suspected ACS,433 patients without elevated hstnI who also underwent CtCA were selected and *** performances of hstnI concentration and HEARt score were compared using sensitivity,specificity,positive predictive value (PPV),and negative predictive value (NPV).RESULtS:Overall,120 (27.7%) patients had obstructive *** with intermediate hstnI concentrations were more likely to have obstructive CAD than those with low hstnI concentrations(40.0%vs.18.1%);patients with non-low-risk HEARt scores (≥4 points) were also more likely to have obstructive CAD than those with low-risk scores (0 to 3 points)(41.0%vs.7.6%).the HEARt score had higher sensitivity and NPV for detecting obstructive CAD in each classification than hstnI concentration (sensitivity:89.2%vs.63.3%;NPV:92.4%vs.81.9%,respectively).CONCLUSION:After excluding myocardial infarction in patients with suspected ACS,adding the HEARt score for selecting candidates for CtCA could improve patient risk stratification more accurately than relying on hstnI concentration.

Carbon fiber reinforced plastic (CFRP) of the advanced composite materials is widely used in lightmass structural materials of air planes, ship and automobiles because of high strength and stiffness. In this study, experimental investigation was performed for each specimen. the square section members consist of aluminum, CFRP and hybrid (aluminum/CFRP) member, and hat-shaped section members consist of CFRP and hybrid (aluminum/CFRP) members specimen. Based on the collapse characteristics of aluminum square section member, the collapse characteristics and energy absorption capability of hat-shaped section members were analyzed. the axial static collapse tests were carried out for each section member. the collapse modes and the energy absorption capability of the members were analyzed. In the lightmass design aspect, the collapse characteristics and energy absorption capability of the members were compared.

this paper re-examines Bagnold’s theorem of sediment transport. Different from the stream power defined by Bagnold, i.e., τoV, this study shows that the total load of sediment-laden flow, gt, is related to near bed energy dissipation rate, i.e., τou*’. An attempt is made to explain the empirical relation. the ratio (=k) of measured total load, gt, to the product of near bed velocity, u*’, and energy dissipation rate, τou*’, is analyzed. It is found that k is independent of the Rouse number, Z, if Z is greater than 2.6, but the coefficient k decreases with the increase of Z when Z tion between k and Z is developed. A systematic and thorough analysis of 1,458 sets of data collected from 16 rivers and canals confirms that the proposed formula can be used to compute the total load with accuracy.

Background Sarcopenia is a risk factor for metabolic disorders and cardiovascular disease, but the association between sarcopenia and cardiovascular risk factors according to age and obesity status in the general population remains unknown. We thus investigated these associations in the Korean population. Methods We included 8,958 and 8,518 subjects from the fifth Korean National Health and Nutrition Examination Survey （KNHANES）（from 2010 and 2011, respectively）. the study was restricted to participants 〉 20 years old who had completed the health examination survey, including whole body dual-energy X-ray absorptiometry scans. After exclusion, 7,366 subjects （3,188 men, 4,178 women） were included in our final analysis. Age was categorized according to three age groups （20-39, 40-59, mad 〉 60 years）, and subjects were catego- rized according to their sarcopenic and obesity status. Cardiovascular risk was assessed with Framingham risk score （FRS）. Results the sarco- penic obese group had a higher FRS than the non-sarcopenic obese group, which had a higher FRS than the non-sarcopenic non-obese group. Age-wise, the 20-39 year-old group showed a non-significant association between sarcopenia and FRS. In the 40-59 year-old group, regardless of obesity status, sarcopenic subjects had a higher FRS than non-sarcopenic subjects. In the 〉 60 year-old group, sarcopenic subjects had a higher FRS than non-sarcopenic subjects for the non-obese group. Conclusions Sarcopenia was associated with cardiovascular disease and may be an early predictor of its susceptibility in both elderly and middle-aged subjects. thus, management of sarcopenia is necessary to prevent cardiovascu- lar disease.
