Self-expandable metal stents for choledocholithiasis in Billroth II gastrectomy patients

Background: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents(FCSEMSs) for the management of common bile duct(CBD) stones in a subset of patients with a history of Billroth II gastrectomy. Methods: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD stones. The factors associated with FCSEMSs placement, treatment success, and procedural complications were analyzed. Results: A group of 15 patients(10 males; median age, 78 years) underwent biliary metal stent placement for high degree of CBD angulation(6), small or flat papilla with unclear margin(5), current use of double antiplatelet agents or an anticoagulant(2), unwanted instrumentation of the cystic duct(1), and insecure position of the scope(1). Ampullary intervention with FCSEMSs was successful in all patients. After dilating the ampulla of Vater and building a durable conduit with FCSEMSs immediately, CBD stones were removed successfully from all patients in a single session. A mild post-ERCP pancreatitis occurred in one patient, who recovered without complications. Conclusion: Ampullary intervention with FCSEMSs is safe and effective for the management of CBD stones in a subset of patients with a history of Billroth II gastrectomy.

Recently,there has been a sudden shift from using traditional office applications to the collaborative cloud-based office suite such as Microsoft Office *** cloud-based systems allow users to work together on the same docu-ment stored in a cloud server at once,by which users can effectively collaborate with each ***,there are security concerns unsolved in using cloud *** of the major concerns is the security of data stored in cloud ser-vers,which comes from the fact that data that multiple users are working together cannot be stored in encrypted form because of the dynamic characteristic of cloud *** this paper,we propose a novel mode of operation,DL-ECB,for AES by which we can modify,insert,and delete the ciphertext based on changes in ***,we can use encrypted data in collaborative cloud-based *** demonstrate that the DL-ECB mode can preserve the confidential-ity,integrity,and auditability of data used in collaborative cloud systems from adversaries,we implement and evaluate the prototype of the DL-ECB mode.

Thoracic pyogenic infectious spondylitis presented as pneumothorax:A case report

BACKGROUND Pyogenic infectious spondylitis(PIS)is a rare condition,with an incidence between 0.2 and 2 cases per 100000 per ***’s most common symptom-back or neck pain-occurs in more than 90%of ***,we reported a case of thoracic PIS accompanied by pneumothorax in a 65-year-old male *** SUMMARY A 65-year-old man presented with right chest pain and *** initial erect posteroanterior chest radiography revealed pneumothorax,which was further evaluated by chest computed tomography,revealing pleural effusion in the right lung and a paravertebral abscess with bony destruction of vertebral *** on magnetic resonance imaging,the patient was diagnosed with thoracic infectious spondylitis with an anterior paravertebral *** was prescribed antibiotics and underwent neurosurgery due to aggravated symptoms and neurologic *** examination revealed that the cause of pleural effusion and pneumothorax was Staphylococcus aureus infection contiguously spread to lung *** several surgical treatments with intravenous antibiotic therapy for two months and transition to oral antibiotics(rifampin 600 mg qd and ciprofloxacin 500 mg bid),the patient received physical therapy to recover *** month after discharge,the patient had no chest pain or dyspnea,and exhibited no elevation in inflammatory markers or new thoracic *** To our knowledge,this is the very first report of a case of thoracic PIS with pneumothorax.

The association between vitamin D levels and burn factors in different burn types

Background:Patients with burns present with different clinical features depending on the types of burn injury and burn patients with lower levels of vitamin D have worse prognoses and more *** study aims to investigate the association between vitamin D levels and burn factors according to each burn type in relation to early intensive rehabilitation therapy initiated for inpatients with ***:In this retrospective study,we enrolled 757 of 1716 inpatients who underwent rehabilitative therapy between May 2013 and April *** types were divided into flame burn,electrical burn and other burns,including scalding,contact and chemical ***,burned body surface area(BSA),wound healing time(WHT),length of hospital stay(LOS)and body mass index were analysed between vitamin D deficient and non-deficient patient groups using Student’s ttests,or Mann-Whitney U test and among three burn types using one-way analysis of variance(ANOVA)or Kruskal-Wallis one-way *** relationship between vitamin D levels and burn factors was evaluated using Pearson’s or Spearman’s correlation coefficient tests,and multiple linear regression analysis in different burn ***:In total,88.9%patients were vitamin D deficient,and these patients had a larger burned BSA(p=0.015)and longer WHT and LOS(all p<0.001)than non-deficient *** BSA,WHT and vitamin D levels showed significant differences in their mean values according to three burn types(all p<0.001).WHT was a communal factor significantly associated with vitamin D levels in all three burn types(p<0.05).The WHT cut-off points to predict vitamin D deficiency were 55 days for flame burn(p<0.001)and 62.5 days for electrical burn(p=0.001).Conclusions:WHTacross all three burn typeswas a common factor associated with vitamin D levels for inpatients with burns who had undergone rehabilitative *** burn patients with vitamin D deficiency,even those with a low burned BSA percentage,showed prol

Objective:To explore the possible neuroprotective activities of Humulus japonicus extract against Parkinson's disease(PD)in a cellular ***:PD was modeled in PC12 cells using 6-hydroxydopamine(6-OHDA).The cell activity,intracellular levels of reactive oxygen species(ROS),anti-oxidative and anti-apoptotic effects,and other related indicators and related signaling pathways were evaluated to elucidate the neuroprotective effects of Humulus japonicus ***:Humulus japonicus extract exhibited anti-oxidative and anti-apoptotic effects in 6-OHDA-stimulated PC12 *** also reduced oxidative stress-induced ROS accumulation;upregulated antioxidant enzymes,such as glutathione,catalase,heme oxidase-1,and 8-oxguanine glycosylase 1;promoted cell survival by decreasing BAX and increasing Bcl-2 and sirtuin 1 expression via the MAPK and/or Nrf2 signaling ***:Humulus japonicus extract has antioxidative and anti-apoptotic effects and could be developed as a promising candidate for preventing and treating oxidative stress-related neurodegenerative diseases.

Structurally engineered colloidal quantum dot phosphor using TiO_(2) photonic crystal backbone

Photonic crystal(PhC)phosphor,in which the phosphor material is periodically modulated for an enhancement in color-conversion efficiency via resonant absorption of excitation photons,is a paradigm-shifting structural phosphor ***-dimensional(2D)square-lattice PhC phosphor is currently considered the most advanced platform because of not only its high efficiency,but also its immunity to excitation *** the present study,two major modifications are made to further improve the performance of the 2D PhC phosphor:increasing the refractive index contrast and planarizing the *** index contrast is improved by replacing the PhC backbone material with TiO_(2)whereas the surface planarization is achieved by removing excessive colloidal quantum dots from the *** comparison with the reference phosphor,the upgraded PhC phosphor exhibits~59 times enhanced absorption(in simulations)and~7 times enhanced emission(in experiments),both of which are unprecedentedly *** results not only brighten the viability and applicability of the PhC phosphor but also spur the phosphor development through structural engineering of phosphor materials.
