Non-orthogonal multiple access(NOMA) is a new access method to achieve high performance gains in terms of capacity and throughput, so it is currently under consideration as one of the candidates for fifth generation(5 G) technologies. NOMA utilizes power domain in order to superimpose signals of multiple users in a single transmitted signal. This creates a lot of interference at the receive side. Although the use of successive interference cancellation(SIC) technique reduces the interference, but to further improve the receiver performance, in this paper, we have proposed a joint Walsh-Hadamard transform(WHT) and NOMA approach for achieving better performance gains than the conventional NOMA. WHT is a well-known code used in communication systems and is used as an orthogonal variable spreading factor(OVSF) in communication systems. Application of WHT to NOMA results in low bit error rate(BER) and high throughput performance for both low and high channel gain users. Further, it also reduces peak to average power ratio(PAPR) of the user signal. The results are discussed in terms of comparison between the conventionalNOMA and the proposed technique, which shows that it offers high performance gains in terms of low BER at different SNR levels, reduced PAPR, high user throughput performance and better spectral efficiency.

BACKGROUND Spontaneous pneumoperitoneum(SP) without gastrointestinal perforation rarely occurs in neonates, with most SP cases being idiopathic. Although SP usually follows a benign clinical course with favorable prognosis, it can become lifethreatening in certain situations. In these cases, urgent surgical intervention may be required. Therefore, it may be difficult to decide when or how to perform prompt *** To demonstrate the distinct clinical features of SP to guide appropriate management by comparing characteristics between SP and typical pneumoperitoneum secondary to gastrointestinal *** We retrospectively reviewed electronic medical records and identified 37 neonates with radiological evidence of pneumoperitoneum who were treated at our institution. Clinical variables were compared between neonates with SP without gastrointestinal perforation(Group A) and those with pneumoperitoneum secondary to gastrointestinal perforation(Group B). Clinical variables between groups were compared using Student’s t-test and the chi-square test. The risk factors related to mortality were examined using multi-logistic regression *** Group A comprised 35.1%(13/37) of the patients. The frequency of persistent pulmonary hypertension(53.8%) and pneumothorax(46.2%) before the development of pneumoperitoneum was significantly higher in group A than in group B(P =0.004). Platelet count and partial pressure of arterial oxygen(PaO) were significantly lower in group A(P = 0.015 and 0.025, respectively). Recently, it has been suggested that the guideline for adjuvant radiotherapy （ART） following radical prostatectomy （RP） sponsored by the American Urological Association and American Society for Radiation Oncology （AUA/ASTRO） may result in a significant overtreatment. Thus, the objective of the present study was to refine the AUA/ASTRO guideline for ART in patients at risk for biochemical recurrence （BCR） after RP. To this end, we reviewed our prospectively maintained database and selected 193 patients who met the AUA/ASTRO ART criteria. With a median follow-up of 24.0 months, BCR rate was 17.6% （34/193）. When stratified by the Gleason score, BCR rate in men with Gleason score 6 was 6.8%. There was no significant association between BCR-free survival and surgical margin （P= 0.690） and pathologic stage （P= 0.353） in patients with the Gleason score 6. However, in patients with positive surgical margins （PSMs）/pathologic stage ≥T3, there was a significant difference in BCR-free survival according to Gleason score （≤7 vs8-10, P= 0.047）. Multivariate Cox regression analysis demonstrated that pathologic stage ≥T3 （HR = 2.106; P = 0.018）, PSMs （HR = 2.411; P = 0.003）, and pathologic Gleason score 8-10 （HR = 4.715; P 〈 0.001） were independent predictors of BCR after RP. Therefore, in addition to pathologic stage 〉T3 and PSMs, Gleason score 8-10 predicts 8CR after RP. In patients with Gleason score 6, observation rather than ART may be more appropriate regardless of stage and surgical margin status.

Air in the main pancreatic duct:A case of innocent air

Air in the main pancreatic duct has been reported only rarely and might be associated with either a spontane- ous or a surgically induced alteration of the anatomy of the biliary tract. We report a case of ＂innocent＂ air found incidentally in the main pancreatic duct. To our knowledge, this is only the third such case reported. A 54-year-old woman presented with hemoptysis that had lasted for 3 d. She underwent a chest computed tomography scan, which revealed not only focal bron- chiectasis in the left lower lobe, but also air in the main pancreatic duct and dilatation of the common bile duct. She was managed conservatively for the hemop- blsis and no further problems developed. She had no specific gastrointestinal symptoms and had no history of surgery or medication. Her laboratory parameters were normal. Magnetic resonance cholangiopancrea- tography also demonstrated air in the main pancreatic duct and a dilated common bile duct （CBD）. Duode- noscopy revealed separate biliary and pancreatic ori-rices with patulous openings and some air bubbles ap- pearing in the pancreatic orifice. Endoscopic retrograde cholangiopancreatography （ERCP） showed the dilated CBD and pancreatic duct with some air bubbles, but no other abnormal lesions. She was discharged with no further problems. Most patients with air in the main pancreatic duct have had a pancreatobiliary disease, or a history of pancreatobiliary disease, pancreatobiliary surgery or sphincterotomy. If the air is innocent, as in our case, ERCP should be performed to evaluate any altered sphincteric function or anatomy such as patu- Ious openings.

OBJECTIVE: To investigate the efficacy of horse oil on lipopolysaccharide(LPS)-induced inflammation in human ***: Western blot analysis was performed to measure the expression of cyclooxygenase-2(COX-2) and IκBα. ELISA was used to analyze prostaglandin E2(PGE2) ***: Horse oil decreased LPS-induced COX-2 and PGE2 levels in a dose-dependent manner. Nuclear factor-kappa B(NF-κB) plays a key role in the expression of inflammatory cytokines and mediators. Therefore, we investigated the influence of horse oil on the NF-κB signaling pathways. Horse oil inhibited translocation of NF-κB from the cytosol to the nucleus. Furthermore, LPS-induced degradation of IκBα was recovered by horse oil. The activation of p38 mitogen-activated protein kinase(MAPK) reportedly induces degradation of IκBα In agreement with this, LPS activated p38 MAPK and caused IκBα degradation. Conversely, horse oil inhibited LPS-induced p38 MAPK activation and IκBαdegradation. In addition, a specific p38 MAPK inhibitor, SB203580, blocked IκBα ***: Horse oil decreased COX-2 and PGE2 by inhibiting p38 MAPK activation, IκBα degradation, and the translocation of NF-κB.

This study aims to create a basis for sustainable conservation by identifying the heritage value of 166 *** on old land registers and cadastral maps of the 1920s and 1930s,it interpreted the construction procedure,social background,and urban architectural characteristics of the road and plot subdivision of the modern Hanok ***-dong Hanok produced four types of ground plans based on the grid-street *** study argues that these innovative experiments have already been attempted through Bak young-hyo’s proposals in his Geonbaekseo and the architectural activities of Jeong Segwon,and that the idea of applying various housing types,layouts,and grid-street system planning was embodied through the development of the Hanok complex in ***,this attempt greatly influenced the design of other Hanok complexes built in colonial Gyeongseong after 1936,taking the Ikseon-dong Hanok as a role *** radiation of new perspectives will be a groundwork for the future sustainable conservation of heritage places.
