AIM： To study the prevalence and risk factors associated with triple infection with human immunodeficiency virus （HIV）/hepatitis B virus （HBV）/hepatitis C virus （HCV） in an urban clinic population. METHODS： Retrospective chart review of 5639 patients followed at St. Luke＇s-Roosevelt Hospital HIV Clinic （Center for Comprehensive Care） in New York City, USA from January 1999 to May 2007. The following demographic characteristics were analyzed： age, sex, race and HIV risk factors. A multiple logistic regression analysis was performed to evaluate the influence of demographic factors on acquisition of these viruses. RESULTS： HIV/HBV, HIV/HCV and HIV/HBV/HCV infections were detected in 252/5639 （4.47%）, 1411/5639 （25.02%） and 89/5639 （1.58%） patients, respectively. HIV/HBV co-infections were associated with male gender （OR 1.711; P = 0.005）, black race （OR 2.091, P 〈 0.001）, men having sex with men （MSM）（OR 1.747, P = 0.001）, intravenous drug use （IDU）（OR 0.114, P 〈 0.001）, IDU and heterosexual activity （OR 0.247; P = 0.018）, or unknown （OR 1.984, P = 0.004）.HIV/HCV co-infections were associated with male gender （OR 1.241; P = 0.011）, black race （OR 0.788; P = 0.036）, MSM （OR 0.565; P 〈 0.001）, IDU （OR 8.956; P 〈 0.001）, IDU and heterosexual activity （OR 9.106; P 〈 0.001）, IDU and MSM （OR 9.179; P 〈 0.001）, or transfusion （OR 3.224; P 〈 0.001）. HIV/HBV/HCV coinfections were associated with male gender （OR 2.156; P = 0.015）, IDU （OR 6.345; P 〈 0.001）, IDU and heterosexual activity （OR 9.731; P 〈 0.001）, IDU and MSM （OR 9.228; P 〈 0.001）, or unknown （OR 4.219; P = 0.007）. CONCLUSION： Our study demonstrates that coinfection with HBV/HCV/HIV is significantly associated with IDU. These results highlight the need to intensify education and optimal models of integrated care, particularly for populations with IDU, to reduce the risk of viral transmission.

AIM:To study factors associated with loss of hepatitis B surface antigen (HBsAg) in patients co-infected with human immunodeficiency virus (HIV) and hepatitis B virus (HBV).METHODS:We retrospectively reviewed the medical records of 5681 patients followed up at two New York City HIV clinics from January 1999 to May *** and laboratory parameters including baseline and follow-up HIV viral loads,CD4 cell counts,alanine transaminase levels,demographics,presence of hepatitis C infection,and treatment with highly active antiretroviral therapy dually active against both HIV and HBV infection,were analyzed to determine factors associated with loss of ***:Three hundred and fifty five patients (355/5681,6.84%) were co-infected with HIV and HBV and were *** these,226 patients with more than 12 mo follow-up were included in further analysis to determine factors associated with loss of HBsAg in the long-term *** the univariate analysis,baseline CD4 cell count was associated with loss of HBsAg (P=0.052).Cox regression analysis revealed that loss of HBsAg was associated with baseline CD4 cell count > 500 cells/mm3 (P=0.016,odds ratio:76.174,95% confidence interval:2.233-2598.481).CONCLUSION:Our study showed an interesting association of loss of HBsAg in HIV-HBV co-infected patients with higher CD4 cell count,suggesting that T-cell cytolytic activity against HBV may still be effective in clearing HBV infection.

Objective:To investigate the anti-obesity activity and the action mechanism of the roots of Adenophora triphylla *** extract(ATE) in high-fat diet(HFD)-induced obese mice and 3T3-LI ***:The roots of Ademption triphylla were extracted with 70%*** demonstrate the compounds,linoleic acid was analyzed by using gas chromatography:and the anti-obesity effects and possible mechanisms of ATE were examined in 3T3-L1 adipocytes and HFD-induced obese ***:Treatment with ATE inhibited the lipid accumulation without cytotoxicity in 3T3-L1 ***,200 and 400 mg/kg ATE treatment significantly decreased the body weight gain,white adipose tissues(WATs) weight and plasma triglyceride level,while 100 and 200 mg/kg ATE treatment increased the plasma high-density lipoprotein cholesterol level in the HFD-induced obese mice,as compared with the HFD *** with 200 and 400 mg/kg ATE also lowered the size of adipocytes in adipose tissue and reduced the lipid accumulation in *** treatment showed significantly lower expression level of adipogenesis-relatcd proteins,such as peroxisome proliferator-activated receptor y,fatty acid binding protein(aP2),fatty acid synthase in 3T3-L1 adipocytes;and furthermore,decreased peroxisome proliferatoractivated receptor γ,aP2,fatty acid synthase,sterol regulatory element binding protein-1c,and lipoprotein lipase mRNA expression levels in WAT of the HFD-induced obese ***:These results suggested that the ATE has an anti-obesity effect,which may be elicited by regulating the expression of adipogenesis and lipogenesis-related genes and proteins in adipocytes and WAT of the HFD-induced obese mice.

Three types of semi-cry stalline photovoltaic polymers were synthesized by incorporating a siloxane-terminated organic/inorganic hybrid side-chain and changing the number of fluorine substituents.A branch point away from a polymer main backbone in the siloxane-containing side-chains and the intra-and/or interchain noncovalent coulombic interactions enhance a chain planarity and facile interchain *** resulting polymers formed strongly agglomerated films with high roughness,suggesting strong intermolecular *** optical band gap of ca.1.7 eV was measured for all polymers with a pronounced shoulder peak due to tight π-π *** increasing the fluorine substituents,the frontier energy levels decreased and preferential face-on orientation was *** siloxane-terminated side-chains and fluorine substitution promoted the intermolecular packing,showing well resolved lamellar scatterings up to(300) for this series of polymers in the grazing incidence wide angle X-ray scattering *** PPsiDTBT,PPsiDTFBT and PPsiDT2 FBT devices showed a power conversion efficiency of 3.16%,4.40%and 5.65%,respectively,by blending with PC_(71)***-type bimolecular charge recombination was similar for three polymeric solar *** main loss in the photocurrent generation for PPsiDTBT:PC_(71)BM was interpreted to originate from the trap assisted charge recombination by measuring light-intensity dependent short-circuit current density(J_(SC)) and open-circuit voltage(V_(Oc)).Our results provide a new insight into the rational selection of solubilizing substituents for optimizing crystalline interchain packing with appropriate miscibility with PC71 BM for further optimizing polymer solar cells.

AIM： To evaluate the clinical significance of preand intra-operative colonoscopy for the detection of synchronous lesions in colon cancer.METHODS： Two hundred and sixty-five pre-operative and 51 intra-operative colonoscopic evaluations were performed in 316 colorectal cancer patients who underwent curative resection from January 2001 to June 2006. The incidence and characteristics of synchronous lesions and their influence on surgery were evaluated.RESULTS： Two hundred and eighty-two synchronous lesions were detected in 124 （39.2%） of 316 patients including all lesions regardless of their histologic type. True adenomatous polyps were found in 91 （28.8%） of 326 patients, and 27 （5.4% of all patients） patients had synchronous colon cancers. The preoperative identification of synchronous lesions altered the planned surgery in 37 （14.0%） of 265 patients. In 18 patients among the surgically removed cases, the lesions were removed by extending the resection range. Further segmental resection or polypectomy through enterotomy was necessary in 29 patients. Nineteen （37.2%） of 52 intraoperative colonoscopy cases had synchronous lesions. Additional surgical procedures including segmental bowel resection and polypectomy with enterotomy were necessary in 7 （23.7%） of 52 intraoperative colonoscopy cases to remove the lesions.CONCLUSION： Synchronous colorectal polyps or cancer are frequent and their preoperative detection is important for optimal surgical planning and treatment. Intraoperative colonoscopy is a useful option in cases where a preoperative colonoscopy is not feasible.

Over the last 40 years, it has been shown at the global level that sustainable forestry can be achieved through comprehensive forest management,with the decentralized institutional arrangements of community-managed forestry coordinated by effective policy implementation. However, there is still a shortage of evidence regarding whether communitybased forestry is well characterized by forest policies,assessing what action is most needed and how best to address the challenges faced by community-based forestry in halting deforestation and promoting rural livelihoods. The study analyzed experts' assessments of the characteristics and success of communitymanaged forestry in Cambodia and explored three case studies of community-managed forestry practice to identify priorities for addressing forest policy implementation inadequacies in halting deforestation and promoting rural livelihoods. There were two methods of data collection. Firstly, this study used a survey of 27 experts to analyze perceptions about how far forest policy supported community-managed forestry effectively, the major challenges faced by the national community-managed forestry program, and the community-managed forestry contribution to halting deforestation and reducing rural ***, data was collected by content analysis of three case studies to explore the knowledge and practical experience of local experts about community-managed forestry practice at local *** study employed Kendall's Coefficient of Concordance to analyze the level of concordance of experts on related forest policies(n=15) considering community-managed forestry, the challenges faced by the national community-managed forestry program,and the actions required to enable communitymanaged forestry to support communities. Analysis revealed that experts were in moderate agreement,denoted by Kendall's W=0.152, on how well forest policies articulate and implement the characteristics of community-managed forestry. A femoral artery pseudoaneurysm is one complication of vascular intervention, and the incidence is increasing. Early management is then needed to avoid potential dangers from it. It differs from a true aneurysm in that it doesn't include any component of the vascular wall, and is not studied as much as a true aneurysm. Here, a model of a femoral pseudoaneurysm was made and a Computational Fluid Dynamics(CFD) simulation was verified with PIV experiment. Afterwards, a CFD simulation with two different models was performed to look for any findings which may help in developing new treatment methods.

Changes in cellular isoflavone （daidzein and genistein） contents were monitored in root cultures of Albizzia kalkora （Roxb.） Prain after feeding different ratios of NH4^＋/NO3^- and treatment with a biotic elicitor （three strains of Rhizobium sp.）. The NH4^＋/NO3^- ratio appears to be positively correlated with daidzein content in the roots and shows a negative correlation with genistein. Among the three different strains of Rhizobium used, the strain ATCC 15834 caused a 35% increase in daidzein production by infection. In the case of genistein, maximum production （94%） was obtained when cultures were treated on Day 6 by the strains ATCC 15834 and KCTC 1541. The biosynthetic pathway of the two isoflavones apparently reacts differently to the same culture conditions and the same strains of Rhizobium. Therefore, the present data suggest that the production of daidzein and genistein could be modulated by changing the NH4^＋/NO3^- ratio and the application of Rhizobium.
