Preparation of human decellularized peripheral nerve allograft using amphoteric detergent and nuclease

Animal studies have shown that amphoteric detergent and nuclease(DNase I and ribonuclease A) is the most reliable decellularization method of the peripheral nerve. However, the optimal combination of chemical reagents for decellularization of human nerve allograft needs further investigation. To find the optimal protocol to remove the immunogenic cellular components of the nerve tissue and preserve the basal lamina and extracellular matrix and whether the optimal protocol can be applied to larger-diameter human peripheral nerves, in this study, we decellularized the median and sural nerves from the cadavers with two different methods: nonionic and anionic detergents(Triton X-100 and sodium deoxycholate) and amphoteric detergent and nuclease(3-[(3-cholamidopropyl)dimethylammonio]-1-propanesulfonate(CHAPS), deoxyribonuclease I, and ribonuclease A). All cellular components were successfully removed from the median and sural nerves by amphoteric detergent and nuclease. Not all cellular components were removed from the median nerve by nonionic and anionic detergent. Both median and sural nerves treated with amphoteric detergent and nuclease maintained a completely intact extracellular matrix. Treatment with nonionic and anionic detergent decreased collagen content in both median and sural nerves, while the amphoteric detergent and nuclease treatment did not reduce collagen content. In addition, a contact cytotoxicity assay revealed that the nerves decellularized by amphoteric detergent and nuclease was biocompatible. Strength failure testing demonstrated that the biomechanical properties of nerves decellularized with amphoteric detergent and nuclease were comparable to those of fresh controls. Decellularization with amphoteric detergent and nuclease better remove cellular components and better preserve extracellular matrix than decellularization with nonionic and anionic detergents, even in large-diameter human peripheral nerves. Prevalence and risk factors of wound complications after transtibial amputation in patients with diabetic foot

Self-expandable metal stents for choledocholithiasis in Billroth II gastrectomy patients

Background: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents(FCSEMSs) for the management of common bile duct(CBD) stones in a subset of patients with a history of Billroth II gastrectomy. Methods: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD stones. The factors associated with FCSEMSs placement, treatment success, and procedural complications were analyzed. Results: A group of 15 patients(10 males; median age, 78 years) underwent biliary metal stent placement for high degree of CBD angulation(6), small or flat papilla with unclear margin(5), current use of double antiplatelet agents or an anticoagulant(2), unwanted instrumentation of the cystic duct(1), and insecure position of the scope(1). Ampullary intervention with FCSEMSs was successful in all patients. After dilating the ampulla of Vater and building a durable conduit with FCSEMSs immediately, CBD stones were removed successfully from all patients in a single session. A mild post-ERCP pancreatitis occurred in one patient, who recovered without complications. Conclusion: Ampullary intervention with FCSEMSs is safe and effective for the management of CBD stones in a subset of patients with a history of Billroth II gastrectomy.

Spatio-temporal variability in sea surface wind stress near and off the east coast of Korea

Sea surface wind stress variabilities near and off the east coast of Korea, are examined using 7 kinds of wind datasets from measurements at 2 coastal (land) stations and 2 ocean buoys,satellite scatterometer (QuikSCAT), and global reanalyzed products (ECMWF,NOGAPS,and NCEP/NCAR). Temporal variabilities are analyzed at 3 frequency bands; synoptic (2-20 d), intra-seasonal (20-90 d),and seasonal (>90 d).Synoptic and intra-seasonal variations are predominant near and off the Donghae City due to the passage of the mesoscale weather system. Seasonal variation is caused by southeastward wind stress during Asian winter monsoon. The sea surface wind stress from reanalyzed *** and KMA-B measurements off the coast show good agreement in the magnitude and direction,which are strongly aligned with the alongshore *** the land-based sites,wind stresses are much weaker by factors of 3-10 due to the mountainous landmass on the east parts of Korea *** first EOF modes(67%-70%) of wind stresses from reanalyzed and QuikSCAT data have similar structures of the strong southeastward wind stress in winter along the coast but show different curl structures at scales less than 200 km due to the orographic *** second EOF modes (23%-25%) show southwestward wind stress in every September along the east coast of the North Korea
