Cost-effective screening using a two-antibody panel for detecting mismatch repair deficiency in sporadic colorectal cancer

BACKGROUND The microsatellite instability(MSI)test and immunohistochemistry(IHC)are widely used to screen DNA mismatch repair(MMR)deficiency in sporadic colorectal cancer(CRC).For IHC,a two-antibody panel of MLH1 and MSH2 or four-antibody panel of MLH1,MSH2,PMS2,and MSH6 are *** general,MSI is known as a more accurate screening test than *** To compare two-and four-antibody panels of IHC in terms of accuracy and cost benefit on the basis of MSI testing for detecting MMR *** We retrospectively analyzed patients with CRC who underwent curative surgery between 2015 and 2017 at a tertiary referral *** IHC with four antibodies and MSI tests were routinely *** sensitivity and specificity of a fourand two types of two-antibody panels(PMS2/MSH6 and MLH1/MSH2)were compared on the basis of MSI testing for detecting MMR *** High-frequency MSI was found in 5.5%(n=193)of the patients(n=3486).The sensitivities of the four-and two types of two-antibody panels were 97.4%,92.2%,and 87.6%,*** specificities of the three types of panels did not differ significantly(99.6%for the four-antibody and PMS2/MSH6 panels,99.7%for the MLH1/MSH2 panel).Based on Cohen's kappa statistic(κ),four-and twoantibody panels were in almost perfect agreement with the MSI test(κ>0.9).The costs of the MSI test and the four-and two-antibody panels of IHC were approximately$200,$160,and$80,*** Considering the cost of the four-antibody panel IHC compared to that of the twoantibody panel IHC,a two-antibody panel of PMS2/MSH6 might be the best choice in terms of balancing cost-effectiveness and accuracy.

Voice activity detection based on deep belief networks using likelihood ratio

A novel technique is proposed to improve the performance of voice activity detection(VAD) by using deep belief networks(DBN) with a likelihood ratio(LR). The likelihood ratio is derived from the speech and noise spectral components that are assumed to follow the Gaussian probability density function(PDF). The proposed algorithm employs DBN learning in order to classify voice activity by using the input signal to calculate the likelihood ratio. Experiments show that the proposed algorithm yields improved results in various noise environments, compared to the conventional VAD algorithms. Furthermore, the DBN based algorithm decreases the detection probability of error with [0.7, 2.6] compared to the support vector machine based algorithm.

Online Approach for Spatio-Temporal Trajectory Data Reduction for Portable Devices

As location data are widely available to portable devices, trajectory tracking of moving objects has become an essential technology for most location-based services. To maintain such streaming data of location updates from mobile clients, conventional approaches such as time-based regular location updating and distance-based location updating have been used. However, these methods suffer from the large amount of data, redundant location updates, and large trajectory estimation errors due to the varying speed of moving objects. In this paper, we propose a simple but efficient online trajectory data reduction method for portable devices. To solve the problems of redundancy and large estimation errors, the proposed algorithm computes trajectory errors and finds a recent location update that should be sent to the server to satisfy the user requirements. We evaluate the proposed algorithm with real GPS trajectory data consisting of 17 201 trajectories. The intensive simulation results prove that the proposed algorithm always meets the given user requirements and exhibits a data reduction ratio of greater than 87% when the acceptable trajectory error is greater than or equal to 10 meters.

Optimal postoperative surveillance strategies for stage III colorectal cancer

BACKGROUND Optimal surveillance strategies for stage III colorectal cancer(CRC)are lacking,and intensive surveillance has not conferred a significant survival *** To examine the association between surveillance intensity and recurrence and survival rates in patients with stage III *** Data from patients with pathologic stage III CRC who underwent radical surgery between january 2005 and December 2012 at Asan Medical Center,Seoul,Korea were retrospectively *** consisted of abdominopelvic computed tomography(CT)every 6 mo and chest CT annually during the 5 year follow-up period,resulting in an average of three imaging studies per *** who underwent more than the average number of imaging studies annually were categorized as high intensity(HI),and those with less than the average were categorized as low intensity(LI).RESULTS Among 1888 patients,864(45.8%)were in HI ***,sex,and location were not different between *** group had more advanced T and N stage(P=0.002,0.010,each).Perineural invasion(PNI)was more identified in the HI group(21.4%vs 30.3%,P<0.001).The mean overall survival(OS)and recurrence-free interval(RFI)was longer in the LI group(P<0.001,each).Multivariate analysis indicated that surveillance intensity[odds ratio(OR)=1.999;95%confidence interval(CI):1.680–2.377;P<0.001],pathologic T stage(OR=1.596;95%CI:1.197–2.127;P=0.001),PNI(OR=1.431;95%CI:1.192–1.719;P<0.001),and circumferential resection margin(OR=1.565;95%CI:1.083–2.262;P=0.017)in rectal cancer were significantly associated with *** mean post-recurrence survival(PRS)was longer in patients who received curative resection(P<0.001).Curative resection rate of recurrence was not different between HI(29.3%)and LI(23.8%)groups(P=0.160).PRS did not differ according to surveillance intensity(P=0.802).CONCLUSION Frequent surveillance with CT scan do not improve OS in stage III CRC *** need to evaluate role of other surveillance method rather tha

A global-scale multidecadal variability driven by Atlantic multidecadal oscillation

Observational analysis shows that there is a predominant global-scale multidecadal variability(GMV) of sea-surface temperature(SST). Its horizontal pattern resembles that of the interdecadal Pacific oscillation(IPO) in the Pacific and the Atlantic multidecadal oscillation(AMO) in the Atlantic Ocean, which could affect global precipitation and temperature over the globe. Here, we demonstrate that the GMV could be driven by the AMO through atmospheric teleconnections and atmosphere–ocean coupling *** reveal a strong negative correlation when AMO leads GMV by approximately 4–8 *** experiments using a climate model driven by observed AMO signals reveal that the tropical Atlantic warm SST anomalies of AMO initiate anomalous cooling in the equatorial central-eastern Pacific through atmospheric teleconnections. Anticyclonic anomalies in the North and South Pacific induce equatorward winds along the coasts of North and South America, contributing to further cooling. The upper-ocean dynamics plays a minor role in GMV formation but contributes to a delayed response of the IPO to the AMO forcing. The possible impact of the GMV on AMO was also tested by prescribing only Pacific SST in the model; however, the model could not reproduce the observed phase relationship between the AMO and the GMV. These results support the hypothesis that the Atlantic Ocean plays a key role in the multidecadal variability of global SST.

2.47 GPa grade ultra-strong 15Co-12Ni secondary hardening steel with superior ductility and fracture toughness

This study investigated the effect of multi-step heat treatment on the microstructure, mechanical properties and fracture behavior of thick 15 Co-12 Ni secondary hardening steel. As-quenched sample was found to have elongated prior austenite grain(PAG) and coarse lenticular martensitic structure. On the other hand, heat-treated sample was observed to have fine lenticular martensitic structure due to fine PAG size and a lot of nano-sized carbides. Also, after heat treatment, nano-scale reverted austenite film was formed at the martensite interfaces. The heat-treated sample showed 2.47 GPa superior tensile strength and superior elongation of about 12 %. The high strength was mainly due to fine block size and high number density of nano-sized carbides. The average value of plane strain fracture toughness(KIC) was 29.3 MPa m1/2, which indicated a good fracture toughness even with the high tensile strength. The tensile fracture surface was observed to have ductile fracture mode(cup-and-cone) and the formation of about ~1 μm ultra-fine dimples. In addition to this, nano-sized carbides were observed within the *** findings suggested that the nano-sized carbide had a positive effect not only on the strength but also on the ductility of the alloy. The fractured surface after toughness test, also showed ductile fracture mode with a lot of dimples. Based on the above results, correlation among microstructural evolution,deformation and fracture mechanisms along the heat-treatment was also discussed.
