Syndrome Pattern and Its Application in Parallel Randomized Controlled Trials

Syndrome pattern （SP） is a core concept of Chinese medicine （CM） and is used to diagnose and treat patients based on an overall analysis of symptoms and signs. This study aimed to systematically review randomized controlled trials （RCTs） using the SP concept and to demonstrate how the SP concept could be applied to the study design of parallel RCTs, considering a gold standard of clinical research. After conducting a brief systematic review by way of a PubMed search, we analyzed how the SP concept was applied to the design of RCT in a CM herbal medicine trial. We then formulated possible research questions, applied the SP concept to answer the research questions, and suggested possible RCT designs to be used for conducting future trials. Fourteen RCTs were included in our systematic review, and three key points of the SP concept were formulated for the design of parallel RCTs： the time point of SP diagnosis between before and after randomization; the relationship between the international classification of diseases （ICD） and SP for the inclusion of target population; and the proper diagnostic method of SP. In this study, we formulated three possible research questions and then suggested perspectives for five possible RCT models arrived at using SP concepts. Future trials applying SP concept to RCTs should overcome the shortcomings of past SP trials, moving CM forward from experience-based to evidence-based medicine.

The short release half-life of carbon monoxide(CO)is a major obstacle to the effective therapeutic use of carbon monoxide-releasing molecule-2(CORM-2).The potential of CORM-2-entrapped ultradeformable liposomes(CORM-2-UDLs)to enhance the release half-life of CO and alleviate skin inflammation was investigated in the present ***-2-UDLs were prepared by using soy phosphatidylcholine to form lipid bilayers and Tween 80 as an edge *** deformability of CORM-2-UDLs was measured and compared with that of conventional liposomes by passing formulations through a filter device at a constant *** release profile of CO from CORM-2-UDLs was evaluated by myoglobin *** vitro and in vivo anti-inflammatory effects of CORM-2-UDLs were assessed in lipopolysaccharide-stimulated macrophages and TPA-induced ear edema model,*** deformability of the optimized CORM-2-UDLs was 2.3 times higher than conventional ***-2-UDLs significantly prolonged the release half-life of CO from 30 s in a CORM-2 solution to 21.6 ***-2-UDLs demonstrated in vitro anti-inflammatory activity by decreasing nitrite production and pro-inflammatory cytokine ***,CORM-2-UDLs successfully ameliorated skin inflammation by reducing ear edema,pathological scores,neutrophil accumulation,and inflammatory cytokines *** results demonstrate that CORM-2-UDLs could be used as promising therapeutics against acute skin inflammation.

Safety of endoscopic ultrasound-guided ethanol ablation for pancreatic cystic lesions: A single-center experience of 214 patients

Background:Endoscopic ultrasound-guided ethanol ablation(EUS-EA)for pancreatic cystic lesions(PCLs)has been used in recent years as a feasible treatment modality for low malignant probability PCLs or patients considered high-risk for *** present study aimed to confirm the safety of EUS-EA and to find predictive factors for adverse event(AE).Methods:A retrospective review was performed from the prospectively maintained database of patients who underwent EUS-EA for PCLs from June 2006 to April 2018 at Seoul National University *** primary outcomes of the study were the rates of AEs and severe AEs by *** secondary outcome was the predictive factors of AEs including acute pancreatitis and abdominal ***:A total of 214 patients were evaluated and the diagnoses of PCLs according to cystic fluid analysis and clinical features were as follows:serous cystic neoplasm(32.2%),mucinous cystic neoplasm(26.6%),branch duct type intraductal papillary mucinous neoplasm(BD-IPMN)(29.4%),and pseudocyst(11.7%).Three patients(1.4%)experienced severe ***,AEs occurred in 71(33.2%)***-IPMN(OR:2.87;95%CI:1.05–7.84;P=0.040),multilocular cysts(OR:3.59;95%CI:1.09–11.85;P=0.036),suspected ethanol leakage during procedure(OR:10.68;95%CI:1.98–57.53;P=0.006),and sticky cystic fluid(OR:3.83;95%CI:1.20–12.24;P=0.024)were predictive factors for post-procedural acute *** of uncinate process(OR:2.99;95%CI:1.22–7.35;P=0.017)and PCLs with exophytic portion(OR:3.70;95%CI:1.96–7.01;P<0.001)were predictive factors for post-procedural abdominal ***:EUS-EA is a safe procedure with a very low rate of severe *** seems possible to predict the AEs according to the features of the procedure and PCLs.

Recently,polymer materials have been at the forefront of other materials in building high-performance flexible electronic skin(e-skin)devices due to con-spicuous advantages including excellent mechanical flexibility,good compatibil-ity,and high ***,most research works just paid considerable attention and effort to the design,construction,and possible application of e-skins that reproduce the tactile perception of the human skin sensory *** with tactile sensing devices,e-skins that aim to imitate the non-contact sensing features in the sensory system of human skin tend to avoid undesired issues such as bacteria spreading and mechanical *** further promote the development of e-skins to the human skin sensory system where tactile perception and non-contact sensing complement each other,significant progress and advances have been achieved in the field of polymer materials enabled e-skins for both tactile perception and non-contact sensing *** this review,the latest progress in polymer material-based e-skins with regard to tactile,non-contact sensing capabilities and their practical applications are *** fabrication strategies of polymer materials and their role in building high-performance e-skins for tactile and non-contact sensing are ***,we also review the research works that integrated the polymer-based tactile and non-contact e-skins into robots and prostheses,smart gloves,and VR/AR devices and addressed some representative problems to dem-onstrate their suitability in practical applications in human–machine ***,the current challenges in the construction of high-performance tactile and non-contact e-skins are highlighted and promising properties in this direction,by taking advantage of the polymer materials,are outlined.

Regulatory T cells （Tregs） are a specialized subpopulation of T cells that control the immune response and thereby maintain immune system homeostasis and tolerance to self-antigens. Many subsets of CD4＋ Tregs have been identified, including Foxp3~, Trl, Th3, and Foxp3neg iT（R）35 cells. In this study, we identified a new subset of CD4＋VEGFR1high Tregs that have immunosuppressive capacity. CD4＋VEGFRlhigh T cells, which constitute approximately 1.0% of CD4＋ T cells, are hyporesponsive to T-cell antigen receptor stimulation. Surface marker and FoxP3 expression analysis revealed that CD4＋VEGFR1high T cells are distinct from known Tregs. CD4＋VEGFR1high T cells suppressed the proliferation of CD4＋CD25- T cell as efficiently as CD4＋CD25high natural Tregs in a contact-independent manner. Furthermore, adoptive transfer of CD4＋VEGFR1＋ T cells from wild type to RAG-2-deficient C57BL/6 mice inhibited effector T-cell-mediated inflammatory bowel disease. Thus, we report CD4＋ VEGFR1high T cells as a novel subset of Tregs that regulate the inflammatory response in the intestinal tract.

Bevacizumab as a second- or later-line of treatment for metastatic colorectal cancer

AIM: To determine the efficacy of bevacizumab in patients with metastatic colorectal cancer (MCRC) who have failed prior chemotherapy without bevacizumab. METhoDS: Between March 2002 and June 2010, 40 patients in South Korea with MCRC who were treated with bevacizumab plus chemotherapy as a second or later-line treatment were analyzed retrospectively for their overall response rate (ORR), overall survival (OS), and progression-free survival (PFS). The tumor responses were assessed using the RECIST (Response Evaluation Criteria in Solid Tumors) guidelines. RESULTS: All of the patients had progressed under prior chemotherapy without bevacizumab. Three patients (7.5%) exhibited an ORR, twenty one patients (52.5%) exhibited stable disease (SD), and fifteen patients (37.5%) exhibited disease progression. The median duration of the OS and PFS were 14.0 and 6.13 mo respectively. The median OSs were 16.60, 14.07 and 13.00 mo for second-line, third-line and fourth- or later-line treatments, respectively. The median PFSs were 7.23, 7.30 and 3.87 mo for the second-line, third-line and fourth- or later-line treatments, respectively. CONCLUSION: In patients with MCRC, bevacizumab combined chemotherapy may be beneficial during second- or later-line treatment.

Neurons in the hippocampal CA1 region,but not the dentate gyrus,are susceptible to oxidative stress in rats with streptozotocin-induced type 1 diabetes

In this study, we investigated the effects of streptozotocin-induced type 1 diabetes on antioxi- dant-like protein-1 immunoreactivity, protein carbonyl levels, and malondialdehyde formation, a marker for lipid peroxidation, in the hippocampus. For this study, streptozotocin （75 mg/kg） was intraperitoneally injected into adult rats to induce type 1 diabetes. The three experimental pa- rameters were determined at 2, 3, 4 weeks after streptozotocin treatment. Fasting blood glucose levels significantly increased by 20.7-21.9 mM after streptozotocin treatment. The number of antioxidant-like protein-1 immunoreactive neurons significantly decreased in the hippocampal CA1 region, but not the dentate gyrus, 3 weeks after streptozotocin treatment compared to the control group. Malondialdehyde and protein carbonyl levels, which are modified by oxidative stress, significantly increased with a peak at 3 weeks after malondialdehyde treatment, and then decreased 4 weeks after malondialdehyde treatment. These results suggest that neurons in the hippocampal CA1 region, but not the dentate gyrus, are susceptible to oxidative stress 3 weeks after malondialdehyde treatment.

BACKGROUND: Although perioperative portal vein (PV) stent implantation is an effective treatment for steno-occlusive disease in adult living donor liver transplantation (LDLT) re-cipients, we experienced high incidence of biliary anastomotic strictures (BAS) after PV stenting. In this study, we sought to clarify the relation between BAS and PV stenting and to suggest the possible mechanism of BAS and measures to reduce its in-cidence. METhoDS: We retrospectively analyzed 44 LDLT recipients who underwent PV stent implantation across the line of PV anastomosis regardless of the location of steno-occlusion (stent group) and their matched controls (non-stented LDLT recipients, n=131). RESULTS: The incidence of BAS was higher in patients in the stent group than that in the control group (43.2% vs 17.6%, P=0.001). Cumulative 6-month and 1-, 2- and 5-year BAS rates were 31.8%, 34.1%, 41.4% and 43.2%, respectively, in the stent group and 13.0%, 13.8%, 16.1% and 17.8%, respectively, in the control group (P=0.001). Multivariate analysis revealed that PV stenting was an independent risk factor for BAS. CONCLUSIONS: Although PV stent implantation is a reliable treatment modality for steno-occlusive PV in adult LDLT recipients, innovative methods to prevent the PV stent from crossing the line of PV anastomosis may be necessary to reduce the incidence of postoperative BAS.
