Effectiveness and safety of a laparoscopic training system combined with modified reconstruction techniques for total laparoscopic distal gastrectomy

Management of the late effects of disconnected pancreatic duct syndrome: A case report

BACKGROUND There have been few reports about the late effects of disconnected pancreatic duct syndrome(DPDS). Although few reports have described the recurrence interval of pancreatitis, it might be rare for recurrence to occur more than 5 years ***, we describe a case of recurrence in an 81-year-old man after the treatment of walled-off necrosis(WON) with pancreatic transection 7 years *** SUMMARY An 81-year-old man visited our hospital with chief complaints of fever and abdominal pain 7 years after the onset of WON due to severe necrotic pancreatitis. His medical history included an abdominal aortic aneurysm(AAA),hypertension, dyslipidemia, and chronic kidney disease. Computed tomography(CT) scan showed that the pancreatic fluid collection(PFC) had spread to the aorta with inflammation surrounding it, and CT findings suggested that bleeding occurred from the vasodilation due to splenic vein occlusion. First, we attempted to perform transpapillary drainage because of venous dilation around the residual stomach and the PFC. However, pancreatic duct drainage failed because of complete main pancreatic duct disruption. Second, we performed endoscopic ultrasound-guided drainage. After transmural drainage, the inflammation improved and stenting for the AAA was performed successfully. The inflammation was resolved, and he has been free from infection for more than 2 years after the *** This case highlights the importance of continued follow-up of patients for recurrence after the treatment of WON with pancreatic transection.

AIM:To clarify whether nonalcoholic fatty liver disease(NAFLD)increases the risk of cardiovascular ***:We carried out a prospective observational study with a total of 1637 apparently healthy Japanese men and women who were recruited from a health check-up *** was diagnosed by abdominal *** metabolic syndrome(MS)was defined according to the modified National Cholesterol Education Program(NCEP)ATP Ⅲ *** years after the baseline evaluations,the incidence of cardiovascular disease was assessed by a self-administered ***:Among 1221 participants available for outcome analyses,the incidence of cardiovascular disease was higher in 231 subjects with NAFLD at baseline(5 coronary heart disease,6 ischemic stroke,and 1 cerebral hemorrhage)than 990 subjects without NAFLD(3 coronary heart disease,6 ischemic stroke,and 1 cerebral hemorrhage).Multivariate analyses indicated that NAFLD was a predictor of cardiovascular disease independent of conventional risk factors(odds ratio 4.12,95% CI,1.58 to 10.75,P = 0.004).MS was alsoindependently associated with cardiovascular *** simultaneous inclusion of NAFLD and MS in a multivariate model revealed that NAFLD but not MS retained a statistically significant correlation with cardiovascular ***:Although both of them were predictors of cardiovascular disease,NAFLD but not MS retained a statistically significant correlation with cardiovascular disease in a multivariate *** is a strong predictor of cardiovascular disease and may play a central role in the cardiovascular risk of MS.

AIM： To analyze the localization of erythropoietin receptor on gastric specimens and characterize the effects of erythropoietin on the normal gastric epithelial proliferation using a porcine gastric epithelial cell culture model. METHODS： Erythropoietin receptor was detected by RT-PCR, Western blotting and immunohistochermistry. Growth stimulation effects of erythropoietin on cultured gastric mucosal cells were determined by ELISA using bromodeoxyuridine （BrdU）. RESULTS： Erythropoietin receptor was detected on cultured porcine gastric mucosal epithelial cells. Erythropoietin receptor was also detected histochemically at the base of gastric mucosal epithelium. BrdU assay demonstrated a dose-dependent increase in growth potential of cultured porcine gastric mucosal epithelial cells by administration of erythropoietin, as well as these effects were inhibited by administration of antierythropoietin antibody （P〈 0.01）. CONCLUSION： These findings indicate that erythropoietin has a potential to proliferate gastric mucosal epithelium via erythropoietin receptor.

Direct Production of High Pressure Hydrogen at Great Rate from Glycerol/Water/Metal Mixture

One of the key issues facing the global society today is to find renewable and sustainable energy sources. Hydrogen has gained much attention in recent years since it is one of fuels for fuel cells. It emits no carbon dioxide when it is used and so on. In this study, a great rate production of high pressure hydrogen rich gas from glycerol/water/metal mixtures was developed since glycerol has become one of the enormous industrial by-products, especially from biodiesel processing plants. It was found that cobalt was the optimum metal additive among tested metals of aluminum, cobalt, magnesium and nickel in terms of a hydrogen producing rate, a hydrogen partial pressure and a conversion ratio from 50 mol% glycerol/water mixtures under an operating temperature of 723 K. Concretely, hydrogen rich gas with concentration about 64%H2 and high partial pressure about 4 MPaN,H2 could be produced at the great producing rate of 42.9 LN,H2 dm-2min-1 and high conversion ratio about 60%H2. All the produced hydrogen rich gases from glycerol/water/metal mixtures were by no means inferior to pure hydrogen as a fuel for the polymer elec-trolyte fuel cell.

Association of social distance toward schizophrenia with help-seeking among mothers of adolescents in Japan

Negative maternal attitudes toward schizophrenia may be linked with delayed treatment of their children. We investigated the relation between negative attitudes toward schizophrenia and help-seeking among mothers of junior and senior high school students in Japan. The participants were 1309 Japanese mothers of junior and senior high school students. Social distance was evaluated by using the Social Distance Scale-Japanese version (SDS-J). In addition, mothers were asked about help-seeking for a child with sleeplessness, social withdrawal, and strange behavior. One-way analysis of variance and Student’s t-test were used to evaluate associations between social distance toward schizophrenia and help-seeking. Most (76.4%) participants were aged 40 - 49 years. Maternal demographic characteristics significantly associated with social distance were employment and participation in welfare activities for people with mental illness. In responding to a child with sleeplessness, social withdrawal, and strange behavior, the level of maternal social distance was not significantly associated with the likelyhood of seeking psychiatric help. However, mothers with greater social distance were less likely to seek help at a psychiatric clinic. Maternal social distance toward schizophrenia was not significantly associated with seeking psychiatric help;however, it did affect the type of facility selected among those would seek such help.

Design and Implementation of a Programming Learning Support System Using the Concepts of Physical Visualization and Serious Gaming

The purpose of this research is the design and implementation of a support system for learning programming. To archive this purpose, in this article, we propose a Puzzle Programming System that uses jigsaw puzzles as an example of the application of physical visualization, which visualizes logical constraints to physical ones. This Puzzle Programming System aims to teach basic programming concepts by presenting the invisible constraints of programming language syntax using the visual constraints of jigsaw puzzle pieces. This system runs on an Apple iPad and was developed using the Unity game engine. We used YAML as a data format for serializing structured data for data management. By inviting high school students to try out a prototype, we could confirm the usefulness of the Puzzle Programming System. The experimental evaluation results also shed light on aspects of the game that need to be redesigned and parts where the visual programming model needs to be modified and expanded.

A Breast Density Classification System for Mammography Considering Reliability Issues in Deep Learning

In a convolutional neural network (CNN) classification model for diagnosing medical images, transparency and interpretability of the model’s behavior are crucial in addition to high classification accuracy, and it is highly important to explicitly demonstrate them. In this study, we constructed an interpretable CNN-based model for breast density classification using spectral information from mammograms. We evaluated whether the model’s prediction scores provided reliable probability values using a reliability diagram and visualized the basis for the final prediction. In constructing the classification model, we modified ResNet50 and introduced algorithms for extracting and inputting image spectra, visualizing network behavior, and quantifying prediction ambiguity. From the experimental results, our proposed model demonstrated not only high classification accuracy but also higher reliability and interpretability compared to the conventional CNN models that use pixel information from images. Furthermore, our proposed model can detect misclassified data and indicate explicit basis for prediction. The results demonstrated the effectiveness and usefulness of our proposed model from the perspective of credibility and transparency.

Nanoindentation method is applied to examine the mechanical properties of materials such as elastic and plastic deformation, fracture and friction. However, although the method has potential applications to determine nanoscale mechanical properties of small areas and thin films, the atomic scale mechanisms underlying the method are not well understood. In order to clarify the mechanisms, atomic scale simulations such as molecular dynamics have been carried out. The molecular dynamics simulations indicated that dislocation loops are nucleated under an indenter and glide on the slip plane, then dislocation structures due to dislocation interactions and the cross slip are formed in single crystalline metals [1,2]. Moreover, the influence of grain boundaries on the plastic deformation process in nanocrystalline metals [3] and the deformation process of ceramic materials [4] under nanoindentation were reported. In this study, we examine the initial stage of plastic deformation in nanoindentation process in a thin iron film using a molecular dynamics simulation. A rigid knife-like indenter is used in the simulation. We obtain a load-displacement curve under the action of an indenter. When the displacement of the indenter exceeds a yield point, the nucleation of dislocation loops is observed below the surface contacting with indenter sides. Then the Lomer-Cottrell reaction between the dislocation loops occurs directly beneath the indenter. In addition, new dislocation generated by the Lomer-Cottrell reaction moves in a different slip plane together with double kink formation with increasing the displacement and finally is separated into two dislocations.

Three chemical forms of tritium has been observed in the atmosphere,those are tritiated water (HTO),elemental tritium(HT) and tritiated hydrocarbons(CHT).The concentrations of these chemical species are not so different,distributing from a few mBq/m to at most a few tens mBq/***,a significant difference are observed in their specific activities(T/H),ranging 3 orders of *** HT has the highest specific activity followed by *** specific activity of HTO is lowest among them and comparable to that of environmental *** enrichment mechanism of tritium to hydrogen(H) and methane(CH) is reported in the environment,suggesting HT and CHT probably coming from artificial sources.
