The Becak (a manpowered tricycle taxi) was exported to major Indonesian cities via China from Yokohama in the 1930s. During the mid-20th century they spread throughout the major cities of Asia, and came to be used as a vehicle for daily use. However, since the 1970s, growing traffic congestion caused by an increase in the number of motorcycles and cars, has pushed the becak into the back streets of cities. In the case of Indonesia, government came to strictly prohibit them through the perception that they were a symbol of backwardness and they have gradually disappeared from the city. This culture of viewing becak as a symbol of backwardness has remained ingrained over the decades. In Jakarta, they were banned from use in lanes and back-alleys to finally disappear from public view through a brutal clearance method known as ‘becak hunting’(crackdowns) in 1992. However, they re-emerged on Jakarta’s streets during the chaos after Suharto’s resignation in 1998. Governor Sutiyoso at the time banned them anew, yet democratic transformation through the rise of civil society did not tolerate subsequent crackdowns As of 2016, hundreds of becak vehicles now plow the streets of Jakarta, mainly concentrated in back-alley neighbourhoods. Based on primary data sources gathered over a 30-year period (1989, 1998, and 2011-2015), this paper examines socio-economic transformation and continuity and investigates the extent in which the changing political economy of Jakarta affects drivers’ accessibility. Furthermore, the paper forecasts the survival strategies of becak drivers under ungoing policy changes and the positions of previous and present Governors.

AIM:To test this hypothesis of barrett esophagus (BE) classified into two types and to further determine if there was any correlation between the shape of endoscopically suspected esophageal metaplasia (ESEM), prevalence of reflux esophagitis (RE) and heartburn. METHODS: A total of 6504 Japanese who underwent endoscopy for their annual stomach check-up were enrolled in this study. BE was detected without histological confirmation that is ESEM. We originally classified cases of ESEM into 3 types based on its shape: Tongue-like (T type), Dome-like (D type) and Wave-like (W type) ESEM. The respective subjects were prospectively asked to complete questionnaires concerning the symptoms of heartburn, dysphagia, and abdominal pain for a one-month period. RESULTS: ESEM was observed in 10.3% of 6504 subjects (ESEM < 1 cm, 9.4%; 1 cm ≤ ESEM < 3 cm, 1.7%; ESEM ≥ 3 cm, 0.5%). The frequency of ESEM was significantly higher in males compared with female subjects. Statistical analysis showed that the prevalence of heartburn and RE were significantly higher in the T type ESEM than in the W type ESEM (P < 0.05). CONCLUSION: The T type ESEM was strongly associated with reflux symptoms and RE whereas the W type ESEM was not associated with GERD.

AIM:To examine the prevalence of gastroesophageal reflux disease (GERD) symptoms in a large unselected general population in Japan. METHODS: In Japan, mature adults are offered regular check-ups for the prevention of gastric cancer. A notice was sent by mail to all inhabitants aged > 40 years. A total of 160 983 Japanese (60 774 male, 100 209 female; mean age 61.9 years) who underwent a stomach check up were enrolled in this study. In addition, from these 160 983 subjects, we randomly selected a total of 82 894 (34 275 male, 48 619 female; mean age 62.4 years) to evaluate the prevalence of abdominal pain. The respective subjects were prospectively asked to complete questionnaires concerning the symptoms of heartburn, dysphagia, and abdominal pain for a 1 mo period. RESULTS: The respective prevalences of the symptoms in males and females were: heartburn, 15.8% vs 20.7%; dysphagia, 5.4% vs 7.8%; and abdominal pain, 6.6% vs 9.6%. Among these symptoms, heartburn was significantly high compared with the other symptoms, and the prevalence of heartburn was significantly more frequent in females than in males in the 60-89-year agegroup. Dysphagia was also significantly more frequent in female patients. CONCLUSION: The prevalence of typical GERD symptoms (heartburn) was high, at about 20% of the Japan population, and the frequency was especially high in females in the 60-89 year age group.

Continuous regional arterial infusion therapy with gabexate mesilate for severe acute pancreatitis

AIM: To evaluate the effi cacy of continuous regional arterial infusion therapy (CRAI) with gabexate mesilate and antibiotics for severe acute pancreatitis (SAP). METHODS: We conducted a prospective study on patients who developed SAP with or without CRAI. Out of 18 patients fulf illed clinical diagnostic criteria for SAP in Japan, 9 patients underwent CRAI, while 9 patients underwent conventional systemic protease inhibitor and antibiotics therapy (non-CRAI). CRAI was initiated within 72 h of the onset of pancreatitis. Gabexate mesilate (2400 mg/d) was continuously administered for 3 to 5 d. The clinical outcome including serum infl ammation-related parameters were examined. RESULTS: The duration of abdominal pain in the CRAI group was 1.9 ± 0.26 d, whereas that in the non-CRAI group was 4.3 ± 0.50. The duration of SIRS in the CRAI group was 2.2 ± 0.22 d, whereas that in the non- CRAI group was 3.2 ± 0.28. Abdominal pain and SIRS disappeared signifi cantly in a short period of time after the initiation of CRAI using gabexate mesilate. The average length of hospitalization signifi cantly differed between the CRAI and non-CRAI groups, 53.3 ± 7.9 d and 87.4 ± 13.9 d, respectively. During the first two weeks, levels of serum CRP and the IL6/IL10 ratio in the CRAI group tended to have a rapid decrease compared to those in the non-CRAI group. CONCLUSION: The present results suggest that CRAI using gabexate mesilate was effective against SAP.

Recent diagnostic and therapeutic progress for severe acute pancreatitis(SAP)remarkably decreased the casemortality *** further decrease the mortality rate of SAP,it is important to precisely evaluate the severity at an early stage,and initiate appropriate treatment as early as *** Committee of Intractable Diseases of the Pancreas in Japan developed simpler criteria combining routinely available data with clinical *** can be evaluated by laboratory examinations or by clinical signs,reducing the defect values of the severity ***,the severity criteria considered laboratory/clinical severity scores and contrastenhanced computed tomography(CE-CT)findings as independent risk ***,CE-CT scans are not necessarily required to evaluate the severity of acute *** was no fatal case in mild AP diagnosed by the CE-CT severity score,whereas case-mortality rate in those with SAP was 14.8%.Case-mortality of SAP that fulfilled both the laboratory/clinical and the CE-CT severity criteria was 30.8%.It is recommended,therefore,to perform CE-CT examination to clarify the prognosis in those patients who were diagnosed as SAP by laboratory/clinical severity *** the mortality rate of these patients with SAP is high,such patients should be transferred to advanced medical units.

AIM:To make clear whether CD147 (EMMPRIN) expression in pathological tumor samples with a fine-needle aspiration biopsy is useful for pathological diagnosis of early hepatocellular carcinoma (HCC). METHODS:Twenty-two patients (15 men and 7 women; median age 68 years,range 56-81 years) underwent a liver tissue biopsy in order to make a diagnosis of HCC. Paraffin-embedded liver biopsy tissue samples from 22 patients were stained with anti-CD147 antibody,murine monoclonal antibody 12C3 (MAb12C3) for immunohistochemical analysis. An immunohistochemical analysis of CD147 was performed and the degree of staining compared between tumor and non-tumor tissue. In addition,the degree of staining within tumor tissue was compared according to a number of clinicopathological variables. RESULTS:The degree of staining of CD147 was significantly higher in tumor tissues than non-tumor tissues,even in tumors less than 15 mm in *** expression of this protein was significantly elevated in HCC tissue specimens from patients with a low value of serum AST and γ-GTP.

Purpose: This study aimed to examine whether the signal-to-noise ratio (SNR) could be increased by combining integrated slice-by-slice shimming (iShim) with a fat suppression (FS) method other than short-tau inversion recovery (STIR) in diffusion-weighted imaging (DWI) and q-space imaging (qsi). Methods: We acquired DWI images (b-values: 0 and nine steps from 400 to 10,000 s/mm2 for six axes) using a prototypical single-shot echo planar imaging sequence by combining two types of shimming (3D Shim and iShim) and two types of FS (STIR and water excitation [WE]) in 10 volunteers. In the DWI study, the SNR for each b-value, FS effect in the b0 image, and distortion in the added image (b0 - b10,000) were evaluated for the above-mentioned four imaging methods. qsi involved original DWI images. In the qsi study, the SNR was evaluated. Results: With regard to both 3D Shim and iShim, the SNRs were significantly higher when using WE than when using STIR in b0 - b900 images (p Conclusion: The combination of iShim and WE has a high SNR on qsi.

In basal squamous cells, plectin-1 interacts with intermediate filaments, whereas trichohyalin, which is distributed primarily in the medulla and inner root sheath cells of human hair follicles, plays a role in strengthening cells during keratinization. Although both cytoskeletal proteins occur in trace amounts in human tongue epithelial cells, there are minimal data on their expression in human tongue primary cancer cells. We therefore investigated the expression of plectin-1 and trichohyalin in human tongue epithelial cell line (DOK) and tongue cancer cell line (BICR31) using western blotting and FITC-labeled immunocytochemistry techniques. DOK and BICR31 cells were cultivated to subconfluence in Dulbecco’s Modified Eagle’s Medium containing 0.4 μg/ml of hydrocortisone and 10% fetal bovine serum, and the levels of trichohyalin and plectin-1 were determined by western blot analysis and immunocytochemical staining. Trichohyalin expression was clearly observed, with no differences between DOK and BICR31 cells. Although DOK cells expressed trace levels of plectin-1, obvious plectin-1 bands were detected in western blot analyses of BICR31 cells. Immunocytochemical staining revealed that trichohyalin and plectin-1 localize in the cytoplasm. Trichohyalin was diffusely distributed in both cell lines, and colocalization of trichohyalin and cytokeratin 1/10 was observed in almost all BICR31 cells. There were no correlations between western blot and immunocytochemical data for trichohyalin. Conversely, correlations in immunochemical reactions for plectin-1 were observed. Most DOK cells showed no localization of plectin-1, but strong reactions were detected in the cytoplasm of BICR31 cells. These results indicate that trichohyalin is expressed by cancerous tongue epithelial cells during various stages of malignancy and that plectin-1 provides an index of malignancy.

A Note on Relation between the Fourier Coefficients and the Effects in the Experimental Design

It has recently been shown that the model in experimental design can be expressed in terms of an orthonormal system. In this case, the model is expressed by using Fourier coefficients instead of the effect of each factor. As there is an abundance of software for calculating the Fourier transform, such a system allows for a straightforward implementation of the procedures for estimating the Fourier coefficients by using Fourier transform. However, Fourier coefficients themselves do not provide a direct representation of the effect of each factor, and the relation between the Fourier coefficients and the effect of each factor has not yet been clarified. In this paper, we present theorems of the relation between the Fourier coefficients and the effect of each factor. By using these theorems, the effect of each factor can be easily obtained from the computed Fourier coefficients. Therefore, with the aid of an orthonormal system, it is possible to easily implement the estimation procedures as well as to understand how each factor affects the response variable in the model.
