Retrospective Analysis of Ultrasound-guided Flexible Ureteroscopy in the Management of Calyceal Diverticular Calculi

Background： Percutaneous nephrolithotomy （PCNL） is the most widely recommended treatment lbr calyceal diverticular calculi, providing excellent stone-free results. However, its invasiveness is not negligible considering its major complication rates. Flexible ureteroscopy （FURS） is currently used to treat calyceal diverticula. However, the greatest drawback of FURS is locating the diverticulum since its neck is narrow and concealed. In such a case, the FURS procedure must be converted to PCNL. The aim of this study was to evaluate ultrasound-guided flexible ureteroscopy （UFURS） identifying diverticulum and the managenlent of calyceal diverticular calculi. Methods： A retrospective analysis was conducted on 24 patients who had calyceal diverticnlar calculi. In all 12 patients in tile UFURS group, direct FURS failed to find evidence of calyceal diverticula but were confirmed with imaging. The other 12 patients in the PCNL group received PCNL plus fulguration of the divenicular walls. Results： Puncture ofcalyceal diverticulum was successful in all 12 UFURS patients. Two patients in this group had postoperative residual calculi and two patients developed lever. In the PCNL group, percutaneous renal access and lithotomy were successful in all 12 patients. One patient in this group had residual calculi, one had perirenal hematoma, and two patients developed fever. No significant difference was found in the operating time （UFURS vs. PCNL, 91.8 ± 24.2 vs. 86.3 ± 18.7 min）, stone-free rate （UFURS vs. PCNL, 9/12 vs. 10/12）, and rate of successful lithotripsy （UFURS vs. PCNL, 10/12 vs. 11/12） between the two groups （all P 〉 0.05）. Postoperative pain scores in the FURS group were significantly lower than that in the PCNL group （2.7 ±1.2 vs. 6.2 ±1.5. P 〈 0.05）. Hospital stay in the UFURS group was significantly shorter than that in the PCNL group （3.4 ± 0.8 vs. 5.4 ± 1.0 days, P 〈 0.05）. A congenital isolated left ventricular apical diverticulum simulating a tako-tsubo

Left ventricular diverticulum is a rare congenital anomaly of which the incidence was reported to be 0.26%. Diverticula are usually localized near the apex and most often involve the inferior or anterior parietal walls of the left ventricle. In this report, we describe a rare case of congenital isolated left ventricular apical diverticulum, which was tako-tsubo-like in systole, ＂dumbbell-like＂ the whole left ventricle, diagnosed by angiography and magnetic resonance imaging.

Niemann-Pick type C2 （NPC2） is a type of small soluble protein involved in lipid metabolism and triglyceride accumulation in vertebrates and arthropods. Recent stud- ies have determined that NPC2 also participates in chemical communication of arthropods. In this work, two novel NPC2 proteins （MmedNPC2a and MmedNPC2b） in Microplitis mediator were identified. Real-time quantitative PCR （qPCR） analysis revealed that Mmed- NPC2a was expressed higher in the antennae than in other tissues of adult wasps compared with MmedNPC2b. Subsequent immunolocalization results demonstrated that NPC2a was located in the lymph cavities of sensilla placodea. To further explore the binding charac- terization of recombinant MmedNPC2a to 54 candidate odor molecules, a fluorescence binding assay was performed. It was found MmedNPC2a could not bind with selected fatty acids, such as linoleic acid, palmitic acid, stearic acid and octadecenoic acid. How- ever, seven cotton volatiles, 4-ethylbenzaldehyde, 3,4-dimethylbenzaldehyde, fl-ionone, linalool, m-xylene, benzaldehyde and trans-2-hexen-l-al showed certain binding abilities with MmedNPC2a. Moreover, the predicted 3D model of MmedNPC2a was composed of seven r-sheets and three pairs of disulfide bridges. In this model, the key binding residues for oleic acid in CjapNPC2 of Camponotus japonicus, Lue68, Lys69, Lys70, Phe97, Thr103 and Phe127, are replaced with Phe85, Ser86, His87, Leu113, Tyr119 and Ile143 in MmedNPC2a, respectively. We proposed that MmedNPC2a in M. mediator may play roles in perception of plant volatiles.

Association of IL-7 with disease progression in Chinese HIV-1 seropositive individuals

Background Elevated levels of interleukin-7 （IL-7） have been correlated with CD4^＋ T cell depletion and the emergence of syncytium-inducing （SI） variants in human immunodeficiency virus type-1 （HIV-1） infection, and suggested as an indicator of acquired immunodeficiency syndrome （AIDS） disease progression. Therefore, we investigated the effects of IL-7 on disease progression and virus phenotype in Chinese HIV/AIDS patients. Methods In a cross-sectional study of 71 untreated HIV-1 seropositive individuals and 12 healthy donors, plasma IL-7 levels were determined by an ultra sensitive enzyme-linked immunosorbent assay （ELISA）, and its relations to CD4^＋ T cells, CD8^＋ T cells, plasma viral loads and HIV phenotypes were analyzed. Results Significant higher IL-7 levels were found in Chinese HIV/AIDS patients [（3.33±3.60） pg/ml] than those of health controls [（1.2±0.81） pg/ml] （P〈0.05）, and IL-7 levels were inversely associated with CD4^＋ T cell counts （r=-0.497, P〈0.01）. Furthermore, IL-7 levels were significant higher in patients with SI variants [（9.12±4.55） pg/ml] than those with non-syncytium-inducing variants [（1.50±2.69） pg/ml] （P〈0.01）. Conclusions Increased IL-7 levels were found in Chinese HIV/AIDS patients and significantly associated with disease progression, thus increased IL-7 plasma levels may indicate disease progression.

Mode converter based on dual-core all-solid photonic bandgap fiber

In this paper, we present a mode-selective coupler based on a dual-core all-solid photonic bandgap fiber（AS-PBGF）. Because they are all-solid, AS-PBGF-based mode converters are easier to splice to other fibers than those based on air-hole photonic crystal fibers. Mode conversions between the LP01 and LP11modes, LP01 and LP21modes, and LP01 and LP02modes are obtained at the wavelength λ=1550 nm. The 3 dB wavelength bandwidth of these mode converters are 47.8,20.3, and 20.3 nm, respectively.

The research and application of responsive materials have long been hampered by their complicated designs and tedious construction ***,many current responsive materials show retard or weak *** this study,responsive hybrid poly(vinyl alcohol)hydrogel membranes with embedded poly(N-isopropylacrylamide-acrylic acid)microgels as valves were constructed by simple mixing and subsequent freezing-thawing *** the structure of the membranes,the matrix poly(vinyl alcohol)chains thread through and entangle with the microgels,and the microgels are firmly constrained within the hybrid hydrogel *** fast and sharp temperature responsiveness of the embedded microgels was largely retained and endowed the hydrogel membrane with excellent temperature and pH ***,the hydrogel membrane showed excellent fatigue resistance in both temperature and pH-responsive flux *** study presented the great potential of these hybrid hydrogel membranes in biomedical applications and provided a new strategy for the future design and construction of responsive biomaterials.

Silencing of odorant binding protein gene AlinOBP4 by RNAi induces declining electrophysiological responses of Adelphocoris lineolatus to six semiochemicals

Odorant binding proteins （OBPs） are believed to be important for transporting semiochemicals through the aqueous sensillar lymph to the olfactory receptor cells within the insect antennal sensilla. Here, we injected AlinOBP4-siRNA into the conjunctivum between prothorax and mesothorax of the lucerne plant bug, Adelphocoris lineolatus and evaluated the silencing of AlinOBP4 by reverse transcription polymerase chain reaction （RT-PCR） analysis, quantitative real-time PCR （qPCR） test and electroantennogram （EAG） assay. The combination of RT-PCR and qPCR analyses revealed that the levels of mes- senger RNA transcript were significantly reduced ~95% in AlinOBP4-siRNA-treated A. lineolatus males and ~75% in RNAi-treated females within 48 hours. It was found that there are different EAG responses between male and female bugs when the AlinOBP4 gene was silenced by RNAi. The EAGs of A. lineolatus to two plant volatiles, tride- canal and hexyl alcohol, were reduced 9.09% and 79.45% in RNAi-treated males, 62.08% and 62.08% in RNAi-treated females compared to the controls, separately. Antennae of RNAi-treated bugs showed significantly lower electrophysiological responses to four sex pheromone analogs, butyl butanoate, 1-hexyl butyrate, （E）-2-hexenyl butyrate and hexyl hexanoate. The EAG recordings were reduced 35.43%, 35.24%, 39.96% and 78.47% in RNAi-treated males and 64.52%, 18.13%, 36.88% and 49.52% in RNAi-treated females, respectively. The results suggested that AlinOBP4 might play dual-roles in the identifi- cation of plant volatiles and sex pheromones. It was suspected that AlinOBP4 may have different functions in odor perception between male and female A. lineolatus.
