Application value of dexmedetomidine in anesthesia for elderly patients undergoing radical colon cancer surgery

BACKGROUND Colon cancer presents a substantial risk to the well-being of elderly people *** advancements in medical technology,surgical treatment has become the primary approach for managing colon cancer ***,due to age-related physiological changes,especially a decline in cognitive function,older patients are more susceptible to the effects of surgery and anesthesia,increasing the relative risk of postoperative cognitive dysfunction(POCD).There-fore,in the surgical treatment of elderly patients with colon cancer,it is of pa-ramount importance to select an appropriate anesthetic approach to reduce the occurrence of POCD,protect brain function,and improve surgical success *** One hundred and seventeen patients with colon cancer who underwent elective surgery under general anesthesia were selected and divided into two groups:A and *** A received Dex before anesthesia induction,and B group received an equivalent amount of normal *** in the mini-mental state exami-nation,regional cerebral oxygen saturation(rSO2),bispectral index,glucose uptake rate(GluER),lactate production rate(LacPR),serum S100βand neuron-specific enolase(NSE),POCD,and adverse anesthesia reactions were compared between the two *** Surgical duration,duration of anesthesia,and intraoperative blood loss were comparable between the two groups(P>0.05).The overall dosage of anesthetic drugs used in group A,including propofol and remifentanil,was significantly lower than that used in group B(P<0.05).Group A exhibited higher rSO2 values at the time of endotracheal intubation,30 min after the start of surgery,and immediately after extubation,higher GluER values and lower LacPR values at the time of endotra-cheal intubation,30 min after the start of surgery,immediately after extubation,and 5 min after extubation(P<0.05).Group A exhibited lower levels of serum S100βand NSE 24 h postoperatively and a lower incidence of cognitive dysfunction on the 1st and

Knowledge Distillation via Hierarchical matching for Small Object Detection

Knowledge distillation is often used for model compression and has achieved a great breakthrough in image classification,but there still remains scope for improvement in object detection,especially for knowledge extraction of small *** main problem is the features of small objects are often polluted by background noise and not prominent due to down-sampling of convolutional neural network(CNN),resulting in the insufficient refinement of small object features during *** this paper,we propose Hierarchical matching Knowledge Distillation Network(HMKD)that operates on the pyramid level P2 to pyramid level P4 of the feature pyramid network(FPN),aiming to intervene on small object features before *** employ an encoder-decoder network to encapsulate low-resolution,highly semantic information,akin to eliciting insights from profound strata within a teacher network,and then match the encapsulated information with high-resolution feature values of small objects from shallow layers as the *** this period,we use an attention mechanism to measure the relevance of the inquiry to the feature *** in the process of decoding,knowledge is distilled to the *** addition,we introduce a supplementary distillation module to mitigate the effects of background *** show that our method achieves excellent improvements for both one-stage and twostage object ***,applying the proposed method on Faster R-CNN achieves 41.7%maP on COCO2017(ResNet50 as the backbone),which is 3.8%higher than that of the baseline.

Clinical features, treatment strategies and outcomes of craniocervical junction arteriovenous fistulas: a cohort study of 193 patients

Nitrogen(N) is an essential nutrient for food production. The rapid increase in population requires high inputs of N to meet the growing food demand. If not managed well, the substantial loss of N from the food system has multiple impacts on grain yield, air and water pollution, and the economic benefits of agricultural. Multi-objective(food security, environmental sustainability and economic sustainability) synergistic consideration of N management in the food system is still lacking. This study employed strategies for optimizing N management in the food system, using Quzhou County as a typical example on the North China Plain. Firstly, a food chain approach was adopted to understand drivers and reasons behind N losses from the food ***, a top-down approach was used to define multi-objective N management, taking into consideration food security, environmental sustainability and economic sustainability. Multi-objective N management aims to reduce N losses to the environment and increase N use efficiencies,while simultaneously increasing yields and economic benefits. Thirdly, 3R(reduce-retain-recycle) N management strategies were identified for specific crops and animals through a bottom-up approach and then analyzed the potential of these strategies to achieve the multi-objectives. Finally, there is a discussion of how to engage different stakeholders to promote the technologies implementation. This study provides new insights into the synergistic achievement of multi-objective N management in the food system and the development of environmentally-friendly agriculture.

Two-dimensional Cr_(2)Cl_(3)S_(3)Janus magnetic semiconductor with large magnetic exchange interaction and high-T_(C)

The two-dimensional(2D)Janus monolayers are promising in spintronic device application due to their enhanced magnetic couplings and Curie *** der Waals CrCl_(3) monolayer has been experimentally proved to have an in-plane magnetic easy axis and a low Curie temperature of 17 K,which will limit its application in spintronic *** this work,we propose a new Janus monolayer Cr_(2)Cl_(3)S_(3) based on the first principles *** phonon dispersion and elastic constants confirm that Janus monolayer Cr_(2)Cl_(3)S_(3) is dynamically and mechanically *** Monte Carlo simulation results based on magnetic exchange constants reveal that Janus monolayer Cr_(2)Cl_(3)S_(3) is an intrinsic ferromagnetic semiconductor with TC of 180 K,which is much higher than that of CrCl_(3) due to the enhanced ferromagnetic coupling caused by S ***,the magnetic easy axis of Janus Cr_(2)Cl_(3)S_(3) can be tuned to the perpendicular direction with a large magnetic anisotropy energy(maE)of 142eV/***,the effect of biaxial strain on the magnetic property of Janus monolayer Cr_(2)Cl_(3)S_(3) is *** is found that the Curie temperature is more robust under tensile *** work indicates that the Janus monolayer Cr_(2)Cl_(3)S_(3) presents increased Curie temperature and out-of-plane magnetic easy axis,suggesting greater application potential in 2D spintronic devices.
