Effects of Health Record Computerization in the Context of the COVID-19 Pandemic in Burundi’ Hospitals: Propensity Score Matching

Introduction: A great number of software are currently used to digitalize the patient records in order to optimize the quality of services offered to patients. The objective of this study was to evaluate the effects of Electronic Health Records use in Burundi’s hospitals, taking into account the COVID-19 pandemic context. Methods: This was a quasi-experimental study based on difference in difference method. Ten district hospitals were included in the study, five of them had the Electronic Health Records and five of which did not yet have the Electronic Health Records. The hospital’s control group were chosen using the propensity score matching method. The period before the project’s implementation was 2014 and the period after were 2019 and 2020. Results: After 5 years of the Electronic Health Record’s implementation, the results showed an increase in outpatient consultation (70%), deliveries (more than 100%), caesarean sections (56%) and major surgeries (43%) indicators. The overall quality score of hospitals’ care had a regressive effect of 37% and the income from performance-based funding had an increase by 31%. The indicators which were affected by the context of the COVID-19 pandemic were especially outpatient consultation, caesarian section, income from performance-based funding decreased by 3%, 5% and 20% respectively. Conclusion: The effects of Electronic Health Records use are effective. As the COVID-19 pandemic impacted the hospital’s indicators negatively, the resilient strategies alongside the potential shocks are recommended.

Evaluation of the Effectiveness of the 2019 School-Based Awareness Campaign on Road Driving in the Municipalities of Cotonou and Abomey-Calavi in Benin

Being the leading death cause among young people in the world, road accidents are avoidable through the adoption of simple attitudes and behaviors. Handicap International Non-Governmental Organization (NGO) has launched an awareness campaign in secondary schools in the municipalities of Cotonou and Abomey-Calavito bring young Beninese users to adopt good attitudes and safe behavior while driving on the road. This study aims at assessing the effectiveness of this awareness campaign. It was an evaluative cross-sectional here-elsewhere type study, held in fourteen high schools in the cities of Cotonou and Abomey-Calavi, of which seven benefited from the campaign and seven did not. In each school, students were selected on the basis of two-stage random sampling. 656 students, 309 of whom were sensitized and 347 of whom were not, were included in this study. The campaign was quite effective. Students, teachers, and school officials’ perceptions of the campaign were positive. In addition, the sensitized students had a better level of knowledge (p = 0.001) and reported safer behaviors (p = 0.003) while driving on the road than those who were not aware. Although the effects are positive, they are still very small. Efforts must be continued and much remains to be done.

Socio-Medical and Demographic Factors Associated with Perinatal Mortality in a Southern Benin Health Zone Hospital in 2020: A Case-Control Study

Background: Perinatal mortality remains a major public health concern in developing countries such as Benin. To better steer response interventions, this study was carried out in southern Benin with the aim of identifying the demographic and socio-medical factors linked to perinatal mortality. Methods: The case-control study, held from January 1 to December 31, 2020, covered 154 targets, including 77 cases of neonates deceased within the perinatal period and 77 live-born control neonates selected on a one-for-one basis. Univariate analysis using the McNemar test and logistic regression were used to identify risk factors for perinatal mortality, at a 5% threshold of significance. Results: Sahoué/mina ethnicities, only associated socio-demographic characteristic, increased the risk of perinatal death by 2.47 times (p = 0.008). Mother’s age, education, occupation, marital status, and household size were not associated (p > 0.05). Risk of death was increased 2.5 times when the mother was referred (p = 0.007), 3.3 times when she came from a hard-to-reach locality (p Conclusion: Although attention must also be paid to referral and access to care, this study identifies antenatal consultation as the main target of any intervention to reduce perinatal mortality.

Background: In accordance with its measles elimination strategic plan 2012-2020, the Democratic Republic of the Congo (DRC) organized a follow-up vaccination campaign against measles from October to December 2019 in 26 provinces. This study aims to establish the contribution of this supplementary vaccination campaign to protecting children against measles. Methods: The survey was carried out in November 2020 among households of the DRC, according to the 2018 revised version of the World Health Organization’s stratified cluster sampling method, using multiple stage sampling. It targeted 280 children aged 6 - 59 months per stratum or province, with 10 children in each of the 28 selected clusters. Data collection using tablets with centralized and real-time data processing was preceded by enumeration to refine the household sampling frame. Clusters and households were selected by random draw. Data collected with CS Pro 7 software were analyzed with SPSS, Epi info 7 and Excel software to determine indicators and make before-after comparisons using the McNemar test, at a precision threshold of 5%. Results: Of the 8535 surveyed children, 89.5% were vaccinatedduring the follow-up campaign and 81.6% were vaccinated before. Only 3.7% had correctly completed campaign vaccination cards. Estimated vaccination coverage increased from 80.8% before the campaign to 92.6% after the campaign (p 0.001). Vaccination coverage after campaign against measles improved in all provinces (p < 0.001) except Bas-Uele and Maniema. Thirteen provinces reached the national coverage target of 95%, compared to five before the campaign. The proportion of zero-dose children dropped significantly after this campaign from 19.2% to 7.4% (p 0.001), and even fell below 1% in six provinces. Conclusion: This measles vaccination campaign improved overall vaccination coverage by 10% and reached more unvaccinated children. Introduction: In serodiscordant couples, sharing serostatus is a crucial step in the HIV risk management process. The aim of this study was to determine the prevalence and factors associated with serostatus sharing among HIV serodiscordant couples in the city of Parakou. Setting and methods: This study was carried out in the city of Parakou, capital of northern Benin. It was a descriptive, analytical cross-sectional study with non-probability sampling for convenience. Socio-demographic data, as well as data on antecedents, behaviours, social and marital life, were collected using a questionnaire gave to the participants. Clinical and therapeutic data were collected from patients’ medical records and follow-up charts. Data were analyzed and processed using R 4.3.1 software. The significance level was set at 5%, and logistic regression was used to identify potential predictors of shared serostatus among serodiscordant couples in the city of Parakou in 2022. Results: A total of 299 participants in long-term serodiscordant relationships were included in our study. The average age of the subjects was 37.55 ± 10.25 years. Women were the infected partner in 80.27% of cases. The most common level of education was secondary (33.45%). Most respondents (230 subjects, 76.92%) were married. The average duration of the couple’s relationship was 121.68 months ± 96.32. The most frequent screening circumstances were: Prevention of mother-to-child transmission (PMTCT) (50.84%) and hospital screening (26.09%). The infected partner shared his or her serostatus with his or her spouse in 47.83% of cases. Potential predictors of serostatus sharing within the couple were: marital status (p Conclusion: The sharing of serostatus within serodiscordant couples needs to be managed in a way that takes into account the social and marital characteristics of the couple.

Introduction: The growing phenomenon of early pregnancies in Benin has adverse consequences on the lives of adolescent girls, impacting various social, health, economic, and educational aspects. This study aims to identify the factors associated with early pregnancies in the municipality of Bohicon in 2022. Materials and Methods: This is a case-control study conducted in Bohicon, involving 47 cases and 94 controls from 16 secondary and technical schools. Cases were selected exhaustively, while controls were randomly drawn from girls aged 10 to 19 in the same class who had never been pregnant. Matching was based on the type of institution and class. Data were collected through a questionnaire and analyzed using Epi info 7.***.1.0 and STATA 15 software, employing multiple conditional logistic regression (5% significance threshold) to identify factors related to early pregnancies. Results: The average age of adolescents was 17.63 years ± 0.09 for cases and 17.12 years ± 0.13 for controls. Associated factors included age over 16 years (p = 0.049), the marital status of the adolescent (single) (p = 0.001), and the monthly pocket money of the adolescent less than 10,000 FCFA (p = 0.031). Conclusion: Three factors related to early pregnancies were identified. To counteract this phenomenon and promote responsible sexuality among adolescents, it is essential to strengthen communication efforts aimed at modifying behaviors.

Factors Associated with Pregnancy Outcomes in Hypertensive Pregnant Women in a District Hospital in Benin

Introduction: Hypertension is a major cause of maternal and perinatal morbidity and mortality. This study aims at exploring the outcomes of pregnancy and associated factors in hypertensive pregnant women in Comè district hospital from 2014 to 2016. Methods: This is a retrospective, analytical study carried out in June 2016, involving a total of 345 hypertensive patients. Socio-demographic and medical features, including pregnancy and foetal outcomes parameters, were recorded in the case files. Top-down stepwise logistic regression was performed at the 5% threshold. Results: The frequency of hypertensive disorders during pregnancy was estimated at 7.32% and unfavourable outcomes at 35.65%. The adverse maternal outcomes recorded were postpartum death and haemorrhage whereas adverse foetal issues were prematurity, low birth weight, low Apgar scores, stillbirth and death. Factors statistically associated with pregnancy outcome were paucigravida (OR = 2.01 ([1.05 - 3.88]), p = 0.035), history of stillbirth (OR = 4.75 ([1.01 - 22.1]), p = 0.048) and anticonvulsant therapy (OR = 0.32 ([0.19 - 0.54]), p Conclusion: Adequate monitoring via Antenatal Care (ANC) check-ups, timely recourse to care and an effective communication strategy should reduce hypertensive disorders incidence and adverse outcomes in pregnant women.

Deep Learning(DL),a type of Machine Learning,has gained significant interest in many fields,including *** paper aims to shed light on deep learning techniques used in agriculture for abiotic and biotic stress detection in fruits and vegetables,their benefits,and the challenges faced by *** papers were collected from Web of Science,Scopus,Google Scholar,Springer,and Directory of Open Access Journals(DOAJ)using combinations of specific keywords such as:’Deep Learning’OR’Artificial Intelligence’in combination with fruit disease’,vegetable disease’,‘fruit stress',OR‘vegetable stress'following PRISMA *** the initial 818 papers identified using the keywords,132 were reviewed after excluding books,reviews,and the *** recovered scientific papers were from 2003 to 2022;93%addressed biotic stress on fruits and *** most common biotic stresses on species are fungal diseases(grey spots,brown spots,black spots,downy mildew,powdery mildew,and anthracnose).Few studies were interested in abiotic stresses(nutrient deficiency,water stress,light intensity,and heavy metal contamination).Deep Learning and Convolutional Neural Networks were the most used keywords,with GoogleNet(18.28%),ResNet50(16.67%),and VGG16(16.67%)as the most used ***-two percent of the data used to compile these models come from the fields,followed by data obtained *** problems due to unbalanced classes and the small size of some databases were also *** provided the research gaps and some perspectives from the reviewed *** research works are required for a deep understanding of the use of machine learning techniques in fruit and vegetable studies:collection of large datasets according to different scenarios on fruit and vegetable diseases,evaluation of the effect of climatic variability on the fruit and vegetable yield using AI methods and more abiotic stress studies.

目的:比较Super Path微创全髋关节置换术与直接前方入路(DAA)全髋关节置换术的临床效果。方法:2018年2月到2019年9月对江苏大学附属人民医院收住骨科的患者中,连续97例患者接受了单侧微创全髋关节置换术,采用了前瞻随机对照设计的方法,将患者分为两组,分别采用Super Path入路,或者采用DAA入路对以上患者进行手术。通过Harris髋关节功能评分(Harris hip score,HHS)、以及疼痛视觉模拟评分(VAS)来对比两组的临床疗效,并且将两组间的手术相关数据以及围手术期并发症进行对比。结果:Super Path组手术切口长度、手术时间以及首次下床时间都明显优于DAA组,对比两组失血量、输血率以及术后肢体长度差均无统计学差异;两组中Super Path组出现手术并发症较少。在Super Path组统计术后1个月、3个月、和6个月时HHS评分明显高于DAA组的评分(P0.05),在Super Path组术后6h、12h、和1周的VAS评分显示明显优于DAA组(P0.05)。结论:Super Path入路相比DAA入路切口更小,功能恢复更快,术后短期HHS评分功能更好,可以提供患者满意的临床结果。

Dear Editor,Monkeypox is an infectious disease that is endemic in a dozen of African *** imported cases have been also reported outside of Africa in the past[1].Since early May 2022,monkeypox infections including human-to-human transmission,were reported in a multi-country outbreak in non-endemic countries and declared Public Health Emergency of International Concern(PHEIC)by the World Health Organization(WHO)in July 2022[2].As of 20 September 2022,a total of at least 62,798 human cases of monkeypox with 20 deaths have been confirmed in 115 countries in five WHO regions[3].
