In this study, the wing design problem for different planforms for supersonic transport (SST) under supersonic and transonic cruise conditions is discussed to obtain knowledge of the supersonic air-foil from the viewpoint of wing planform dependency. Two types of planforms were considered—a cranked arrow wing with a high sweep-back angle and a tapered wing with a low sweep- back angle. The optimum airfoils of these planforms were designed by efficient global optimization, which combined the evolutionary algorithm with the Kriging surrogate model. To acquire design knowledge, the functional analysis of variance was applied to the solution space and the design space. The design results show that the optimum airfoil and the contribution ratios of design variables for the airfoils of the two planform are different.

Liaison conference for adolescent psychosomatic disease in pediatric ward

We held the liaison case conferences for two adolescent cases with psychosomatic disease in the pediatric ward to understand the patient's emotional problems and family situations, and to discuss how approach their psychological problems. We considered that the liaison conference played a useful role in both the clinical diagnosis and treatment. Furthermore, the liaison conference was useful for reducing the staff’s anxieties and for contributing to have a proper understanding for the disease. We suggest that a liaison conference should be implemented in pediatric units, for pediatric patients with emotional and behavioral problems.

Fibrous dysplasia is a benign osseous disease, although radical resection is commonly recommended because of the possibility of recurrence or malignant change. However, radical resection is undesirable, particularly in the maxillofacial region, because facial bones are difficult to replace in an aesthetically-pleasing manner after radical excision. In our department, conservative shaving, as reported by Chen and Noordhoff, is used to reduce tumor volume while maintaining facial aesthetics that are as normal as possible. We have experienced eight cases of craniomaxillofacial fibrous dysplasia and successfully applied conservative shaving to the maxillofacial lesions. These cases have exhibited no recurrence, and their facial contours have improved remarkably.

A 38-year-old female presenting with a high fever of 39 ℃ developed severe liver dysfunction and acute renal failure(ARF).In tests for a hepatitis associated virus,an Immunoglobulin M-anti-hepatitis B virus core antibody was the only positive ***,the progression of ARF coincided with the pole period of liver damage and all the other assumed causes for the ARF were ***,this case was diagnosed as ARF caused by acute hepatitis *** associated with non-fulminant hepatitis has been infrequently reported,usually in association with acute hepatitis *** case is considered to be an extremely rare and interesting case.

Backgrounds: Reconstruction after hemipelvectomy is very important for rehabilitation into society. The pelvis plays an important role for support of the intra-abdominal organs. Methods: We operated 3 cases using fillet flaps with the femoral periosteum for reconstruction of hemipelvectomy defect. Results: It is useful to elevate the flap all around with the femoral periosteum, because the periosteum can be sutured to supporting pelvic structures with the aim to support intra-abdominal organs. Conclusion: Without alternative supports for the bony pelvis, pelvic reconstructions are at risk for hernia and it may be difficult for outpatients to fit their habiliments after radical cure. The rigid support for the intra-abdominal organs occurs in association with the suture pelvic and femoral periosteum.

Complete surgical excision of venous malformations (VMs) is frequently complicated by massive bleeding and nerve damage, particularly in the cervicofacial area. This report describes a novel technique for the resection of cervicofacial VMs using the LigaSureTM (Valleylab, Tyco International Healthcare, Boulder, CO) vessel sealing system. We report three cases of VM excision using this vessel sealing system, and recorded intraoperative and postoperative blood loss, operative time, and postoperative complications. In all cases employing the LigasureTM, perioperative blood loss was less than 600 ml, no patients developed postoperative hematomas or other bleeding complications, and no patients developed a facial palsy even if the VM was located close to the facial nerve. LigaSureTM provides excellent hemostasis with few complications when used for cervicofacial VMs.

Amputation is frequently used to treat malignant tumors invading into the shoulder joint. When the vessels and major nerves of the upper arm are preserved, however, the limb may be salvaged with a combined osseous, myocutaneous flap. We reconstructed large defects, which included the shoulder joint, using a vascularized rib-latissmus dorsi combined flap. Ribs were connected via the ninth or tenth posterior intercostal artery. Several slits were cut in the ribs, and the rib was glass stick fractured to straighten it. The thoracodorsal nerve was preserved and the latissmus dorsi muscle was transferred to the defect previously occupied by the biceps humerus. The ribs were connected to the residual clavicle via a Leeds-Keio artificial ligament. The flaps completely engrafted. The patient was able to flex her elbow relatively early postoperatively. The function of the hand was preserved intact. In conclusion, the combined vascularized rib-latissmus dorsi flap is useful for patients who require functional or bony reconstruction of large defects involving the shoulder joint.

Rhinoplasty for Saddle Nose with Sarcoidosis

Background: Cases of acquired saddle nose have been reported;however, reports of sarcoidosis with acquired saddle nose are very rare. The main cause of sarcoidosis is estimated to be an abnormal immune reaction rather than infection. Aim: Rhinoplasty using an autologous rib was planned and performed without plate fixation. Case Presentation: Our patient was diagnosed with sarcoidosis at the age of 25 years. She developed nasal congestion and rhinitis at the age of 42 years. Her nose began to show the saddle nose deformity at the age of 43 years. A rhinoplasty using an autologous rib was performed without plate fixation. Conclusion: After the operation, her nose maintained a favorable shape without sarcoidosis worsening. This use of rhinoplasty for treating saddle nose possibly prevents the worsening of sarcoidosis.

AIM: To investigate whether the pro- and anti-inflammatory cytokine gene polymorphisms, IL1B-511C/T, IL1B-31C/T, IL6-634C/G, TNF-1031T/C, TNF-857C/T, and IL10-1082A/G, interact with smoking and drinking habits to influence infection with H ***: The subjects were 410 Japanese transit company employees. C-reactive protein and conventional cardiovascular risk factors were evaluated. Serum anti-H pylori antibodies were measured. The genotypes of IL1B-511C/T, IL1B-31C/T, IL6-634C/G, TNF-1031T/C, TNF-857C/T, and IL10-1082A/G polymorphisms were determined by allelic discrimination using fluorogenic probes and a 5′nuclease ***: In gender- and age-adjusted logistic analyses, the subjects with TNF-857T/T had a significantly lower odds ratio (OR) for H pylori seropositivity (reference -857C/C; OR = 0.15, 95% CI: 0.03-0.59, P = 0.007). After stratification according to smoking and drinking status, among never-smokers, the subjects with IL1B-511C/T had a significantly lower OR (reference -511C/C; OR = 0.30, 95% CI: 0.10-0.90, P = 0.032). Among drinkers in the 1-5 times/wk category, the subjects with IL1B-511T/T had a significantly lower OR (reference C/C; OR = 0.38, 95% CI: 0.16-0.95, P = 0.039), and the subjects with IL1B-31C/T and T/T had a significantly higher OR (reference C/C; C/T: OR = 2.59, 95% CI, P = 0.042: 1.04-6.47; C/C: OR = 3.17, 95% CI: 1.23-8.14, P = 0.017). Among current smokers, the subjects withIL6-634C/G had a significantly higher OR (reference C/C; OR = 2.28, 95% CI: 1.13-4.58, P = 0.021). However, the interactions terms between the aforementioned genotypes and lifestyles were not statistically ***: Contrary to previous findings, the results herein suggest that the TNF-857T/T genotype may be protective against chronic infection with H pylori. Drinking and smoking habits may influence the effect of cytokine gene polymorphisms. Further studies are required to clarify the effects of the pro- and anti-inflammatory cytokine
