Urbanization and Quality of Urban Environment Using Remote Sensing and GIS Techniques in East Delhi-India

An explosive increase of urban population, practically in all major cities and towns, has the consequent strain on the existing system manifested in an environmental chaos. The phenomena of accelerated urbanisation is the main culprit, wherein besides bringing higher standard of living, it has also brought problems, as growth of dense and unplanned residential areas, environmental pollution, non-availability of services and amenities, solid waste etc. Remote sensing satellite data is suitable for urban land use mapping to get detail and up- to-date information for environmental management. Where as GIS helps in developing database system for urban information, which supports decision making process. Development of digital database on all aspects of land use and urban planning is the next crucial task for the future in which remote sensing based informa-tion is going to play a major role. In Delhi, rise in population and growth in economic activity has led to en-vironmental degradation. With this view an attempt has been made to study the quality of urban environment in the East district of Delhi, which is experiencing very high urban growth with 98.75% urban population in 2001. For this study Landsat ASTER (MSS) data of year 2001 (15 m Ground resolution), Guide map of the year 1982 and demographic and environmental data has been used. Eight parameters were selected, which affect the urban environmental quality, namely built-up area, open spaces, household density, occupancy ra-tio, population density, accessibility to roads, noise and smell affected area. The study shows that the quality of environment has been degraded when we compare 1982 and 2003 data. Most of the East district was in a better state of environment in 1982, but in 2003 things have been changed and now 50% area is in very good, fair and desirable condition. We read with great interest the article, ＂Non-alcoholic fatty liver disease may not be a severe disease at presentation among Asian Indians＂ by Madan et at in the recent issue of WJG. Twenty-eight （55%） out of 51 patients with nonalcoholic fatty liver disease （NAFLD） who presented with abnormal transaminases had histological evidence of nonalcoholic steatohepatitis （NASH）. The majority of patients had grade 1 [32 （63%）] or grade 2 [16 （31%）] inflammation and either had no [23 （45%）] fibrosis or stage I [19 （37%）] fibrosis. None of the patients had cirrhosisf11. We agree with Madan et al that Asian Indians with NAFLD who present with unexplained increase in transaminases may have mild disease at presentation on the basis of similar observations made by us. NAFLD has a spectrum which includes patients with only steatosis and NASH that can progress to cirrhotic and hepatocellular carcinoma.

The main objective of the present investigation is to study the vibration of visco-elastic parallelogram plate whose thickness varies parabolically. It is assumed that the plate is clamped on all the four edges and that the thickness varies parabolically in one direction i.e. along length of the plate. Rayleigh-Ritz technique has been used to determine the frequency equation. A two terms deflection function has been used as a solution. For visco-elastic, the basic elastic and viscous elements are combined. We have taken Kelvin model for visco-elasticity that is the combination of the elastic and viscous elements in parallel. Here the elastic element means the spring and the viscous element means the dashpot. The assumption of small deflection and linear visco-elastic properties of “Kelvin” type are taken. We have calculated time period and deflection at various points for different values of skew angles, aspect ratio and taper constant, for the first two modes of vibration. The aim of present study was to use QbD approaches to evaluate the effect of independent product variables and their interaction on particle size of sodium fluoride and then obtain the optimized experimental condition for predefined particle size of sodium fluoride. The sodium fluoride is mainly used in dental preparation for delivering the fluoride ion to the tooth enamel for that nano-particle size is required. Nowadays the milling process is used to reduce the particle size. But that process has some limitations due to crystalline nature of sodium fluoride;for overcoming those limitations, lyophilization method is used. A 43 level full factorial design was used to study the significant influence of process and product variables i.e. 1) Concentration of sodium fluoride, 2) Concentration of PVP, 3) Sample volume, 4) Drying surface, on particle size of sodium fluoride. The experimental design result shows that independent product variables significantly modify the structure and improve particle size reduction of sodium fluoride.

AIM: The pathogenesis of occurrence of liver inflammation and fibrosis in patients with nonalcoholic steatohepatitis (NASH) is not completely understood. Other than insulin resistance, iron abnormalities have been thought to be one of the triggering factors. Therefore, our aim was to study the role of iron abnormalities and HFE gene mutations in patients with NASH. METHODS: Thirty-one patients of NASH diagnosed on the basis of clinical examination biochemistry, ultrasonography and liver biopsy (n = 14) were included in the study. Serum iron parameters (n = 23) (iron, ferritin, total iron-binding capacity and transferrin saturation), Perls' iron staining on liver biopsies (n = 14) and HFE gene mutations (C282Y and H63D) (n = 16) were studied in these patients. The association between iron staining, necroinflammatory activity and fibrosis stage on liver biopsies was also determined. RESULTS: Elevated serum iron, ferritin and transferrin saturation above 55% were observed in 4.3% of patients. On histology, 71% of the patients had negative iron staining, 21.4% had 1+ staining, 7.2% had 2+ staining and none had 3+ or 4+ staining. There was no association between the degree of iron staining and necroinflammatory activity (P=0.55) and fibrosis stage (P= 0.09) on histology. None of the patients had C282Y HFE gene mutation and four patients (25%) were found to be heterozygotes for H63D gene mutation. CONCLUSION: Our study does not favor iron overload and HFE gene mutations as major factors in the pathogenesis of NASH in Asian Indians.

AIM: To assess the usefulness of contrast-enhanced harmonic endoscopic ultrasonography (CH-EUS) for lymph node metastasis in pancreatobiliary ***: All patients suspected of pancreatobiliary carcinoma with visible lymph nodes after standard EUS between June, 2009 and January, 2012 were enrolled. In the primary analysis, patients with successful EUS-fine needle aspiration (FNA) were included. The lymph nodes were assessed by several standard EUS variables (short and long axis lengths, shape, edge characteristic and echogenicity), color Doppler EUS variable [central intranodal blood vessel (CIV) presence] and CH-EUS variable (heterogeneous/homogeneous enhancement patterns). The diagnostic accuracy relative to EUS-FNA was calculated. In the second analysis, N-stage diagnostic accuracy of CH-EUS was compared with EUS-FNA in patients who underwent surgical ***: One hundred and nine patients (143 lymph nodes) fulfilled the criteria. The short axis cut-off ≥ 13 mm predicted malignancy with a sensitivity and specificity of 72% and 85%, respectively. These values were 72% and 63% for the long axis cut-off ≥ 20 mm, 62% and 75% for the round shape variable, 81% and 30% for the sharp edge variable, 66% and 61% for the hypoechogenicity variable, 70% and 72% for the CIV-absent variable, and 83% and 91% for the heterogeneous CH-EUS-enhancement variable, respectively. CH-EUS was more accurate than standard and color Doppler EUS, except the short axis cut-off. Notably, three patients excluded because of EUS-FNA failure were correctly N-staged by ***: CH-EUS complements standard and color Doppler EUS and EUS-FNA for assessment of lymph node metastases.

Diagnosis and management of laryngeal cleft: a single centre experience and a novel endoscopic technique

AIM： To examine the presentation, diagnosis and out-comes of patients with laryngeal ***： An 18 mo （from mid-2012 to 2013） pro-spective longitudinal study was performed at the Barts Children’s and Royal London Hospital, a tertiary referral centre. Chart review was performed for all patients including data extraction of demographics, outpatient clinic review documentation, speech therapy findings, medication list, operative findings alongside technique and follow up. A systematic review of contemporary English medical literature was also reviewed to compare series. The study was approved and registered by the hospital clinical governance and audit board. Biosta-tistician review was not ***： Twenty-two children aged 1 to 72 mo （mean age 23.5 mo） with a 7：4 male-female ratio. Twenty had Benjamin-Evans type 1 clefts and 2 had a type 2 cleft. All were symptomatic despite medical management including anti-reflux therapy. Patients presented with dyspnoea （81%）, feeding diffculty （63%）, stridor （54%） and recurrent pneumonia （36%）. Several patients had concomitant aerodigestive abnormalities including 7 with laryngomalacia, 4 subglottic stenosis, 2 subglottic webs and 1 tracheo-oesophageal fstula. To date, 18 patients have undergone endoscopic repair, all of whom have shown radiological and/or clinical signs of improvement. All endoscopic repairs were performed with the novel use of a Negus knot pusher, with Baby Benjamin rigid suspension, to more reliably and easily suture at ***： This is a significant single unit series demonstrating the strong association of laryngeal cleft with combined aerodigestive symptoms and other laryngeal abnormalities. Endoscopic management of type 1 and 2 laryngeal clefts is successful. We recommend the use of a Negus knot pusher to facilitate endoscopic repair.

Maternal dystocia in cows and buffaloes: A Review

The maternal causes of dystocia in cattle and buffaloes are analyzed. Uterine torsion appears to be the most frequent maternal cause of dys-tocia in buffaloes whereas improper cervical dilation appears to be more frequent maternal cause of dystocia in cattle. Failure of uterine expulsive forces (Uterine Inertia) and neo-plasm’s of vagina, vulva and uterus are com-monly seen in cows and buffaloes. The various maternal causes of dystocia in cattle and buf-faloes and their management are described.
