The effects of stent porosity on the endovascular treatment of intracranial aneurysms located near a bifurcation

Intracranial aneurysm occurs when a cerebral artery develops an abnormal sac-like dilatation, and will cause massive bleeding in the subarachnoid space upon rupture. Endovascular stenting is a minimally invasive procedure in which a flow-diverting stent is deployed to cover the aneurysm neck, thereby restricting blood from entering the aneurysm and reducing the risk of rupture. The stent porosity, a crucial factor determining the intra-aneurysmal hemodynamics following treatment, is investigated by computational fluid dynamics techniques. Based on the computational results, a low porosity stent will dramatically reduce the flow velocity and the flow rate inside the side branch vessel. Conversely, a high porosity stent may not provide adequate flow reduction inside the aneurysm, possibly causing treatment failure. An advisable range of optimal stent porosity would be 60% to 75%, which can drastically reduce the flow rate into the aneurysm while preserving enough blood flow for the side branch vessel. Clinically, deployment of two or more flow-diverting stents may not increase treatment efficacy but can potentially lead to adverse effects due to side-branch hypoperfusion. The present quantitative analysis can also provide practical insight for future stent design.

Acupuncture transcutaneous electrical nerve stimulation reduces discomfort associated with barostat-induced rectal distension:A randomized-controlled study

AIM:To explore the effectiveness of acupuncture transcutaneous electrical nerve stimulation(Acu-TENS), a non-invasive modality in reduction of rectal discomfort during barostat-induced rectal distension. METHODS:Forty healthy subjects were randomized to receive 45 min of either Acu-TENS or placebo-TENS(no electrical output)over acupuncture points Hegu(largeintestine 4),Neiguan(pericardium 6)and Zusanli(stomach 36).A balloon catheter attached to a dual-drive barostat machine was then inserted into the subjects'rectum.A step-wise(4 mmHg)increase in balloon pressure was induced until maximal tolerable or 48 *** analogue scale and a 5-point subjective discomfort scale(no perception,first per-ception of distension,urge to defecate,discomfort/ pain and extreme pain)were used to assess rectal discomfort at each distension *** beta-endorphin levels were measured before,immediately after intervention,at 24 mmHg and at maximal toler- able distension pressure. RESULTS:There was no difference in the demographic data and baseline plasma beta-endorphin levels between the two *** threshold levels were higher in the Acu-TENS group when compared to the placebo group,but the difference reached statistical significance only at the sensations"urge to defecate"and"pain".The distension pressures recorded at the"urge to defecate"sensation for the Acu-TENS and placebo-TENS groups were 28.0±4.5 mmHg and 24.6±5.7 mmHg,respectively(P=0.043);and the pressures recorded for the"pain"sensation for these two groups were 36.0±4.2 mmHg and 30.5± 4.3 mmHg respectively(P=0.002).Compared to the placebo group,a higher number of participants in the Acu-TENS group tolerated higher distension pressures (>40 mmHg)(65%in Acu-TENS vs 25%in placebo, P=0.02).The plasma beta-endorphin levels of the Acu-TENS group were significantly higher than that of the placebo group at barostat inflation pressure of 24 mmHg(1.31±0.40 ng/mL vs 1.04±0.43 ng/mL,P= 0.044)and at maximal inflation p

BACKGROUND Cetuximab in combination with oral fluoropyrimidine(FP)remains controversial in metastatic colorectal cancer(mCRC).In view of the regional variation in the tolerability of FP,we conducted a retrospective analysis to compare oral FP with infusional FP in combination with cetuximab in Chinese *** To compare the efficacy and safety profile of cetuximab in combination with oral FP and infusional FP in Chinese population in the real-world *** A retrospective cohort study was done to analyse consecutive patients with Kras wild-type mCRC who received first-line treatment with cetuximab and FP-based chemotherapy in our unit from January 2010 to December ***-five eligible patients were *** median follow-up of our cohort was 65.0 *** The median progression-free survival(mPFS)and median overall survival(mOS)of the entire cohort were 9.66 mo(95%CI:7.72–12.5)and 25.8 mo(95%CI:18.7–35.6),*** oral FP and infusional FP,there was no statistical significant difference in the mPFS[9.79 mo(95%CI:7.49–12.7)vs 9.63 mo(95%CI:6.34–13.4);P=0.72]and mOS[25.8 mo(95%CI:15.2–35.6)vs 26.3 mo(95%CI:18.7–41.2);P=0.63].Grade 3 or above adverse events were reported in 28.4%of patients,being similar with oral and infusional FP,and included 10.5%of neutropenia and 2.1%of diarrhoea *** The current analysis demonstrates comparable efficacy and safety profiles of cetuximab in combination with oral and infusional FP in Chinese *** results expand treatment options for Chinese patients and invite revision of existing treatment guidelines to incorporate oral FP-based chemotherapy plus cetuximab.

采用反应性加工制备了环氧树脂（EP）和马来酸酐接枝氢化苯乙烯／乙烯- 丁烯／苯乙烯嵌段共聚物（MA-SEBS）同时改性的尼龙6／玻璃纤维复合材料,研究了EP和MA—SEBS对复合材料结构与力学性能的影响。结果表明在尼龙6／GF 复合材料中,单独加入环氧树脂或10％或20％的MA—SEBS,尼龙6／GF复合材料呈脆性断裂。但同时添加EP和MA—SEBS,当MA—SEBS和EP含量分别为10％和1份或MA—SEBS和EP含量分别为20％和0．67份时,复合材料的断裂伸长率和缺口冲击强度显著增加,EP和MA—SEBS对尼龙6／GF复合材料有协同增韧作用,其原因是EP和MA—SEBS为尼龙6／GF复合材料提供了一个粘接强度适中、易形变的柔性界面。

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Pseudobacteremia with amoxycillin-clavulanic acid-resistant Escherichia coli traced to cross-contamination during blood culture processing

Escherichia coli has seldom been reported to cause pseudobacteremia. The investigation of an outbreak of amoxycillin clavulanic acid resistant E. coli pseudobacteremia is described. Seventeen cases occurred over a five day period. The source of the E. coli was traced to the blood culture specimen of a patient (index patient) with genuine bacteremia as a result of urinary tract infection. The other 16 case patients had pseudobacteremia which was found to be the result of cross contamination during subculture of blood specimens. The E. coli strain was carried over from the culture bottle of the index patient, through the contaminated gloved hands of a technician to the culture bottles of the other 16 cases. Although the pseudobacteremia occurred over a five day period, they all resulted from cross contamination during blood culture processing within one day. An early outbreak investigation was prompted by the unusual finding of amoxycillin clavulanic acid resistance in the case E. coli isolates in a short period. The relatedness of the E. coli strains from the 17 cases was confirmed by arbitrary primed polymerase chain reaction. Clinicians should be alerted to the possibility of a blood E. coli isolate being a contaminant despite its predominant role as a true pathogen.
