Relationship between intralobar pulmonary sequestration and type A aortic dissection:A case report

BACKGROUND Pulmonary sequestrations often lead to serious complications such as infections,tuberculosis,fatal hemoptysis,cardiovascular problems,and even malignant degeneration,but it is rarely documented with medium and large vessel vasculitis,which is likely to result in acute aortic *** SUMMARY A 44-year-old man with a history of acute Stanford type A aortic dissection status post-reconstructive surgery five years *** contrast-enhanced computed tomography of the chest at that time had also revealed an intralobar pulmonary sequestration in the left lower lung region,and the angiography also presented perivascular changes with mild mural thickening and wall enhancement,which indicated mild *** intralobar pulmonary sequestration in the left lower lung region was long-term unprocessed,which was probably associated with his intermittent chest tightness since no specific medical findings were detected but only positive sputum culture with mycobacterium avium-intracellular complex and *** performed uniportal video-assisted thoracoscopic surgery with wedge resection of the left lower *** over the parietal pleura,engorgement of the bronchus due to a moderate amount of mucus,and firm adhesion of the lesion to the thoracic aorta were histopathologically *** We hypothesized that a long-term pulmonary sequestration-related bacterial or fungal infection can result in focal infectious aortitis gradually,which may threateningly aggravate the formation of aortic dissection.

Effect of Selenium Supplementation on Activity and mRNA Expression of Type 1 Deiodinase in Mice With Excessive Iodine Intake

Objective To investigate the effect of selenium supplementation on the selenium status and selenoenzyme, especially the activity and mRNA expression of type 1 deiodinase （D1） in mice with excessive iodine （EI） intake and to explore the mechanism of selenium intervention on iodine-induced abnormities. Methods Weanling female BALB/c mice were given tap water or 3 mg/L of iodine or supplemented with 0.5 mg/L or 1.0 mg/L of selenium in the presence of excessive iodine for 5 months. Selenium status, thyroid hormone level, hepatic and renal D 1 activity and mRNA expression were examined. Results Excessive iodine intake significantly decreased the selenium concentration in urine and liver, and the activity of glutathione peroxidase （GSH-Px） in liver. Meanwhile, serum total T4 （TT4） increased while serum total T3 （TT3） decreased. Hepatic D1 enzyme activity and mRNA expression were reduced by 33% and 86%, respectively. Renal D1 enzyme activity and mRNA were reduced by 30% and 55%, respectively. Selenium supplementation obviously increased selenium concentration, activity of GSH-Px and D1 as well as mRNA expression of D1. However, increasing the supplementation of Se from 0.5 to 1.0 mg/L did not further increase selenoenzyme activity and expression. Conclusion Relative selenium deficiency caused by excessive iodine plays an essential role in the mechanism of iodine-induced abnormalities. An appropriate dose of selenium supplementation exercises a beneficial intervention.

Intrathoracic caudate lobe of the liver:A case report and literature review

Heterotopic supradiaphragmatic livers are rare. A total of 23 cases of primary supradiaphragmatic livers have been reported in the literature. The clinical presentations of heterotopic supradiaphragmatic liver are variable. The simultaneous detection of intrathoracic accessory liver and pulmonary sequestration is extremely rare, and only one case has previously been reported. It is difficult to make a correct diagnosis preoperatively. We presented a 53-year-old woman with complaints of an intermittent, productive cough and dyspnea for two months that was refractory to medical treatment. She had no previous history of trauma or surgery. A chest radiograph only showed a widening of the mediastinum. Contrast-enhanced computed tomography of the chest revealed a well-circumscribed homogenous soft-tissue mass, approximately 4.35 cm × 2.5 cm × 6.14 cm in size, protruding through the right diaphragmatic crura to the right pleural cavity, attached to the inferior vena cava, esophagus and liver. There was no conclusive diagnosis before surgery. After the operation, we discovered that this patient was the first case of a supradiaphragmatic heterotopic liver, which passed through the inferior vena cava foramen and was coincidentally combined with an intralobar pulmonary sequestration that was found intraoperatively. We discussed its successful management with surgical resection via a thoracic approach and reviewed the published literature.

Developing efficient and long wavelength sensitive unimolecular photoinitiators(PIs)is still facing a great *** this work,a series of thioxanthone-based N-hydroxyphthalimide esters(TX-NHPIEs)were synthesized by installing NHPIEs along the TX backbone and *** investigated TX-NHPIEs have a 60 nm redshift and demonstrate sterling initiating efficiency for free radical photopolymerization(FRP)under LED@450 nm light irradiation compared with the commercialized isopropylthioxanthone(ITX).Real-time1Hnuclear magnetic resonance(1H NMR),electron spin resonance(ESR),decarboxylation and gas chromatograph-mass spectrometer(GC–MS)experiments and density functional theory(DFT)reveal that TX-NHPIEs can generate one alkyl radical and one N-centered iminyl radical,which can initiate FRP directly and indirectly,*** other words,TX-NHPIEs absorb one photon and can generate two active radicals,which break through the limitations of common ***-NHPIE-Cpe demonstrates the highest initiating efficiency,and its application in coatings and 3D printing was also studied,indicating TX-NHPIEs have broad potential applications in photopolymerization processes.

Towards accurate mapping of loess waterworn gully by integrating google earth imagery and DEM using deep learning

Accurate mapping of loess waterworn gully(LWG)is essential to further study gully erosion and geomorphological evolution for the Chinese Loess Plateau(CLP).Due to the vertical joint and collapsibility of loess,LWGs have the characteristics of zigzag and unique slope abruptness under synthetic action of hydraulic force and *** forces existing LWG mapping methods to either focus on the improvement of mapping accuracy or center on the increase of mapping ***,simultaneously achieving accurate and efficient mapping of LWG is still in its infancy under complex topographic ***,we proposed a method that innovatively integrates the loess slope abruptness feature into an improved deep learning semantic segmentation framework for LWG mapping using 0.6 m Google imagery and 5 m DEM *** selected four study areas representing typical loess landforms to test the performance of our *** proposed method can achieve satisfactory mapping results,with the F1 score,mean Intersection-over-Union(mIoU),and overall accuracy of 90.5%,85.3%,and 92.3%,*** addition,the proposed model also showed significant accuracy improvement by inputting additional topographic information(especially the slope of slope).Compared with existing algorithms(Random forests,original DeepLabV3+,and Unet),the proposed approach in this study achieved a better accuracy-efficiency ***,the method can ensure high accuracy and efficiency of the LWG mapping for different loess landform types and can be extended to study various loess gully mapping and water and soil conservation.

Gene therapy for monogenic disorders: challenges, strategies, and perspectives

Monogenic disorders refer to a group of human diseases caused by mutations in single genes. While disease-modifying therapies have offered some relief from symptoms and delayed progression for some monogenic diseases, most of these diseases still lack effective treatments. In recent decades, gene therapy has emerged as a promising therapeutic strategy for genetic disorders. Researchers have developed various gene manipulation tools and gene delivery systems to treat monogenic diseases. Despite this progress, concerns about inefficient delivery, persistent expression, immunogenicity, toxicity, capacity limitation, genomic integration, and limited tissue specificity still need to be addressed. This review gives an overview of commonly used gene therapy and delivery tools, along with the challenges they face and potential strategies to counter them.

A group of massive galaxies at redshifts of z■7 have been recently detected by the James Webb Space Telescope(JWST),which were unexpected to form at such an early time within the standard Big Bang *** this work,we propose that this puzzle can be explained by the presence of some primordial black holes(PBHs)with a mass of~1000M_(⊙).These PBHs act as seeds for early galaxy formation with masses of~10^(8)-10^(10)M_(⊙)at high redshift,hence accounting for the JWST *** use a hierarchical Bayesian inference framework to constrain the PBH mass distribution models,and find that the Lognormal model with the M_(c)~750M_(⊙)is preferred over other *** rapidly growing BHs are expected to have strong radiation and may appear as high-redshift compact objects,similar to those recently discovered by *** we focused on PBHs in this work,the bound on the initial mass of the seed black holes remains robust even if they were formed through astrophysical channels.

Optimizing FeCoNiCrTi high-entropy alloy with hydrogen pumping effect to boost de/hydrogenation performance of magnesium hydride

The exploration of efficient,long-lived and cost-effective transition metal catalysts is highly desirable for the practical hydrogen storage of magnesium hydride(MgH_(2)) in sustainable energy ***,FeCoNiCrTi high-entropy alloy(HEA) nanosheets were prepared via a facile wet chemical ball milling strategy and they were introduced into MgH_(2) to boost the hydrogen storage *** refined HEA exhibited superior catalytic activity on MgH_(2).In contrast to additive-free MgH_(2),the initial desorption temperature of the constructed MgH_(2)-HEA composite was reduced from 330.0 to 198.5℃ and a remarkable 51% reduction in the dehydrogenation activation energy was ***,the MgH_(2)-HEA composite only required one-twentieth time of that consumed by pure MgH_(2) to absorb 5.0 wt% of H_(2) at 225℃.The synergy between the "hydrogen pumping" effect of Mg_2Ni/Mg_2NiH_4 and Mg_2Co/Mg_2CoH_5 couples,as well as the good dispersion of Fe,Cr and Ti on the surface of MgH_(2) contributed to the enhanced de/hydrogenation performance of the MgH_(2)-HEA *** study furnishes important steering for the design and fabrication of multiple transition metal catalysts and may push the commercial application of magnesium-based hydrides one step forward.

Particle Distribution Informed by Chain Rigidity in Diblock Copolymer Melts: The Effect of Entropy

We study the effect of chain rigidity on tailoring the nanoparticle locations for neutral and selective particles embedded in the lamellar morphology formed by semiflexible diblock copolymer chains using self-consistent field calculations. The nanoparticles are modeled through a cavity function, and the semiflexible chains are represented by the continuous Kratsky-Porod chain model. In general situation, the nanoparticles prefer to stay at the interface in order to reduce the interface areas and thus the system free energy. However, the particle distribution at the domain center is subtle, and the underlying physics is intrinsically different depending on the polymer flexibility. In the case of flexible chains, the entropy just contributes a constant shift to the free energy when the nanoparticles move around the domain center indicating that the local metastable state if appears at the domain center is wholly attributed to the local minimum in the enthalpy. If the polymers are rigid, the variation of the particle distribution at the domain center has a close relation with the polymer rigidity and nanoparticle size. In the case of strongly rigid polymers with small nanoparticles, a nearly uniform particle distribution at the domain center is observed, while in other cases, a local enhancement of particle distribution there is found. In contrast to the case of flexible chains, further analysis reveals the crucial role of entropy in controlling the shape of particle distributions at the phase domain. Specifically, the local metastable state appears in the domain center is determined by the large entropy there which arises from the weak coupling of bond orientations that allows the polymer chains to be relatively relaxed. When the particle becomes selective, its distribution in the phase domain exhibits a shift almost uniformly rather than changes its profile, and the underlying physics still holds.
