Clinical Performance of Cell-Free Fetal DNA Testing for Fetal Aneuploidies and Subchromosomal Deletions/Duplications in a Cohort of 19,531 Pregnancies

Objective:We aim to assess the clinical performance of cell-free fetal DNA(cffDNA)testing for detecting common fetal aneuploidies as well as subchromosomal deletions/duplications and explore the pregnancy decisions in screen-positive ***:A cohort of 19,531 pregnant women was offered cffDNA testing for detection of trisomies 21,18,and 13(T21,T18,and T13);sex chromosome aneuploidies(SCAs);and subchromosomal deletions/***-positive cases were confirmed by karyotyping and single-nucleotide polymorphism array ***:A total of 47 cases failed the *** overall screen-positive rate of chromosomal abnormalities was 1.07%(208/19,484),including 57 cases with T21,18 cases with T18,7 cases with T13,106 cases with SCAs,and 20 cases of subchromosomal deletions/*** predictive values were 91.30%(42/46),38.46%(5/13),33.33%(2/6),41.33%(31/75),and 27.78%(5/18),*** was no significant difference in the screening of fetal chromosomal aneuploidies in the high-risk group compared with the low-risk group(P>0.05).All of the pregnant women who had confirmed fetal T21,T18,or T13 terminated their pregnancies,except for a case of T13 mosaic,whereas 45.16%(14/31)of women with fetal SCAs continued their ***,17 pregnant women with positive screens for T21,T18,or T13 without a subsequent diagnosis chose to terminate their pregnancy,whereas 29 of 31 women with SCAs chose to continue their ***:CffDNA testing exhibited good screening accuracy for T21,T18,and T13 and also contributed to detecting fetal SCAs and subchromosomal deletions/*** women with fetal 47,XXX or 47,XYY were more willing to terminate their pregnancy than those with fetal 45,X or 47,XXY.

Background, Motivation and Objective Green energy has long been a hot topic to solve the energy problem which is induced by vast consumption of traditional energy. An alternative efficient way is to utilize piezoelectric materials to capture mechanical energy from environment. The paper focuses mainly on energy harvesting from the deformation of asphalt pavement under the action of traffic load from the theoretical and experimental aspects, and in order to provide some useful advises for the energy harvesting system designing. Statement of Contribution/Methods To study the energy harvesting from the asphalt pavement, the paper gives some the theoretical model. At first, a laminated beam model of plane strain state is proposed wherein a piezoelectric layer is embedded between two asphalt layers. Later, to consider a more practical situation of using distributed piezoelectric materials for energy harvesting in road pavement, a Timoshenko asphalt beam model with embedded piezoelectric patches is proposed and solved analytically using the state space method. Then, the Winkler foundation model is introduced into the above two models to account for the effects of the under beneath stable and subgrade layers of a practical road pavement. Moreover, a series lab experiments are performed to measure the electric outputs when the pavement is subject to a dynamic load. The wheel tracking test is proposed wherein piezoelectric bimorphs are embedded in a square asphalt specimen, that is further loaded using a back-and-forth rolling wheel which is used to simulate the moving traffic load. Finally, a simple compression model is proposed to study the physical mechanism of the wheel tracking test. Results The harvested electric power under harmonic motion is found to depend on various material and geometrical parameters, electric resistive, as well as on the foundation stiffness in the harvesting circuit. Background, Motivation and Objective Implantable electronic devices(IEDs), which aim to solve problems such as heart failure, obesity, blood pressure control, neural-stimulation and drug delivery, have made significant progress in clinical applications in recent years. However, the currently available battery technologies are developing relatively slowly and imped the pace of improvement in design and functionality of IEDs. Self-powered implantable devices provide a potential way to address the shortcomings of battery replacement problem with its breakthrough energy harvesting technology. The flexible electronic devices with piezoelectric materials have been employed to harvest energy from the motion of organs in recent researches. Experiment results show that the output voltage of the device mounted on porcine left ventricle(LV) when chest is closed decreases significantly compared with chest open in special cases. We deliberately analyze the underling mechanisms of the reduction of output electric power and provide guidelines for the optimal design of in vivo piezoelectric energy harvesting from motions of biological organs. Statement of Contribution/Methods A restricted-space deformation model is presented to simulate the impeding effect of the surrounding tissues to the deformation of the flexible device analytically, and the post-buckling deformation of the flexible device is solved by employing energy method and variation principle. Then the theoretical output voltage is calculated by using piezoelectric effect and the numerical results are compared with the experimental results. The effect of the respiration is also considered. At last, a scaling law is established to analyze the intrinsic relationship between normalized output electric power and other experimental parameters. Do the lungs contribute to propofol elimination in patients during orthotopic liver transplantation without veno-venous bypass?

BACKGROUND: The clearance of propofol is very rapid, and its transformation takes place mainly in the liver. Some reports indicated extrahepatic clearance of the drug and that the lungs are the likely place where the process occurs. This study was undertaken to compare the plasma concentrations of propofol both in the pulmonary and radial arteries after constant infusion during the dissection, anhepatic and reperfusion phases of orthotopic liver transplantation (OLT) without veno-venous bypass, attempting to investigate extrahepatic clearance and to determine whether the human lungs take part in the elimination of propofol. METHODS: Fifteen patients undergoing OLT without veno-venous bypass were enrolled in the study, and propofol was infused via a forearm vein at a rate of 2 mg· kg-1·h-1. Blood samples were simultaneously collected from pulmonary and radial arteries at the end of the first hepatic portal dissection (T0), at the clamping of the portal vein (T1), 30, and 60 minutes after the beginning of the anhepatic phase (T2, T3), and 30, 60, and 120 minutes after the unclamping of the new liver (T4, T5, T6). Plasma propofol concentrations were measured using a reversed- phase, high-performance liquid chromatographic method with fluorescence detection. RESULTS: The concentrations of plasma propofol in the pulmonary and radial arteries at T2 and T3 rose significantly compared with T0 and T1 (P<0.01) respectively. After reperfusion, the drug concentrations at T4, T5 and T6 decreased significantly compared with T2, T3 (P<0.01)respectively. There were no significant differences in plasma propofol concentrations between the pulmonary and radial arteries at any time points. CONCLUSIONS: Propofol is eliminated mainly by the liver, and also by extrahepatic organs. The lungs seem to be not a major site contributing to the extrahepatic metabolism of propofol in humans.
