Predictors for partial suppression of spermatogenesis of hormonal male contraception

Aim： To analyze factors influencing the efficacy of hormonal suppression of spermatogenesis for male contraception. Methods： A nested case-control study was conducted, involving 43 subjects, who did not achieve azoospermia or severe oligozoospermia when given monthly injections of 500 mg testosterone undecanoate （TU）, defined as partial suppressors compared with 855 subjects who had suppressed spermatogenesis （complete suppressors）. Sperm density, serum testosterone, luteinizing hormone （LH） and follicle stimulating hormone （FSH） concentrations at the baseline and the suppression phase were compared between partial and complete suppressors. Polymorphisms of androgen receptor （AR） and three single nucleotide variants and their haplotypes of FSH receptor （FSHR） genes determined by polymerase chain reaction （PCR） and DNA sequencing technique were compared between 29 partial and 34 complete suppressors. Results： Baseline serum LH level was higher and serum LH as well as FSH level during the suppression phase was less suppressed in partial suppressors. Additionally, in a logistic regression analysis larger testis volume, higher serum FSH concentrations alone, or interaction of serum LH, FSH, testosterone and sperm concentrations were associated with degree of suppression. The distribution of polymorphisms of AR or FSH receptor genes did not differ between partial and complete suppressors. In cases with incomplete FSH suppression （FSH 〉 0.2 IU/L）, the chances of reaching azoospermia were 1.5 times higher in the subjects with more than 22 CAG triplet repeats. Conclusion： Partial suppression of spermatogenesis induced by 500 mg TU monthly injections is weakly influenced by hormonal and clinical features but not polymorphism in AR and FSHR genes.

Late-onset hypogonadism （LOH） has been described as a clinical syndrome comprising symptoms consistent with androgen deficiency and a circulating concentration of testosterone in the low-to-normal range in some middle-aged and elderly men. The presence of symptoms may affect the health and quality of life in aging men and it has become a hot spot in global medical research, diagnosis, and treatment in the recent years.

We are very glad to see that the Asian Journal of Andrology published a Special Issue on Semen Analysis in 21st Century Medicine, which well revealed some behind-the-scene controversies of the 5th edition of the WHO laboratory manual for the examination and processing of human semen [1]. Three articles from the special issue, two on the reference values of semen parameters [2, 3] and another presenting the investigation results of 118 laboratories performing semen analysis in China's Mainland [4], are very thoughtprovoking and we would like to share some of our views on these topics.

This study aimed to propose an operational definition of late-onset hypogonadism(LOH)that incorporates both clinical symptoms and serum testosterone measurements to evaluate the prevalence of LOH in aging males in China.A population-based sample of 6296 men aged 40 years-79 years old was enrolled from six representative provinces in *** total testosterone(TT),sex hormone-binding globulin(SHBG),and luteinizing hormone(LH)were measured and free testosterone(cFT)was *** Aging Males’Symptoms(AMS)scale was used to evaluate the LOH ***,5078 men were included in this *** TT levels did not decrease with age(P=0.59),and had no relationship with AMS symptoms(P=0.87 for AMS total score,P=0.74 for≥3 sexual symptoms).The cFT levels decreased significantly with age(P<0.01)and showed a negative association with the presence of≥3 sexual symptoms(P=0.03).The overall estimated prevalence of LOH was 7.8%(395/5078)if a cFT level<210 pmol l−1 combined with the presence of≥3 sexual symptoms was used as the criterion of *** them,26.1%(103/395)and 73.9%(292/395)had primary and secondary hypogonadism,*** adjustment for confounding factors,primary and secondary hypogonadism was positively related to age and *** mass index was an independent risk factor for secondary *** results suggest that the AMS total score is not an appropriate indicator for decreased testosterone,and that the cFT level is more reliable than TT for LOH *** hypogonadism is the most common form of LOH.

This multi-center, cross-sectional study investigated the association between serum testosterone （T） levels, serum sex hormone-binding globulin （SHBG） levels, and the risk of metabolic syndrome （MS） in 3332 adult Chinese men. The prevalence of MS was 34.7%, and men with MS had lower serum levels of total T （TT） and SHBG than those without MS （P 〈 0.001）. There was no significant difference in serum free T （FT） levels between subjects with and without MS （P = 0.627）. In logistic regression analysis, the association between MS and serum SHBG levels persisted after adjusting for age, body mass index （BMI）, smoking and drinking status, and serum TT （odds ratio [OR] 0.962, 95% confidence interval [95% CI] 0.954-0.969, P 〈 0.01）. However, the association between serum TT level and the risk of MS was weak after adjusting for age, BMI, SHBG level, and smoking and drinking status （OR 0,981, 95% CI 0.960-1.007）. Our study reveals that both serum TT and SHBG levels, but not serum FT, are inversely associated with the prevalence of MS and that serum SHBG is an independent and dominant risk factor for *** multi-center, cross-sectional study investigated the association between serum testosterone （T） levels, serum sex hormone-binding globulin （SHBG） levels, and the risk of metabolic syndrome （MS） in 3332 adult Chinese men. Because of unavoidable complications of vasectomy, this study was undertaken to assess the efficacy and safety of male sterilization with a nonobstructive intravas device （IVD） implanted into the vas lumen by a mini-surgical method compared with no-scalpel vasectomy （NSV）. IVDs were categorized into two types： IVD-B has a tail used for fixing to the vas deferens （fixed wing） whereas IVD-A does not. A multicenter prospective randomized controlled clinical trial was conducted in China. The study was comprised of 1459 male volunteers seeking vasectomy who were randomly assigned to the IVD-A （n = 487）, IVD-B （n = 485） or NSV （n = 487） groups and underwent operation. Follow-up included visits at the 3rd-6TM and 12~ postoperative months, The assessments of the subjects involved regular physical examinations （including general and andrological examinations） and semen analysis. The subjects＇ partners also underwent monitoring for pregnancy by monthly interviews regarding menstruation and if necessary, urine tests, There were no significant differences in pregnancy rates （0.65% for IVD-A, 0 for IVD-B and 0.21% for NSV） among the three groups （P 〉 0.05）. The cumulative rates of complications at the 12th postoperative month were zero, 0.9% and 1.7% in the three groups, respectively. In conclusion, IVD male sterilization exhibits a low risk of long-term adverse events and was found to be effective as a male sterilization method, similar to the NSV technique. IVD male sterilization is expected to be a novel contraceptive method.

To avoid the secondary frequency dip caused by the steep drop of the electrical power of wind turbines(WTs)at the end of frequency support stage,the torque limit-based iner-tial control(TLIC)method sets the power reference as a linear function of rotor speed,rather than the step form for the step-wise inertial ***,the compensation effect on the frequency nadir(FN)caused by the load surge is weakened as the TLIC power is no longer in the step ***,the maximum point of the frequency response component(FRC)contributed by TLIC occurs earlier than the minimum point of FRC corresponding to the load surge,so that the FN cannot be adequately ***,this paper first investigates the relation between the peak and nadir times of FRCs stimulated by the TLIC and load *** this basis,with the compensation principle of matching the peak and nadir times of FRCs,the improved TLIC method based on delayed support is ***,the effectiveness of the proposed method is validated via the experiments on the test bench of wind-integrated power system.
