Clinical Value of Trans-parenchymal Compressing Suture to Decrease the Cutting Surface Related Complications after Non-anatomical Liver Resection

Non-anatomical liver resection with appropriate resection margin is regarded as a potential curative treatment for selected major hepatic carcinoma due to preserving maximal normal liver,especially in cirrhotic *** occurrence of cutting surface related complications becomes a main *** June 2010 to June 2016,448 patients with major hepatic carcinoma received non-anatomical liver resection in our liver surgery *** excluding 66 cases that were incongruent with the purpose of study,235 patients undergoing transparenchymal compressing suture(TCS)to“not good”cutting surface were allocated as study group;147 patients with exposed surface(ES)were matched as control *** characteristics of postoperative drainage,postoperative hepatic and renal functions,hospital days,and outcomes were collected *** further compared cutting surface related complications under different levels of liver cirrhosis between the two *** with ES group,patients in TCS group had a decreased incidence of cutting surface related complications(14.3%vs.6.8%,P=0.011)and a decreased probability of interventions for cutting surface related complications(8.2%vs.3.4%,P=0.042).TCS application was much more effective to prevent cutting surface related complications in patients with moderate and severe cirrhosis(5.4%vs.15.8%,P=0.003).Postoperative hepatic and renal function,hospita 1 days and mortality did not differ between the two *** conclusion,TCS decreases the probability of cutting surface related complications and postoperative interventions for related complications,especially in patients with moderate and severe cirrhosis.

Primary combined hepatocellular-cholangiocellular sarcoma:An unusual case

Primary liver carcinosarcoma is rare. Here we report an unusual case of liver carcinosarcoma containing combined hepatocellular cholangiocarcinoma. A mass in the right liver lobe of a 45-year-old man was accidentally discovered by ultrasonic inspection and computed tomography(CT) scan. Surgical resection was performed following a diagnosis of primary liver cancer. Micropathologically, both carcinomatous and sarcomatous elements were present, and diagnosis of liver carcinosarcoma was confirmed. The carcinomatous element consisted of hepatocellular carcinoma and foci of cholangiocellular carcinoma. The sarcomatous element was composed of spindle cells and bizarre cells,as well as foci of osteosarcoma and *** carcinoma cells diffusely expressed both hepatocyte specific markers cytokeratin(CK)8/18 and cholangiocyte specific markers CK19, and sarcoma cells were positive for vimentin. Interestingly,both carcinomatous and sarcomatous cells expressed epithelial membrane antigen. CD117-positive ductular reactions and small undifferentiated cells were observed.A liver progenitor cell origin of the liver carcinosarcoma was proposed.

Hematite-facilitated microbial ammoxidation for enhanced nitrogen removal in constructed wetlands

Constructed wetlands(CWs)are widely applied for decentralized wastewater ***,achieving efficient removal of ammonia(NH+-N)has proven challenging due to insufficient *** this study,natural hematite(Fe,O3)was employed as a CW substrate(H-CWs)for the first time to drive anaerobic ammonia oxidation coupled with iron(I)reduction(Feammox).Compared to gravel constructed wetlands(G-CWs),ammonia removal was enhanced by 38.14%to 54.03%and nitrous oxide(N_(2)0)emissions were reduced by 34.60%in *** synergistic removal of ammonia and nitrate by H-CWs also resulted in the absence of ammoxidation *** and 15N isotope tracer incubations showed that Feammox accounting for approximately 40%of all ammonia removal in the *** enrichment of iron phosphate(Fe_(3)Fe_(4)(PO_(4))_(6))promoted the accumulation of the Feammox intermediate compound *** nanowires were observed on the surface of H-CW substrates as well,suggesting that the observed biological ammoxidation was most likely related to extracellular electron transfer(EET).Microbial and metagenomics analysis revealed that H-CWs elevated the integrity and enhanced the abundance of functional microorganisms and genes associated with nitrogen ***,the efficient ammonia removal in the absence of O2 together with a reduction in N_(2)O emissions as described in this study may provide useful guidance for hematite-mediated anaerobic ammonia removal in CWs.

Evaluation of water management on arsenic methylation and volatilization in arsenic-contaminated soils strengthened by bioaugmentation and biostimulation

Arsenic(As) fate in paddy fields has been one of the most significant current issues due to the strong As accumulation potential of rice plants under fooded conditions. However,no attempt was done to explore As methylation and volatilization under non-fooded conditions. Herein, we investigated the effects of water management on As methylation and volatilization in three arsenic-contaminated soils enhanced by biostimulation with strawderived organic matter and bioaugmentation with genetic engineered Pseudomonas putida KT2440(GE P. putida). Under fooded conditions, the application of biochar(BC), rice straw(RS)and their combination(BC+RS) increased total As in porewater. However, these effects were greatly attenuated under non-fooded conditions. Compared with RS amendment alone, the combination of GE P. putida and RS further promoted the As methylation and volatilization,and the promotion percentage under non-fooded conditions were significantly higher than that under fooded conditions. The combined GE P. putida and RS showed the highest efficiency in As methylation(88 μg/L) and volatilization(415.4 μg/(kg·year)) in the non-fooded soil with moderate As contamination. Finally, stepwise multiple linear regression analysis presented that methylated As, DOC and p H in porewater were the most important factors contributing to As volatilization. Overall, our findings suggest that combination of bioaugmentation with GE P. putida and biostimulation with RS/BC+RS is a potential strategy for bioremediation of arsenic-contaminated soils by enhancing As methylation and volatilization under non-fooded conditions.

Mechanical Performance of Bio-inspired Bidirectional Corrugated Sandwich Pressure Shell Under External Hydrostatic Pressure

This paper aims to enhance the compression capacity of underwater cylindrical shells by adopting the corrugated sandwich structure of *** cuttlebone suffers uniaxial external compression,while underwater cylindrical shells are in a biaxial compressive stress *** suit the biaxial compressive stress state,a novel bidirectional corrugated sandwich structure is proposed to improve the bearing capacity of cylindrical *** static and buckling analysis for the sandwich shell and the unstiffened cylindrical shell with the same volume-weight ratio are studied by numerical *** is indicated that the proposed sandwich shell can effectively reduce the ratio between circumferential and axial stress from 2 to 1.25 and improve the critical buckling load by about 1.63 *** simulation shows that optimizing and adjusting the structural parameters could significantly improve the advantage of the sandwich ***,the hydrostatic pressure tests for shell models fabricated by 3D printing are carried *** to the experimental results,the overall failure position of the sandwich shell is at the center part of the sandwich *** has been found the average critical load of the proposed sandwich shell models exceeds two times that of the unstiffened shell ***,the proposed bio-inspired bidirectional corrugated sandwich structure can significantly enhance the pressure resistance capability of cylindrical shells.

Effect of droplet characteristics on liquid-phase distribution in spray zone of internal mixing air-mist nozzle

In continuous casting production,droplet characteristics are important parameters for evaluating the nozzle atomization quality,and have a significant impact on the secondary cooling effect and the slab *** order to study the behavior of atomized droplets after reaching the slab surface and to optimize the spray cooling effect,the influence of droplet diameter and droplet velocity on the migration behavior of droplets in the secondary cooling zone was analyzed by FLUENT *** show that the droplets in the spray zone and on the slab surface are mainly concentrated in the center,thus,the liquid volume fraction in the center is higher than that of either *** the droplet diameter increases,the region of high liquid volume fraction on the slab surface becomes wider,and the liquid phase distribution in the slab width direction becomes *** increasing the droplet velocity at the nozzle exit has little effect on droplet diffusion in the spray zone,the distribution becomes more uneven due to more liquid reaches the slab surface per unit time.A prediction formula of the maximum water flow rate on the slab surface for specific droplet characteristics was proposed based on dimensionless analysis and validated by simulated data.A nozzle spacing of 210 mm was recommended under the working conditions in this study,which ensures effective coverage of the spray water over the slab surface and enhances the distribution uniformity of water flow rate in the transverse direction.
