目的探讨劈离式肝移植(SLT)右侧供肝肝内胆管的解剖分型与胆道重建。方法采用回顾性描述性研究方法。收集2014年7月至2022年1月中山大学附属第三医院收治的85例行SLT患者的临床资料;男65例,女20例;年龄为45(1~82)岁。观察指标:(1)手术情况。(2)右侧供肝肝内胆管解剖情况。(3)胆道重建情况。(4)术后胆道并发症情况。(5)随访情况。正态分布的计量资料以x±s表示;偏态分布的计量资料以M(范围)或M(Q1,Q3)表示。计数资料以绝对数或百分比表示,组间比较采用χ2检验或Fisher确切概率法。结果(1)手术情况。85例供肝中,采用左右半肝劈离方式11例,采用经典的右三叶和左外叶劈离方式74例。85例供肝冷缺血时间为291(273,354)min;受者手术时间为(497±97)min,无肝期时间为51(40,80)min,输血量为8(7,12)U。(2)右侧供肝肝内胆管解剖情况。85例供肝中,经典胆管解剖模型(1型)47例,占比为55.3%(47/85),解剖变异型38例,占比为44.7%(38/85)。38例解剖变异型分别为:2型7例,3a型16例,3b型2例,3c型2例,4型1例,5a型3例,5b型4例,6型3例。85例供肝的胆管劈离方式中,84例为保留肝总管主干于右半肝或右三叶,1例行完全左右半肝劈离将肝总管主干保留于左半肝,右半肝保留右肝管(1型);84例仅有1个胆管开口,1例有2个胆管开口(3c型)。(3)胆道重建情况。85例受者胆道重建方案为供肝胆总管-受者胆总管端端吻合69例(1型38例、2型5例、3a型14例、3b型2例、4型1例、5a型3例、5b型4例、6型2例),供肝胆总管-受者空肠吻合11例(1型7例、2型2例、3c型1例、6型1例),供肝肝总管-受者空肠吻合3例(1型1例、3a型2例),供肝右肝管-受者空肠吻合1例(1型),供肝右后支-受者肝总管端端吻合+供肝肝总管-受者空肠Roux-en-Y吻合1例(3c型)。(4)术后胆道并发症情况。85例受者中,6例发生胆道并发症,发生率为7.1%(6/85)。6例发生胆道并发症受者中,5例为1型,1例为3b型。5例发生胆道并发症的1型受者中,3例为术后胆道狭窄合并胆漏,2例为术后胆管吻合口狭窄。1例发生胆道并发症的3b型受者术后出现胆管吻合口狭窄合并肝断面胆漏。47例经典胆管解剖模型和38例解剖变异型受者发生胆道并发症分别为5例和1例,两者比较,差异无统计学意义(P>0.05)。(5)随访情况。83例受者获得随访,随访时间为52(12,96)个月。随访期间,2例受者因非胆道并发症因素死亡(1型1例,3a型1例)。结论SLT右侧供肝肝内胆管解剖分型主要为经典胆管解剖模型,胆道重建方案主要为供肝胆总管-受者胆总管端端吻合。

Despite the precise controllability of droplet samples in digital microfluidic(DMF)systems,their capability in isolating single cells for long-time culture is still limited:typically,only a few cells can be captured on an *** fabricating small-sized hydrophilic micropatches on an electrode aids single-cell capture,the actuation voltage for droplet transportation has to be significantly raised,resulting in a shorter lifetime for the DMF chip and a larger risk of damaging the *** this work,a DMF system with 3D microstructures engineered on-chip is proposed to form semi-closed micro-wells for efficient single-cell isolation and long-time *** optimum results showed that approximately 20%of the micro-wells over a 30×30 array were occupied by isolated single *** addition,low-evaporation-temperature oil and surfactant aided the system in achieving a low droplet actuation voltage of 36V,which was 4 times lower than the typical 150 V,minimizing the potential damage to the cells in the droplets and to the DMF *** exemplify the technological advances,drug sensitivity tests were run in our DMF system to investigate the cell response of breast cancer cells(MDA-MB-231)and breast normal cells(MCF-10A)to a widely used chemotherapeutic drug,Cisplatin(Cis).The results on-chip were consistent with those screened in conventional 96-well *** novel,simple and robust single-cell trapping method has great potential in biological research at the single cell level.

Alagille综合征(alagille syndrome,ALGS)是一类可累及多个脏器的常染色体显性遗传性疾病,其突变位点为JAG1或NOTCH2^([1])。ALGS的发病率约1/70000,是儿童慢性胆汁淤积的重要原因之一^([2-3])。ALGS累及的脏器包括肝脏、心脏、骨骼、眼、颜面部等,大多数ALGS患者因胆汁淤积导致的临床症状就诊,但导致患者死亡的原因多为心血管受累。约21%~31%的患者需行肝移植,严重的皮肤瘙痒、门静脉高压.

目的探讨劈离式供肝儿童肝移植门静脉并发症的诊断及治疗策略。方法回顾性分析接受劈离式肝移植的88例儿童受者的临床资料。术中根据受者门静脉内径、发育情况,利用门静脉左右分支处进行吻合或间置供者髂静脉搭桥吻合,围手术期采用规范... 详细信息

目的探讨劈离式供肝儿童肝移植门静脉并发症的诊断及治疗策略。方法回顾性分析接受劈离式肝移植的88例儿童受者的临床资料。术中根据受者门静脉内径、发育情况,利用门静脉左右分支处进行吻合或间置供者髂静脉搭桥吻合,围手术期采用规范化的门静脉血流监测,术后按肝素钠桥接华法林的方案进行抗凝治疗。经增强CT或门静脉造影确诊门静脉狭窄或血栓形成后,予切开取栓、全身抗凝、介入下取栓、球囊扩张和(或)支架置入等处理。结果88例受者中共10例患儿确诊门静脉并发症,其中4例门静脉狭窄,确诊时间分别为术后1 d、2个月、8个月、11个月,6例门静脉血栓形成,确诊时间分别为术中、术后2 d、术后3 d(2例)、术后6 d、术后11个月。1例门静脉狭窄者和1例门静脉血栓形成者于围手术期死亡,门静脉并发症相关病死率为2%(2/88)。其余8例患者中,1例行全身抗凝治疗、2例行门静脉切开取栓术、1例行介入下球囊扩张、4例行介入下球囊扩张及支架置入,术后均长期随访,未再出现门静脉相关症状,复查门静脉血流参数正常。结论规范化的术中及术后门静脉血流监测有助于早期发现门静脉并发症,及时采取术中门静脉切开取栓,术后介入下球囊扩张、支架置入等手段可有效治疗门静脉并发症,减少门静脉并发症导致的移植物丢失和受者死亡。

A patient with steroid-resistant acute rejection 50 days after ABO-compatible orthotopic liver transplantation(LT)received regular infusion of allogeneic mesenchymal stem cells(MSCs)after three sessions of steroid pulse therapy which failed to control the pathogenetic condition as shown by *** function improved gradually after intravenous injection of MSCs once weekly for 10 weeks(as confirmed by biopsy)and remained stable under administration of conventional immunosuppressive *** was no evidence of neoplasms 5 years after *** infusion appears to successfully reverse resistance to immunosuppressive agents and may be a useful treatment for post-liver transplant steroid-resistant rejection.

With the dawn of organ donation after a citizen's death in China,the use of split-liver transplantation(SLT)can effectively increase the source of donor liver,reduce the waiting time for organ transplantation in patients,and particularly solve the problem of organ shortage in *** recent years,many transplantation centers have been performing SLT to varying degrees and *** the current stage,the experiences of countries with advanced transplantation techniques should be used to establish an SLT consensus that is suitable for China to further increase the ratio and efficacy of *** this paper,we combined expert experiences to generate an SLT expert consensus that included donor and donor liver evaluation,recipient selection criteria,donor and recipient matching,selection of splitting form and tools,blood vessels and bile ducts dissection and allocation,perioperative management of SLT,and organ allocation.
