Analysis of colonoscopic perforations at a local clinic and a tertiary hospital

AIM： To define the clinical characteristics, and to as- sess the management of colonoscopic complications at a local clinic. METHODS： A retrospective review of the medical re- cords was performed for the patients with iatrogenic colon perforations after endoscopy at a local clinic be- tween April 2006 and December 2010. Data obtained from a tertiary hospital in the same region were also analyzed. The underlying conditions, clinical presenta- tions, perforation locations, treatment types （operative or conservative） and outcome data for patients at the local clinic and the tertiary hospital were compared. RESULTS： A total of 10 826 colonoscopies, and 2625 therapeutic procedures were performed at a local clinic and 32 148 colonoscopies, and 7787 therapeutic proce-dures were performed at the tertiary hospital. The clinic had no perforations during diagnostic colonoscopy and 8 （0.3%） perforations were determined to be related to therapeutic procedures. The perforation rates in each therapeutic procedure were 0.06% （1/1609） in polyp- ectomy, 0.2% （2/885） in endoscopic mucosal resection （EMR）, and 3.8% （5/131） in endoscopic submucosal dis- section （ESD）. Perforation rates for ESD were significantly higher than those for polypectomy or EMR （P 〈 0.01）. All of these patients were treated conservatively. On the oth- er hand, three （0.01%） perforation cases were observed among the 24 361 diagnostic procedures performed, and these cases were treated with surgery in a tertiary hospi- tal. Six perforations occurred with therapeutic endoscopy （perforation rate, 0.08%; 1 per 1298 procedures）. Perfo- ration rates for specific procedure types were 0.02% （1 per 5500） for polypectomy, 0.17% （1 per 561） for EMR, 2.3% （1 per 43） for ESD in the tertiary hospital. There were no differences in the perforation rates for each therapeutic procedure between the clinic and the tertiary hospital. Analysis of ABC (D) stratification for screening patients with gastric cancer

Polymorphism Near the Interleukin-28B Gene and Anti-Hepatitis C Viral Response

In a recent genome-wide association study, single nucleotide polymorphisms (SNPs) located near the interleukin-28B gene (IL28B), which encodes type Ⅲ interferon (IFN) λ3, were shown to be strongly associated with a viral response to pegylated IFNα (PEG-IFNα) and ribavirin (RBV) combination therapy and spontaneous viral clearance in patients chroni-cally and acutely infected with hepatitis C virus (HCV), respectively. The global distribution of allele frequencies shows a remarkable pattern, in which a favorable allele is nearly fixed in East Asia, has an intermediate frequency in Europe, and is least frequent in Africa. Although the under-lying mechanisms responsible for viral responses associated with IL28B SNPs have not been completely elucidated, IFN-stimulated gene expression in patients with unfavorable IL28B genotypes tends to be high at baseline and is insufficiently induced by exogenous IFN administration, resulting in poor treatment outcomes. Clinically, triple therapy with PEG-IFNα/RBV together with direct-acting anti-viral agents (DAAs) is currently used to treat chronic hepatitis C as a first-line therapy. Although the predictive power of IL28B status may be attenuated, the IL28B genotype will remain relevant to the outcomes of DAA therapy when used in combination with PEG-IFNα as a backbone. Even with the introduction of IFN-free therapies with a new class of highly effective DAAs, IL28B SNPs are still useful predictors of treatment outcomes and can be used to individualize treat-ment strategies to maximize cost-effectiveness and identify patients at risk of being refractory to treatment. This review summarizes the current understanding of the clinical sig-nificance and role of IL28B in HCV infection and response to therapy.

Taurolithocholate impairs bile canalicular motility and canalicular bile secretion in isolated rat hepatocyte couplets

AIM: To investigate the effects of taurolithocholate (TLC)on the canalicular motility in isolated rat hepatocyte cou-plets (IRHC).METHODS: TLC was added to IRHC at concentrationsof 10 and 50 μmol/L, respectively. In each group, fi vetime-lapse movies containing 3 representative bile cana-liculi were taken under phase-contrast microscopy for12 h. The number of bile canalicular contractions andthe intervals between consecutive canalicular contrac-tions were calculated. Furthermore, the effects of TLC onIRHC were examined by transmission electron ***: The bile canalicular contractions were spon-taneous and forceful in the controls. Active vesicularmovement was observed in the pericanalicular *** after the addition of TLC, the bile canaliculiwere deformed, and canalicular bile was incorporatedinto the vacuoles. The canaliculi were gradually dilated,and canalicular contractions were markedly inhibited byTLC. The vesicular movements became extremely slowin the pericanalicular region. The number of canalicularcontractions significantly decreased in the TLC-treatedgroups, as compared with that in the controls. The timeintervals were prolonged, as the TLC dosage increased,indicating that bile secretion into the canaliculi wasimpaired with TLC. Transmission electron microscopyrevealed the lamellar transformation of the canalicularmembranes in IRHC treated with ***: TLC impairs both the bile canalicularcontractions and the canalicular bile secretion, possiblyby acting directly on the canalicular membranes in TLC-induced cholestasis.

New tomographic reconstruction technique based on Laplacian eigenfunction

This letter proposes a new tomographic reconstruction procedure based on the Laplacian eigenfunction(LEF) patterns, which are independent of the plasma cross-section and do not require the flux surface information. The process is benchmarked for the experimental data of Heliotron J plasma and the results are compared with the least-squares approximation by a Phillips–Tikhonov(PT)-type regularization, which is widely used as the standard technique for tomographic reconstruction. The reconstruction based on the LEF is found to be capable of determining the magnetic axis at different time locations efficiently in comparison with the PT-type regularization.

High-pressure phase transitions of anorthosite crust in the Earth's deep mantle

We investigated phase relations, mineral chemistry, and density of lunar highland anorthosite at conditions up to 125 GPa and 2000 K. We used a multi-anvil apparatus and a laser-heated diamond-anvil cell for this purpose. In-situ X-ray diffraction measurements at high pressures and composition analysis of recovered samples using an analytical transmission electron microscope showed that anorthosite consists of garnet,CaAl_4Si_2O_(11)-rich phase(CAS phase), and SiO_2 phases in the upper mantle and the mantle transition *** lower mantle conditions, these minerals transform to the assemblage of bridgmanite, Ca-perovskite,corundum, stishovite, and calcium ferrite-type aluminous phase through the decomposition of garnet and CAS phase at around 700 km depth. Anorthosite has a higher density than PREM and pyrolite in the upper mantle, while its density becomes comparable or lower under lower mantle conditions. Our results suggest that ancient anorthosite crust subducted down to the deep mantle was likely to have accumulated at660-720 km in depth without coming back to the Earth's surface. Some portions of the anorthosite crust might have circulated continuously in the Earth's deep interior by mantle convection and potentially subducted to the bottom of the lower mantle when carried within layers of dense basaltic rocks.

An examination of active sites on Au-Ag bimetallic catalysts based on CO oxidation over Au/Ag_2O and a comparison to Ag-contaminated Au powder

There are two theories regarding the origin of the remarkable synergistic effect observed in Au‐Ag bimetallic catalysts when applied to various oxidative reactions. One is based on the importance of the contact interfaces between AgOx regions and the surface of the bulk Au as active working sites, while the other holds that charge transfer from Ag to Au in a surface Au‐Ag alloy causes the catalytic activity. One key point in examining these theories and determining the origin of the synergy in‐volves determining whether or not Ag exists as an oxide or as a metallic alloy on the Au surface. To confirm that enhanced activity results from contact between Ag2O and Au nanoparticles （NPs）, a comparative study of catalytic CO oxidation over Au/Ag2O and Ag2O was performed in the present work, using a closed recirculation reaction system. A reaction mixture consisting of a stoichiometric composition of CO and O2 （CO/O2=2/1） was supplied to both catalysts and the resulting pressure decrease rates were tracked, from which the amounts of gas consumed as well as the quantity of CO2 produced were determined. The steady state reactions of both Au/Ag2O and Ag2O did not lead to any meaningful difference in the rate of pressure decrease during the oxidation. The pressure decrease over both catalysts was attributed to the reduction of surface lattice O on Ag2O by CO. The results obtained for Au/Ag2O are in good agreement with previous data resulting from the use of Ag‐contaminated Au powder （Ag/Au‐b） having an oxidized surfaces. This finding suggests that the perimeters between AgOx zones and the bulk Au surface may not function as active sites during CO oxidation. A review of previous results obtained with Ag/Au‐b specimens having so‐called steady state surfaces indicates that AgOx species in such materials are reduced to the 0 state to form a Ag‐Au alloy that provides the active sites.

Salvage brachytherapy for locally recurrent prostate cancer after external beam radiotherapy

External beam radiotherapy （EBRT） is a standard treatment for prostate cancer. Despite the development of novel radiotherapy techniques such as intensity-modulated conformal radiotherapy, the risk of local recurrence after EBRT has not been obviated. Various local treatment options （including salvage prostatectomy, brachytherapy, cryotherapy, and high-intensity focused ultrasound [HIFU]） have been employed in cases of local recurrence after primary EBRT. Brachytherapy is the first-line treatment for low-risk and selected intermediate-risk prostate tumors. However, few studies have examined the use of brachytherapy to treat post-EBRT recurrent prostate cancer. The purpose of this paper is to analyze the current state of our knowledge about the effects of salvage brachytherapy in patients who develop locally recurrent prostate cancer after primary EBRT. This article also introduces our novel permanent brachytherapy salvage method.

Potent water extracts of Indonesian medicinal plants against PTP1B

Objective:To examine the potent of water as a solvent agent in the preparation of traditional herbal ***:Water extracts of 18 plants were prepared through reflux and examined(25μg/mL) to evaluate their possibility for inhibiting protein tyrosine phosphatase 1B(PTP1B).The determination of IC50 values was performed for the samples possessing more than 80%***,those exhibiting IC50 values more than 7.0 μg/mL were further profiled for their chemical constituents through nuclear magnetic resonance(NMR) ***:About 44%(8) of the examined samples showed more than 80%inhibition against PTP1 *** water extracts of Elephantopus scaber,Helicteres isora aerial parts,Elaeocarpus grandiflorus(***) fruits,Melaleuca leucadendron leaves,and Quercus infectoria gum had IC50 values ranging from 2.05 to 6.90 μg/***,Andropogon nardus and Centella asiatica were at the area of δ 3.0-4.0 ***,the13 C NMR observation of samples possessing the most intensive signals in their proton NMR Cinnamomum burmannii and *** showed the peaks at the area of δ 60-90 ppm as the supportive evidence for sugar group ***,a disaccharide from *** could be an active inhibitor towards PTB1 ***:In contrast to the mainstream solvents currently used in modern herbal manufactures especially Jamu medicine in Indonesia,pure-water-extracted materials should be reconsidered and could be reemerged for future studies and for the manufacture of herbal *** addition,the activity of Jamu components should be confirmed that their antidiabetes and antiobesity activities could be through the inhibition of PTP1 B.
