Urinary Vascular Endothelial Growth Factor (VEGF) as a Novel Marker for Early Prediction of Esophageal Varices in Patients with Chronic Liver Disease

Background/Aims: Vascular Endothelial Growth Factor (VEGF) has a crucial role in portal hypertension and collateral vessels formation. This study aims to assess urinary VEGF in cirrhotic patients as a predictor of presence of esophageal varices, and variceal bleeding. Settings and Design: 42 cirrhotic patients were randomly selected and classified into 2 groups according to the presence of variceal bleeding. Methods and Material: Urinary VEGF was measured and corrected against urinary creatinine. Platelet count, liver functions, abdominal ultrasonography and upper endoscopy were done. Statistical Analysis Used: Comparison was done by Mann Whitney and Kruskal Wallis tests. Correlation was done using Spearman rank correlation. Multivariable logistic regression was done to identify predictors of variceal bleeding and presence of large varices. Receiver operator characteristic curve (ROC) analysis was used to determine the optimum cut off value of predictors. Results and Conclusions: Urinary VEGF was lower in cirrhotic patients with esophageal varices than those without. Low VEGF, low platelet count and splenomegaly were found to be independent predictors of both the presence of large esophageal varices, and variceal bleeding. Cut-off values for platelet count ≤ 166.3 × 103/μL, and corrected VEGF ≤ 59.12 pg/mg were predictive of large esophageal varices with 93.1%, 86.2% sensitivity and 74.5%, 58.2% specificity respectively. While variceal bleeding could be predicted at a platelet count ≤ 153 × 103/μL, and corrected VEGF ≤ 45.08 pg/mg with 90.9%, 81.8% sensitivity and 72.6%, 59.7% specificity respectively. The study concludes that urinary VEGF can be used as an alternative to upper endoscopic screening.

The Role of Electrocardiogram DETERMINE Score in Prediction of Coronary Artery Disease Severity

Background: A major cause of mortality and disability on a global scale is myocardial infarction (MI). These days, the most reliable way to detect and measure MI is via cardiovascular magnetic resonance imaging (CMR). Aims and Objectives: To evaluate the effectiveness of the Electrocardiogram DETERMINE Score in predicting the severity of coronary artery disease (CAD) in patients who have experienced an Acute Myocardial Infarction (AMI) & to assess improvements in left ventricular function at 6 months following coronary artery bypass grafting (CABG). Subjects and Methods: This Observational cohort study was done at the Cardiology and Radiology department and cardiac surgery department, Al-Azhar university hospitals and Helwan University hospital. The study involved 700 cases who patients diagnosed with Acute Myocardial Infarction and fulfilled specific criteria for selection. Result: There was highly statistically significant relation between Myocardial infarction size and ECG Marker Score as mean infarct size elevated When the number of ECG markers increased. There was a highly statistically significant relation between myocardial infarct segments, myocardial infarction size and improvement of cardiac function 6 months post-CABG. Conclusion: The study found that larger myocardial infarctions corresponded with higher DETERMINE Scores. It concluded that an ECG-based score better estimates infarct size than LVEF alone. Additionally, there was a significant statistical correlation between the size and segmentation of myocardial infarction and better cardiac function six months after CABG.

Complications of Corticosteroid Therapy in Rheumatological Practice in Guinea

Introduction: Corticosteroid therapy used in rheumatology is effective but can lead to complications. The objective was to describe the complications of corticosteroid therapy in rheumatological practice at Ignace Deen University Hospital (Conakry). Methods: Cross-sectional study from July 1, 2022 to March 31, 2023. All patients who had been diagnosed with a rheumatological disease and who had received corticosteroid therapy were included. Results: Of the 1489 patients suffering from rheumatological diseases, 238 (15.9%) had received corticosteroid therapy. The average age of the patients was 48.5 ± 17.16 years with a female predominance (70.5%). The main indications for corticosteroid therapy were lower back pain due to probable disco-radicular impingement (35.8%) followed by rheumatoid arthritis (21.9%). The predominant method of administration was infiltration (56.9%). The average dosage was 24.2 ± 13.5 mg prednisone equivalent (range: 5 mg and 40 mg). Complications were present in 26 patients (10.9%) such as muscle cramps (3.8%), hypocalcemia (3.4%), edema of the lower limbs (3%), stimulation of appetite and weight gain (3%), high blood pressure (2.5%), hyperglycemia (2.5%). The occurrence of complications was associated with the high dose of corticosteroid (p = 0.014) and long-term use (p = 0.000). Conclusion: Corticosteroid therapy used in 15.9% of consultations in the rheumatology department of Ignace Deen University Hospital led to 10.9% of complications. These complications were related to the dose and duration of corticosteroid therapy.

This paper presents a fast adaptive iterative algorithm to solve linearly separable classification problems in R *** each iteration,a subset of the sampling data （n-points,where n is the number of features） is adaptively chosen and a hyperplane is constructed such that it separates the chosen n-points at a margin and best classifies the remaining *** classification problem is formulated and the details of the algorithm are ***,the algorithm is extended to solving quadratically separable classification *** basic idea is based on mapping the physical space to another larger one where the problem becomes linearly *** illustrations show that few iteration steps are sufficient for convergence when classes are linearly *** nonlinearly separable data,given a specified maximum number of iteration steps,the algorithm returns the best hyperplane that minimizes the number of misclassified points occurring through these *** with other machine learning algorithms on practical and benchmark datasets are also presented,showing the performance of the proposed algorithm.

One of the continuity conditions identified by Utumi on self-injective rings is the C3-condition, where a module M is called a C3-module if whenever A and B are direct summands of M and A A B = 0, then A B is a summand of M. In addition to injective and direct-injective modules, the class of C3-modules includes the semisimple, continuous, indecomposable and regular modules. Indeed, every commutative ring is a C3-ring. In this paper we provide a general and unified treatment of the above mentioned classes of modules in terms of the C3-condition, and establish new characterizations of several well known classes of rings.

Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Accurate forecasting of emerging infectious diseases can guide public health officials in making appropriate decisions related to the allocation of public health *** to the exponential spread of the COVID-19 infection worldwide,several computational models for forecasting the transmission and mortality rates of COVID-19 have been proposed in the *** accelerate scientific and public health insights into the spread and impact of COVID-19,Google released the Google COVID-19 search trends symptoms open-access *** objective is to develop 7 and 14-day-ahead forecasting models of COVID-19 transmission and mortality in the US using the Google search trends for COVID-19 related ***,we propose a stacked long short-term memory(SLSTM)architecture for predicting COVID-19 confirmed and death cases using historical time series data combined with auxiliary time series data from the Google COVID-19 search trends symptoms *** the SLSTM networks trained using historical data only as the base models,our base models for 7 and 14-day-ahead forecasting of COVID cases had the mean absolute percentage error(MAPE)values of 6.6%and 8.8%,*** the other side,our proposed models had improved MAPE values of 3.2%and 5.6%,*** 7 and 14-day-ahead forecasting of COVID-19 deaths,the MAPE values of the base models were 4.8%and 11.4%,while the improved MAPE values of our proposed models were 4.7%and 7.8%,*** found that the Google search trends for“pneumonia,”“shortness of breath,”and“fever”are the most informative search trends for predicting COVID-19 *** also found that the search trends for“hypoxia”and“fever”were the most informative trends for forecasting COVID-19 mortality.

Herein,iron oxide/hydroxides deposits(gossans)were utilized,for the first time,in the fabrication of magnetite nanoparticles(MNPs)to load modified coal(MC).The as-synthesized MNPs@MC composite was characterized via different techniques and utilized for the Cr(Ⅵ)*** studies supported by theoretical treatment were applied to offer a new overview of the Cr(Ⅵ)adsorption geometry and mechanism at 25-45℃.Experimental results suggested that the Cr(Ⅵ)uptake was mainly governed by adsorption-reduction coupled *** Langmuir model fitted well the Cr(Ⅵ)adsorption data with maximum adsorption capacities extended from 115.24 to 129.63 mg·g^(-1).Theoretical calculations indicated that Cr(Ⅵ)ions were adsorbed on the MNPs@MC following the theory of the advanced monolayer statistical *** number of ions removed per site ranged from 1.88 to1.23 suggesting the involvement of vertical geometry and multi-ionic mechanism at all *** increment of the active sites density and the adsorption capacity at saturation with improving temperature reflected an endothermic ***,the Cr(Ⅵ)adsorption was controlled by physical forces as the adsorption energies were less than 40 kJ·mol^(-1).The calculated free enthalpy,*** internal energy explained the spontaneous nature and the viability of Cr(Ⅵ)adsorption on the MNPs@MC *** results offer a new approach in utilizing the iron-rich deposits as gossans in the preparation of magnetic and low-cost adsorbents for wastewater remediation.
