Perilunate dislocations are rare injuries of the wrist and complete dislocation of the lunate is also rare. There is controversy in literature regarding the optimal management of such injuries. Complete lunate enucleation is associated with high rate of osteonecrosis of lunate thus wrist arthrodesis or proximal row carpectomy has been ad- vocated as primary treatment for such injuries. We report a case of transradial styloid complete palmar lunate enucle- ation in a 25-year-old male patient who sustained injury to the left wrist due to fall on outstretched hand. Carpal row salvage surgery initially by closed reduction and wrist distractor application followed by open reduction and in- ternal fixation with K-wires along with ligamentous repair resulted in restoration of normal wrist anatomy and good functional outcome. As many of these injuries are missed on initial presentation and outcome is poor for missed injuries, prompt diagnosis and early surgical management to restore vascularity of lunate is recommended.

Agriculture is the major activity in the state of Haryana and large volume of water is required to meet the irrigation demands of the crops grown. But, there is limited water availability in the state. Haryana receives water from Yamuna River and Bhakra system. sowmelt, rainfall and groundwater are main sources of water in the catchment. It is essential to integrate the manmade canal system with hydrological system. This paper focuses on integrated hydrological modeling framework to conceptualize the system and to assess the Water Resources of the state. snowmelt and Rainfall runoff modeling using GR4JsG model were combined to model the inflows to the irrigation system of Haryana. Irrigator canal model of eWater source has been used to generate water demands from crops grown. The water balance and water use efficiency have been worked out for each district of Haryana. The hydro climate input data, stream flows, crop data and soil data have been used in the study. The flows modeled at Tuini (P), yashwant Nagar, Bausan, Haripur, Poanta and HKB sites were compared with the observed flows. The objective function of NsE Daily and log Flow duration was used for model calibration and validation at various locations up to Mathura, the outlet of the study area. The value of the objective function at Mathura was 0.54, a fairly good value. The results of the Irrigator canal model have shown that all the Inflows, Outflows and the Utilizations of water have been properly balanced for each district. The water use efficiency of districts varies from 27% to 59%. The overall water use efficiency for Haryana canal system has been calculated as 39%. This is low value indicating excess water is being extracted to meet the water demands.

Triple plating of tibia in a complex bicondylar tibial plateau fracture

High-energy tibial plateau fracture poses a significant challenge and difficulty for orthopaedic surgeons. Fracture of tibial plateau involves major weight bearing joint and may alter knee kinematics. Anatomic reconstruction of the proximal tibial articular surfaces, restoration of the limb axis （limb alignment） and stable fixation permitting early joint motion are the goals of the treatment. In cases of complex bicondylar tibial plateau fractures, isolated lateral plating is frequently associated with varus malalignment and better results have been obtained with bilateral plating through dual incisions. However sometimes a complex type of bicondylar tibialplateau fractures is encountered in which medial plateau has a biplaner fracture in posterior coronal plane as well as sagittal plane. In such fractures it is imperative to fix the medial plateau with buttressing in both planes. One such fracture pattern of the proximal tibia managed by triple plating through dual posteromedial and anterolateral incisions is discussed in this case report with emphasis on mechanisms of this type of injury, surgical approach and management.

We report a case of a 42-year-old male with symptomatic choledochocele and incidental pancreas divisum diagnosed with magnetic resonance cholangiopancreatography (MRCP). small choledochocele is rare congenital malformation associated with non-specific symptoms and a delay in diagnosis. The coexistence of choledochocele and pancreas divisum is extremely rare with only two case reports published in literature. In both cases MRCP failed to diagnose any biliary or pancreatic abnormality. This case suggests that the patients with recurrent abdominal pain and pancreas divisum should not be presumed to be suffering from pancreatitis. Careful evaluated for additional anomalies in the biliary tree should be sought for refractory symptoms. MRCP is a useful one-stop-shop for diagnosing pancreatic and biliary ductal anomalies.

Conformal Analysis of spatial shift in High Resolution satellite Data (HRsD)

Advent of High Resolution satellite Data (HRsD) with development of high spatial resolution sensors have revolutionized the generation of large scale maps. Generation of large scale digital utility maps using HRsD involves different methodologies and includes several steps wherein errors or spatial shift may be induced at any stage of data generation. It may be interesting to note that the characteristics of the spatial shift vary with methodologies adopted in its processing and has unique implications with respect to the data usage along with its application. spatial shifts of points on a satellite data is result of unexpected translation and rotation of pixel with respect to the original location. Present study analyzes the spatial shift generated in satellite data with reference to the change in area and orientation of a group of pixels i.e. conformal and equal area properties of the rectified satellite data. This study aims to establish a relationship between the spatial resolutions of the satellite image used for digital map generation with the spatial accuracy achieved. In this study, Ground Control Points (GCP’s) identified on satellite data for a sample study area were validated using Differential Global Positioning system. Five different high resolution satellite images were analyzed to verify changes in area and shape with reference to the GCP’s. The results indicate that with improvement in the spatial resolution, higher precision in the digital maps is accomplished in terms of spatial shift of the points.
