Longitudinal Alterations of Local Spontaneous Brain Activity in Parkinson's Disease

We used resting-state fMRI to evaluate longitu- dinal alterations in local spontaneous brain activity in Parkinson＇s disease （PD） over a 2-year period. Data were acquired from 23 PD patients at baseline and follow-up, and 27 age- and sex-matched normal controls. Regional homogeneity （ReHo） and voxel-based-morphometry （VBM） were used to identify differences in local sponta- neous brain activity and grey matter volume. With disease progression, we observed a progressive decrease in ReHo in the sensorimotor cortex, default-mode network, and left cerebellum, but increased ReHo in the supplementary motor area, bilateral temporal gyrus, and hippocampus. Moreover, there was a significant positive correlation between the rates of ReHo change in the left cerebellum and the rates of change in the Unified Parkinson＇s Disease Rating Scale-III scores. VBM revealed no significant differences in the grey matter volume among the three sets of acquisitions. We conclude that ReHo may be a suitable non-invasive marker of progression in PD.

What MELD score mandates use of entecavir for ACLF-HBV HBeAg-negative patients?

AIM: To investigate optimal timing for therapeutic efficacy of entecavir for acute-on-chronic hepatitis B liver failure (ACLF-HBV) in hepatitis B e antigen (HBeAg)negative patients. METHODS: A total of 109 inpatients with ACLF-HBV were recruited from the department of Infectious Diseases of the Third Affiliated Hospital, Sun Yat-sen university from October 2007 to October 2010. Entecavir 0.5 mg/d was added to each patient's comprehensive therapeutic regimen. Patients were divided into threegroups according to model for end-stage liver disease (MELD) score: high (≥ 30, 20 males and 4 females, mean age 47.8 ± 13.5 years); intermediate (22-30, 49 males and 5 females, 45.9 ± 12.4 years); and low (≤ 22, 28 males and 3 females, 43.4 ± 9.4 years). Statistical analysis were performed using SPSS 11.0 software. Data with normal distribution were expressed as mean ± SD and comparisons were made with Student's t tests. A value of P < 0.05 was considered statistically significant. Viral loads were related exponentially and logarithmic data were used for analysis. RESULTS: For 24 patients with MELD score ≥ 30, treatment lasted 17.2 ± 16.5 d. Scores before and after treatment were significantly different (35.97 ± 4.87 and 40.48 ± 8.17, respectively, t = -2.762, P = 0.011); HBV DNA load was reduced (4.882 ± 1.847 copies log10/mL to 3.685 ± 1.436 copies log10/mL); and mortality rate was 95.83% (23/24). Of 54 patients with scores of 22-30, treatment lasted for 54.0 ± 43.2 d; scores before and after treatment were 25.87 ± 2.33 and 25.82 ± 13.92, respectively (t = -0.030, P = 0.976); HBV DNA load decreased from 6.308 ± 1.607 to 3.473 ± 2.097 copies log10/mL; and mortality was 51.85% (28/54). Of 31 patients with scores ≤ 22, treatment lasted for 66.1 ± 41.9 d; scores before and after treatment were 18.88 ± 2.44 and 12.39 ± 7.80, respectively, (t = 4.860, P = 0.000); HBV DNA load decreased from 5.841 ± 1.734 to 2.657 ± 1.154 copies log10/mL; and mortality was 3.23% (1/31). Significant benefits of osimertinib in treating acquired resistance to first-generation EGFR-TKIs in lung squamous cell cancer: A case report

BACKGROUND Lung squamous cell cancer(LSCC)rarely harbors epidermal growth factor receptor(EGFR)mutations,even much rarer for acquired T790M *** clinical trials of AURA series illustrated that non-small cell lung cancer(NSCLC)with EGFR T790M mutation can benefit from osimertinib,only five LSCC patients were enrolled in total;moreover,the efficacy for LSCC was not shown in the ***,the response of LSCC to osimertinib is still unclear to *** SUMMARY We report an LSCC case with T790M-related acquired resistance after treatments with first-generation EGFR-tyrosine kinase inhibitors(EGFR-TKIs)and benefited from osimertinib significantly.A 63-year-old Chinese man was diagnosed with stage IV(cT2N2M1b)LSCC harboring an EGFR exon 19-deletion *** disease progression after gefitinib and multi-line chemotherapy,rebiopsy was *** testing of EGFR by amplification refractory mutation system-polymerase chain reaction detected the exon 19-deletion without T790M ***,the patient was given erlotinib,but progression developed only 3 mo *** the frozen re-biopsy tissue was tested by next-generation sequencing(NGS),which detected an EGFR T790M ***,he was very weak with symptoms of dysphagia and ***,osimertinib was started,leading to alleviation from the *** months later,normal deglutition was restored and partial response was ***,the patient achieved an overall survival time period of 29 *** Our findings highlight that EGFR T790M mutation may also be an important acquired drug resistance mechanism for LSCC and offer direct evidence of the efficacy of osimertinib in LSCC with T790M *** and better preservation conditions may contribute to higher sensitivity of EGFR T790M detection.

Tacrolimus treatment for relapsing-remitting chronic inflammatory demyelinating polyradiculoneuropathy:Two case reports

BACKGROUND This study describes the efficacy of a tacrolimus treatment regimen used to treat two patients with relapsing-remitting chronic inflammatory demyelinating polyradiculoneuropathy(CIDP).CASE SUMMARY Two patients(17-year-old female and 27-year-old male)were enrolled in the current study and were followed up for 12 *** first patient was administered tacrolimus(2 mg/d)for 12 mo and prednisolone(40 mg/d)for six *** second patient was administered tacrolimus(3 mg/d)for six *** patients were followed up for 12 mo and the degree of recurrent weakness or normalized motor function was *** addition,nerve conduction studies and tacrolimus levels were *** tacrolimus treatment,both patients showed marked improvement in clinical *** the first patient,prednisolone treatment was successfully withdrawn after six *** as well as motor nerve conduction velocities showed evident recovery following ***,conduction velocities did not completely return to normal,suggesting that electrophysiological recovery can be slower than clinical *** Neither patient exhibited any adverse effects due to the tacrolimus ***,tacrolimus can be effective for the treatment of patients with steroidresistant CIDP.

低危孕妇孕产保健服务方案的临床实施效果评价

目的评价以助产士为主导的低危孕妇孕产保健服务方案的应用效果。方法将在上海市某三甲妇产科医院预约初诊建卡的低危孕妇随机分为干预组485例和对照组499例。对照组接受以产科医生为主导的常规孕产保健服务;干预组在对照组的基础上接受以助产士为主导的低危孕妇孕产保健服务方案,包括孕期8次助产士门诊访视、微信群组干预及电话访视、产时陪伴分娩及产后访视。结果干预组助产士门诊访视中位数为7次,分娩计划制定率和执行率分别为76.70%和84.68%。干预组剖宫产率为27.68%,显著低于对照组的35.25%(P<0.05)。干预组硬膜外镇痛率、人工破膜率、会阴切开率、新生儿转入NICU率、第三产程出血量、产后2 h出血量及总出血量显著低于对照组(均P<0.05)。干预组孕期保健服务体验、分娩体验、产后保健服务体验、孕晚期生殖健康素养、产前母乳喂养自我效能、分娩自我效能评分及出院前纯母乳喂养率显著高于对照组(均P<0.05)。结论以助产士为主导的低危孕妇孕产保健服务方案的临床实施有利于促进正常妊娠分娩,提升孕产保健服务体验,实现孕产妇和助产士之间的双重赋能。
