Splenic artery aneurysms (SAA) are third most common intra-abdominal aneurysm (after aneurysms of the abdominal aorta and iliac arteries). A splenic artery aneurysm larger than 3 cm is even rare. Splenic artery aneurysms are fatal vascular lesions if ruptured. The incidence of rupture is increased in larger aneurysms. Transcatheter arterial embolization is an alternative method of treatment in a patient with splenic arterial aneurysm which has relative lower morbidity and mortality than surgical procedures. Here, we report a case of giant ruptured proximal splenic artery pseudoaneurysm detected by contrast enhanced computed tomography (CECT) & referred to Interventional Radiology for Transcatheter arterial embolisation.

Preeclampsia is a pregnancy specific syndrome of elusive etiology, developing in 2nd trimester and associated with high maternal and perinatal morbidity and mortality. The spectrum ranges from mild preeclampsia with no systemic involvement to multi-system involvement. The course is unpredictable and delivery is the only curative treatment. Elevated blood pressure(> 160/110 mm Hg) should be reduced gradually to a safe level(140/90) using antihypertensive drugs. Prophylaxis and treatment of convulsions using Mg SO4 is indicated for severe preeclampsia. Fluid therapy is controversial due to potential delicate balance between constricted plasma volume and risk of fluid overload and pulmonary oedema secondary to increased capillary permeability and reduced colloid osmotic pressure. Single shot spinal anaesthesia is the technique of choice for caesarean delivery unless contraindicated. General anaesthesia is indicated in patients with coagulopathy or eclampsia but is associated with risk of difficult airway and exaggerated sympathetic response during laryngoscopy. Epidural analgesia and anaesthesia is safe in absence of coagulopathy.

Macrophage plasticity is critical for normal tissue repair following *** pathologic states such as diabetes,macrophage plasticity is impaired,and macrophages remain in a persistent proinflammatory state;however,the reasons for this are ***,using single-cell RNA sequencing of human diabetic wounds,we identified increased JMJD3 in diabetic wound macrophages,resulting in increased inflammatory gene ***,we report that in wound healing,JMJD3 directs early macrophage-mediated inflammation via JAK1,3/STAT3 ***,in the diabetic state,we found that IL-6,a cytokine increased in diabetic wound tissue at later time points post-injury,regulates JMJD3 expression in diabetic wound macrophages via the JAK1,3/STAT3 pathway and that this late increase in JMJD3 induces NFκB-mediated inflammatory gene transcription in wound macrophages via an H3K27me3 ***,RNA sequencing of wound macrophages isolated from mice with JMJD3-deficient myeloid cells(Jmjd3f/fLyz2Cre+)identified that the STING gene(Tmem173)is regulated by JMJD3 in wound *** limits inflammatory cytokine production by wound macrophages during ***,in diabetic mice,its role changes to limit wound repair and enhance *** finding is important since STING is associated with chronic inflammation,and we found STING to be elevated in human and murine diabetic wound macrophages at late time ***,we demonstrate that macrophage-specific,nanoparticle inhibition of JMJD3 in diabetic wounds significantly improves diabetic wound repair by decreasing inflammatory cytokines and *** together,this work highlights the central role of JMJD3 in tissue repair and identifies cell-specific targeting as a viable therapeutic strategy for nonhealing diabetic wounds.

目的:探讨肾茶对UUO大鼠肾组织Fas(Fas蛋白)、FasL(FasL蛋白)表达的影响。方法:将清洁级SD大鼠随机分为模型组、假手术组、肾茶组。模型组、肾茶组分离、结扎左侧输尿管,并离断;假手术组只分离左侧输尿管,不结扎、离断。肾茶组给予剂量为8g/kg的肾茶煎剂灌胃,模型组、假手术组给予等体积的生理盐水灌胃,分别于术后3d,7d,14d在每组大鼠中随机取8只,麻醉后心脏取血测定血尿素氮、血肌酐,处死大鼠,摘取左侧(术侧)肾脏后采用免疫组化方法检测大鼠肾组织中Fas和FasL的表达。结果:同模型组相比,术后3d、7d、14d肾茶组血尿素氮、血肌酐下降,肾间质纤维化程度减轻,Fas、FasL表达下调,各组间差异有统计学意义(P<0.05)。结论:肾茶能够延缓单侧输尿管结扎大鼠肾间质纤维化程度,其疗效可能是使进行性纤维化的肾组织Fas、FasL的表达降低。

AIM: To ascertain fine needle aspiration(FNA) tech-niques by endosonographers with varying levels of ex-perience and ***: A survey study was performed on United States based endosonographers. The subjects complet-ed an anonymous online electronic survey. The main outcome measurements were differences in needle choice, FNA technique, and clinical decision making among endosonographers and how this relates to years in practice, volume of EUS-FNA procedures, and prac-tice ***: A total of 210(30.8%) endosonographers completed the survey. Just over half(51.4%) identified themselves as academic/university-based practitioners. The vast majority of respondents(77.1%) identified themselves as high-volume endoscopic ultrasound(EUS)(> 150 EUS/year) and high-volume FNA(> 75 FNA/year) performers(73.3). If final cytology is non-diagnostic, high-volume EUS physicians were more likely than low volume physicians to repeat FNA with a core needle(60.5% vs 31.2%; P = 0.0004), and low volume physicians were more likely to refer patients for either surgical or percutaneous biopsy,(33.4% vs 4.9%, P < 0.0001). Academic physicians were more likely to repeat FNA with a core needle(66.7%) compared to community physicians(40.2%, P < 0.001). CONCLUSION: There is significant variation in EUS-FNA practices among United States endosonographers. Differences appear to be related to EUS volume and practice environment.

目的：探讨并得出吉林省健康儿童经体表面积（BSA）调整的冠状动脉的Z值,运用Z值来判断冠状动脉是否异常,从而指导可能影响冠状动脉的疾病的临床诊治.方法：研究收录了2009-05～2012-10吉林大学第一医院儿科超声门诊检查的15岁以下健康儿童,共998例.将其分成6组,分别为≤1岁、1～3岁、3～6岁、6～9岁、9～12岁、12～15岁.使用东芝SSH-880CV仪器分别测量左冠状动脉主干（LMCA）和右冠状动脉（RCA）内径.将测量的内径值转换为经BSA调整的Z值.结果：在不同年龄组的健康儿童之间经BSA调整的LMCA和RCA内径Z值存在明显差异.Z值在低年龄组之间以及高年龄组之间没有统计学意义.结论：该研究建立了吉林健康儿童冠状动脉Z值的正常区间,运用Z值来判断冠状动脉是否扩张,对于指导川崎病以及其他可能影响冠状动脉的疾病的临床诊治工作有着重要的意义.
