Medical comorbidities at admission is predictive for 30-day in-hospital mortality in patients with acute myocardial infarction： analysis of 5161 cases

Background The present study investigated the prognostic value of medical comorbidities at admission for 30-day in-hospital mortality in patients with acute myocardial infarction （AMI）. Methods A total of 5161 patients with AMI were admitted in Chinese PLA General Hospital between January 1, 1993 and December 31, 2007. Medical comorbidities including hypertension, diabetes mellitus, previous myocardial infarction, valvular heart disease, chronic obstructive pulmonary disease （COPD）, renal insufficiency, previous stroke, atrial fibrillation and anemia, were identified at admission. The patients were divided into 4 groups based on the number of medical comorbidities at admission （0, 1, 2, and ≥3）. Cox regression analysis was used to calculate relative risk （RR） and 95% confidence intervals （CI）, with adjustment for age, sex, heart failure and percutaneous coronary intervention （PCI）. Results The mean age of the studied population was 63.9 ± 13.6 years, and 80.1% of the patients were male. In 74.6% of the patients at least one comorbidity were identified. Hypertension （50.7%）, diabetes mellitus （24.0%） and previous myocardial infarction （12%） were the leading common comorbidities at admission. The 30-day in-hospital mortality in patients with 0, 1, 2, and ≥3 comorbidities at admission （7.2%） was 4.9%, 7.2%, 11.1%, and 20.3%, respectively. The presence of 2 or more comorbidities was associated with higher 30-day in-hospital mortality compared with patients without comorbidity （RR： 1.41, 95% CI： 1.13-1.77, P = 0.003, and RR： 1.95, 95% CI： 1.59-2.39, P = 0.000, respectively）. Conclusions Medical comorbidities were frequently found in patients with AMI. AMI patients with more comorbidities had a higher 30-day in-hospital mortality might be predictive of early poor outcome in patients with AMI.

Rock physical characteristics of deep dolomite under complex geological conditions: A case study of 4th Member of Sinian Dengying Formation in the Sichuan Basin, China

The deep-ultra deep carbonate reservoir in China, commonly subjected to modification of multi-stage diagenesis, has extremely high heterogeneity. Conventional rock physics analysis cannot accurately identify the elastic responses of reservoir. Here, the rock physics properties of the dolomite from the 4th Member of the Sinian Dengying Formation are experimentally measured, and the change law of rock physics characteristics is investigated within the framework of the diagenetic processes by the analysis of the elastic and petrologic characteristics, pore structure, and sedimentary environments. The results show that the differentiated diagenesis results in different pore structure characteristics and microtexture characteristics of the rock. The microbial dolomite of the algal mound-grain beach facies is subjected to the contemporaneous microbial dolomitization and seepage-reflux dolomitization, penecontemporaneous selective dissolution, burial dolomitization, and hydrothermal dolomitization. The resultant crystalline dolomite is found with one main type of the dolomite crystal contact boundaries,and the dissolution pore is extensive development. The siliceous, muddy, and limy dolomite of the interbeach sea environment mainly experiences the weak capillary concentration dolomitization, intensive mechanical compaction-induced densification, and burial dolomitization. Such crystalline dolomite is observed with four types of contact boundaries, namely the dolomite contact, clay contact, quartz contact, and calcite contact boundaries, and porosity mostly attributed to residual primary inter-granular or crystalline pores. The samples with the same crystal boundary condition have consistent correlations between the compressional-and shear-wave velocities, and between the compressional-wave velocity and the velocity ratio. Combined use of fly ash and silica to prevent the long-term strength retrogression of oil well cement set and cured at HPHT conditions

The long-term strength retrogression of silica-enriched oil well cement poses a significant threat to wellbore integrity in deep and ultra-deep wells, which is a major obstacle for deep petroleum and geothermal energy development. Previous attempts to address this problem has been unsatisfactory because they can only reduce the strength decline rate. This study presents a new solution to this problem by incorporating fly ash to the traditional silica-cement systems. The influences of fly ash and silica on the strength retrogression behavior of oil well cement systems directly set and cured under the condition of 200°C and 50 MPa are investigated. Test results indicate that the slurries containing only silica or fly ash experience severe strength retrogression from 2 to 30 d curing, while the slurries containing both fly ash and silica experience strength enhancement from 2 to 90 d. The strength test results are corroborated by further evidences from permeability tests as well as microstructure analysis of set cement. Composition of set cement evaluated by quantitative X-ray diffraction analyses with partial or no known crystal structure(PONKCS) method and thermogravimetry analyses revealed that the conversion of amorphous C-(A)-S-H to crystalline phases is the primary cause of long-term strength *** addition of fly ash can reduce the initial amount of C-(A)-S-H in the set cement, and its combined use with silica can prevent the crystallization of C-(A)-S-H, which is believed to be the working mechanism of this new admixture in improving long-term strength stability of oil well cement systems.

Wettability and corrosion behavior between alkaline slag from sodium smelting of vanadium-titanium magnetite and refractory substrates

With distinct advantages such as high gravimetric and volumetric capacity(5855 mAh·cm-3and 820mAh·g-1), low redox potential(-0.762 V vs. standard hydrogen electrode（SHE）), high abundance, low toxicity and intrinsic safety of Zn metal anode, Zn-ion batteries have become a potential alternative to li-ion ***, several challenges still need to be addressed prior to the practical applications of Zn-ion batteries, such as dendrite growth during Zn plating/stripping and interfacial side reactions on the Zn surface. Such issues can be addressed by introducing additives to regulate the components and structures of the electrolyte. In this review, we systematically discussed the core issues of metallic Zn anodes and comprehensively summarized a novel perspective of the regulation mechanism of inhibiting dendrite growth or interfacial side reactions in Zn anodes by introducing additives into aqueous ***, some discussions and prospects for aqueous Zn ion batteries（AZIBs） are presented for future research.

Development of a clinical nomogram for prediction of response to neoadjuvant chemotherapy in patients with advanced gastric cancer

BACKGROUND The efficacy of neoadjuvant chemotherapy(NAC)in advanced gastric cancer(GC)is still a controversial *** To find factors associated with chemosensitivity to NAC treatment and to provide the optimal therapeutic strategies for GC patients receiving *** The clinical information was collected from 230 GC patients who received NAC treatment at the Central South University Xiangya School of Medicine Affiliated Haikou Hospital from January 2016 to December *** absolute shrinkage and selection operator logistic regression analysis was used to find the possible predictors.A nomogram model was employed to predict the response to *** In total 230 patients were finally included in this study,including 154 males(67.0%)and 76 females(33.0%).The mean age was(59.37±10.60)years,ranging from 24 years to 80 *** to the tumor regression grade standard,there were 95 cases in the obvious response group(grade 0 or grade 1)and 135 cases in the poor response group(grade 2 or grade 3).The obvious response rate was 41.3%.Least absolute shrinkage and selection operator analysis showed that four risk factors significantly related to the efficacy of NAC were tumor location(Plinical T stage(P=0.008),and carbohydrate antigen 724(P=0.008).The C-index for the prediction nomogram was *** calibration curve revealed that the predicted value exhibited good agreement with the actual *** curve analysis showed that the nomogram had a good value in clinical *** A nomogram combining tumor location,histological differentiation,clinical T stage,and carbohydrate antigen 724 showed satisfactory predictive power to the response of NAC and can be used by gastrointestinal surgeons to determine the optimal treatment strategies for advanced GC patients.
