BACKGROUND Endoscopic rubber band ligation(ERBL)is a nonsurgical technique for the treatment of symptomatic internal hemorrhoids but is limited by recurrence and post-procedural *** To evaluate satisfaction,long-term recurrence,and post-procedural pain in managing internal hemorrhoids using a combination of polidocanol foam sclerotherapy and *** This was a prospective,multicenter,randomized study.A total of 195 consecutive patients diagnosed with grade II-III internal hemorrhoids were enrolled from four tertiary hospitals and randomly divided into a cap-assisted endoscopic polidocanol foam sclerobanding(EFSB)or an ERBL *** patients were followed-up for 12 ***-based severity and post-procedural pain were assessed using a hemorrhoid severity score(HSS)and a visual analog scale(VAS).Continuous variables were reported as medians and interquartile *** One hundred and ninety-five patients were enrolled,with 98 in the EFSB *** was lower in the EFSB group than in the ERBL group at 8 weeks[4.0(3.0-5.0)vs 5.0(4.0-6.0),P=0.003]and 12-month[2.0(1.0-3.0)vs 3.0(2.0-3.0),Plinear regression analysis demonstrated that EFSB treatment[B=-0.915,95%confidence interval(CI):−1.301 to−0.530,P=0.001]and rubber band number(B=0.843,95%CI:0.595-1.092,Plief from the recurrence of prolapse and pain 24 hours post-procedure.

Postoperative cognitive dysfunction(POCD)remains a major issue that worsens the prognosis of elderly surgery *** article reviews the current research on the effect of different anesthesia methods and commonly utilized anesthetics on the incidence of POCD in elderly patients,aiming to provide an understanding of the underlying mechanisms contributing to this condition and facilitate the development of more reasonable anesthesia protocols,ultimately reducing the incidence of POCD in elderly surgery patients.

Due to the high electrophilic nature of azo-dienophiles, azo-Diels–Alder proceeds rapidly even without the need of a catalyst and is therefore regarded as the “click reaction”. This spontaneity causes strong background reaction and poses a daunting challenge to chemists for developing the catalytic asymmetric version. Reported herein is the first catalytic asymmetric dearomative azo-Diels–Alder reaction between2-vinylindoles and triazoledione. This protocol makes use of the high energy barrier of dearomatization to avert the strong background reaction of azo-Diels–Alder reaction, allowing the implementation of the projected reaction at ambient temperature. Density functional theory calculations have been performed to gain insights into the reaction mechanism and the origins of the enantioselectivity. By using this method,a variety of tetracyclic indole derivatives have been readily prepared in good to excellent yields and with excellent diastereo-and enantio–selectivities(33 examples, up to 97% yield and >99% ee, >20:1 dr).

Owing to the broad energy coverage of Insight-HXMT in the hard X-ray band,we detected the highest energy of pulsation exceeding 200 keV around the 2017–2018 outburst peak of the first Galactic pulsating ultraluminous X-ray source (PULX) Swift J0243.6+6124,which is the highest energy detected from PULXs to *** also obtained the highest energy of pulsation of every exposure during the outburst in 2017–2018,and found the highest energy is roughly positively correlated with *** our newly developed method,we identified the critical luminosity being 4×10^(38)erg s^(-1) when the main peaks of the low and high energy pulse profiles became aligned,which separates the fan-beam dominated and pencil-beam dominated accretion *** the critical luminosity,the phase of the main peak shifted gradually from 0.5 to 0.8 until the outburst peak in al energy bands is reached,which is in agreement with the phase shift found previously at low *** result is consistent with what is derived from spectral analysis.

Slickwater fracturing fluids are widely used in the development of unconventional oil and gas resources due to the advantages of low cost,low formation damage and high drag reduction ***,their performance is severely affected at high *** reducing agent is the key to determine the drag reducing performance of *** this work,in order to further improve the temperature resistance of slickwater,a temperature-resistant polymeric drag reducing agent(PDRA)was synthesized and used as the basis for preparing the temperature-resistant *** slickwater system was prepared with the compositions of 0.2 wt%PDRA,0.05 wt%drainage aid nonylphenol polyoxyethylene ether phosphate(NPEP)and 0.5 wt%anti-expansion agent polyepichlorohydrindimethylamine(PDM).The drag reduction ability,rheology properties,temperature and shear resistance ability,and core damage property of slickwater were systematically studied and *** contrast to on-site drag reducing agent(DRA)and HPAM,the temperature-resistant slickwater demonstrates enhanced drag reduction efficacy at 90℃,exhibiting superior temperature and shear resistance ***,the drag reduction retention rate for the slickwater achieved an impressive 90.52%after a 30-min shearing ***,the core damage is only 5.53%.We expect that this study can broaden the application of slickwater in high-temperature reservoirs and provide a theoretical basis for field applications.
