BACKGROUND Acute hyperglycemia due to insulin resistance is common in critically ill patients,typically managed with insulin ***,the occurrence of transient extreme insulin resistance(EIR)requiring exceptional high-dose insulin is *** SUMMARY We present the case of a 68-year-old woman with pneumonia who suffered an out-of-hospital cardiac arrest,subsequently developing transient EIR following a new episode of ***,insulin resistance rapidly reversed when the insulin infusion rate peaked at 960 units/hour(a total of 18224 units on that day),and it was promptly titrated down to zero upon achieving the target glucose *** Exceptional high-dose insulin infusion may be required in critically ill patients with stress-related EIR,which is typically *** should be aware of the phenomenon and cautious to avoid hypoglycemia and fluid overload during the steep titration of high-dose insulin infusion.

This paper proposes a new adaptive post-filtering algorithm to remove coding artifacts in block-based video coder. The proposed method concentrates on blocking and ringing artifacts removal. For de-blocking, the blocking strength is identified to determine the filtering range, and the maximum quantization parameter of the image is used to adapt the 1D fuzzy filter. For de-ringing, besides the edge detection, a complementary ringing detection method is proposed to locate the neglected ringing blocks, and the gradient threshold is adopted to adjust the parameter of 2D fuzzy filter. Experiments are performed on the MPEG-4 sequences. Compared with other methods, the proposed one achieves better detail preservation and artifacts removal performance with lower computational cost.

Using ethanol or acetone as the working fluid, visualization of oscillations in steady state was observed visually by high-speed cameras, and temperature oscillating and heat transfer characteristics of closed-loop plate oscillating heat pipe with parallel channels(POHP-PC) were experimentally investigated by varying liquid filled ratios(50%, 70%, 85%), section scales(1 mm×1 mm and 1 mm×1.5 mm), inclination angles, working fluids and heating inputs. It was found that during operating there was mixed flow consisting of plug flow and annular flow in channels of oscillating heat pipe at steady-state. There was an equilibrium position for working fluid of condenser during oscillating, and periodic oscillations occurred up and down in the vicinity of equilibrium position. With heat input increasing, equilibrium position rose slowly as a result of vapor pressure of *** temperature oscillating amplitude possessed a trend of small-large-small and frequency trend was of small-large during steady-state. It may be generally concluded that temperature, whether evaporator or condenser, fluctuated sharply or rose continuously when oscillating heat pipe coming to dry burning state. Simultaneously, it was found that temperature difference of cooling water possibly dropped with heat input rising during dry burning state. Thermal resistance of No. 2 with acetone was lower than that of No. 1 during experiments, but No. 2 achieving heat transfer limit was earlier than No. 1. However, with ethanol, thermal resistance of No. 1 and No. 2 were similar with the heating input less than 110-120 W and filling ratios of 50% and 70%. And with filling ratio of 85%, heating transfer performance of No. 2 was better compared to No. 1 during all the experiments.

Bronchoesophageal fistulas are usually diagnosed in the neonatal period. As such, the condition is rare in adults. We present a case of a congenital bronchoesophageal fistula in a 62-year-old man with the complaint of severe bouts of cough and choking after swallowing liquid. His workup included a barium esophagogram that revealed a fistula between the esophagus and a right lower lobe bronchus. The diagnosis should be considered in certain individuals with suggestive symptomatology and unexplained respiratory pathology. The fistula was divided and resected, The patient had an uneventful recovery.

A novel interpenetrating structure [（HMA^＋）（FB^- ）] ·2H2O of melamine（MA） with 4-fluoro-benzoic acid （HFB） was synthesized. It crystallized in the monoclinic system with space group P21/c. The complex has a interpenetrating 2D structure with hydrogen-bonded grid networks. Carboxylic acid to melamine proton transfer occurs in the complex. The adjacent HMA^＋ cations related by an inverse center form cationic [HMA^＋]∞ ribbons via a pair of N-H…N hydrogen bonds. Adjacent FB-anions are paired by C-H…F hydrogen bonds to form dimers, which are connected to [HMA^＋]∞ ribbons through N-H…O interactions. The supramolecular features in the complex are guided by control of strong N-H…N, N-H…O and O-H…O hydrogen bonds, as well as highly directional weak C-H…F interactions and aromatic π-π stacking interactions.

Mathematical model of the direct reduction of dust composite pellets containing zinc and iron

Direct reduction of dust composite pellets containing zinc and iron was examined by simulating the conditions of actual production process of a rotary hearth furnace （RHF） in laboratory. A mathematical model was constructed to study the reduction kinetics of iron oxides and ZnO in the dust composite pellets. It was validated by comparing the calculated values with experimental results. The effects of furnace temperature, pellet radius, and pellet porosity on the reduction were investigated by the model. It is shown that furnace temperature has obvious influence on both of the reduction of iron oxides and ZnO, but the influence of pellet radius and porosity is much smaller. Model calculations suggest that both of the reduction of iron oxides and ZnO are under mixed control with interface reactions and Boudouard reaction in the early stage, but only with interface reactions in the later stage.

Ab initio Study of Anharmonic Force Field and Spectroscopic Constants for Germanium Dichloride

Ab initio study of the equilibrium structure, spectroscopy constants, and anharmonic force field for several isotopomers of germanium dichloride （70GeCl2, 72GECl2, and 76GeCl2） have been carried out at the MP2 and CCSD（T） levels of theory using cc-pVTZ basis set. The cal- culated geometries, rotational constants, vibration-rotation interaction constants, harmonic frequencies, anharmonic constants, quartic and sextic centrifugal distortion constants, cubic and quartic force constants are compared with experimental data. For small mass differences of the Ge isotopes, the isotopic effects for germanium dichloride are much weaker. The agreements are satisfactory for these two methods, but the deviations of CCSD（T） results are slightly larger than that of MP2, because of CCSD（T）＇s inadequate treatment of electron correlation in hypervalent Cl atom.
