BACKGROUND Tumor budding(TB)has emerged as a promising independent prognostic biomarker in colorectal cancer(CRC).The prognostic role of TB has been extensively studied and currently affects clinical decision making in patients with stage I and II ***,existing prognostic studies on TB in stage III CRC have been confined to small retrospective cohort ***,this study investigated the correlation among TB categories,clinicopathological features,and prognosis in stage III-IV CRC to further enhance the precision and individualization of treatment through refined prognostic risk *** To analyze the relationship between TB categories and clinicopathological characteristics and assess their prognostic value in stage III-IV CRC to further refine the prognostic risk stratification of stage III-IV *** The clinical data of 547 CRC patients were collected for this retrospective *** at the front edge of the tumor buds was counted according to the 2016 International Tumor Budding Consensus Conference *** Multivariate Cox proportional hazards regression analysis demonstrated that chemotherapy(P=0.004),clinical stage IV(P<0.001),≥4 regional lymph node metastases(P=0.004),left-sided colonic cancer(P=0.040),and Bd 2-3(P=0.002)were independent prognostic factors in patients with stage III-IV ***,the density of tumor infiltrating lymphocytes was higher in Bd 1 than in Bd 2-3,both in the tumor stroma and its invasive *** TB has an independent predictive prognostic value in patients with stage III-IV *** is recommended to complete the TB report of stage III-IV CRC cases in the standardized pathological report to further refine risk stratification.

广州南沙某项目的深基坑采用了新型的砼芯类刚性大直径三重管(搅拌)旋喷桩+二道混凝土内支撑的组合支护结构体系。为探究该新型结构体系中钢筋混凝土加劲桩、大直径三重管(搅拌)旋喷桩及压顶板在不同工况条件下的相互作用效果,运用三维有限元分析方法,对支护体系的稳定性(主要包括位移、内力)进行数值模拟仿真分析,为保障施工安全提供数据支持。

近年来,随着新型城镇化建设工作的快速推进,大量的老旧建筑需要进行改扩建,改扩建工程迎来新的发展机遇。锚杆静压桩由于受施工场地限制较小、施工振动小等优点被广泛应用于改扩建工程的旧有地基加固处理。本文针对某改扩建工程原有地基承载力不足、需要进行地基加固处理这一实际情况,通过理论分析、方案论证,最终确定使用锚杆静压桩进行地基加固并进行了工艺创新,具体包括:运用BIM技术进行地基加固施工工序模拟及细部节点模拟实现构配件的标准化生产,设计使用了一种凹凸咬合型连接钢板用于方桩接桩提高桩抵抗拉裂、抗弯剪的能力,采用钢模板外包气珠薄膜增强体系结构的内部粘结等。本技术解决了锚杆静压桩施工易断桩、整体性不易保证等问题,提高了桩基施工质量,保证了施工安全,为类似项目提供了借鉴和参考。

目的探讨伴有色素分化的Xp11易位性肿瘤/“伴有黑色素分化的TFE3基因重排的软组织肿瘤”的临床病理特征、免疫表型、分子特征及预后。方法收集5例伴有色素分化的Xp11易位性肿瘤/“伴有黑色素分化的TFE3基因重排的软组织肿瘤”的临床病理资料,行免疫组化EnVision法、组织化学染色、FISH、二代测序(NGS)及随访,并进行文献复习。结果5例患者中男性1例,女性4例。发病年龄16~59岁,平均28.2岁。肿瘤最大径3.0~6.0 cm(平均4.7 cm)。发病部位:右侧肾脏3例(其中1例发生胸椎转移),左侧输卵管间质与圆韧带之间1例,盆腔部位1例。组织学上,肿瘤由丰富的纤维血管网分隔成巢状、腺泡状结构,胞质透亮或嗜酸性、颗粒样,胞质内不同程度的出现黑色素聚集;间质内可见淋巴细胞浸润;4例检出肿瘤性坏死;核分裂象<3个/10 HPF;2例检出个别脉管内瘤栓,其余3例均未检出脉管内瘤栓及神经侵犯。免疫表型:5例TFE3均弥漫强阳性,HMB45及Melan A不同程度阳性;CK(AE1/AE3)、CK7、EMA、PAX8、TFEB、S-100、SOX10、SMA、desmin均阴性;Ki67增殖指数<20%。FISH检测:TFE3分离探针显示4例存在明确的TFE3分离信号,1例因信号不典型判读为阴性,经NGS检测证实为RBM10-TFE3基因融合。5例均获得随访:随访时间2~60个月,患者均存活,4例无瘤生存,1例胸椎转移患者情况稳定且未进展。结论伴有色素分化的Xp11易位性肿瘤/“伴有黑色素分化的TFE3基因重排的软组织肿瘤”具有独特的形态学特征、免疫表型及分子遗传学改变,建议作为一种独立的恶性间叶性肿瘤实体进行分类。

Objective:Lynch syndrome(LS)pre-screening methods remain under-investigated in colorectal cancers(CRCs)in ***,we aimed to systematically investigate LS pre-screening and comprehensively characterize LS ***:Microsatellite instability(MSI)and germline variants of DNA mismatch repair(MMR)genes were examined in 406 deficient MMR(dMMR)and 250 proficient MMR *** genetic differences between LS and sporadic CRCs were studied with whole exome sequencing ***:The incidence of dMMR in Chinese patients with CRCs was 13.8%.Consistency analysis between MMR immunohistochemistry(IHC)and MSI testing showed the kappa value was *** next-generation sequencing(NGS),germline variants were detected in 154 ***,88 patients with CRC were identified as having LS by Sanger *** them,we discovered 21 previously unreported pathogenic germline variants of MMR *** patients with LS,compared with sporadic CRCs,tended to be early-onset,right-sided,early-stage and ***,the performance of MMR IHC and MSI testing for LS pre-screening was comparable:the area under the ROC curve for dMMR,MSI-H,and MSI-H/L was 0.725,0.750,and 0.745,***_MSI-H LS and sporadic CRCs showed substantial differences in somatic genetic characteristics,including different variant frequencies of APC,CREBBP,and KRAS,as well as different enriched pathways of VEGF,Notch,TGFβR,mTOR,ErbB,and Rac protein signal ***:MMR IHC and MSI testing were effective methods for LS *** revealed clinical and somatic genetic characteristics in LS CRCs may have the potential to improve the performance of LS pre-screening in combination with dMMR/MSI.
