目的:分析居家医养照护团队在居家失能老年人服务体系中发挥的作用。方法:选择2022年1月至12月我院管理的100例居家失能老年人为研究对象,居家医养照护团队对其加强居家康复护理服务及指导、健康风险指导、心理支持、营养指导及干预,并比较干预前后管理成效。结果:干预后照护者或失能老人对健康风险管理、心理支持、营养干预的掌握度,康复锻炼的依从率均高于干预前,压力性损伤发生率低于干预前(P<0.05);干预后老人对居家医养照护团队工作的满意率为95%。结论:居家医养照护团队在居家失能老年人护理服务体系中发挥着重要作用,对于构建机构-社区-居家三级服务健康管理体系扮演着重要角色。
