Report of China's innovation increase and research growth in radiation oncology

Aims： To investigate the research status of radiation oncology in China through survey of literature in international radiation oncology journals and retrospectively compare the outputs of radiation oncology articles of the three major regions of China--Mainland （ML）, Taiwan （TW） and Hong Kong （HK）. Methods： Radiation oncology journals were selected from ＂oncology＂ and ＂radiology, nuclear ＆ medical image＂ category from Science Citation Index Expand （SCIE）. Articles from the ML, TW and HK were retrieved from MEDLINE. The number of total articles, clinical trials, case reports, impact factors （IF）, institutions and articles published in each journals were conducted for quantity and quality comparisons. Results： A total 818 articles from 13 radiation oncology journals were searched, of which 427 are from ML, 259 from TW, and 132 from HK. Ninety-seven clinical trials and 5 case reports are reported in China. Accumulated IF of articles from ML （1,417.11） was much higher than that of TW （1,003.093） and HK （544.711）, while the average IF of articles from ML is the lowest. Conclusions： The total number of articles from China especially ML increased significantly in the last decade. The number of articles published from the ML has exceeded those from TW and HK. However, the quality of articles from TW and HK is better than that from ML.

Intermolecular reductive coupling of aliphatic ketones with alkynes and alkenes by BCN photocatalysts

A photocatalytic process was developed for intermolecular reductive coupling of aliphatic ketones with alkynes/alkenes to furnish tertiary alcohols. Mechanistic studies support that boron carbonitrides(BCN) promote the single electron reduction of unactivated aliphatic ketones to generate ketyl radicals with visible light irradiation, which then react with alkynes and alkenes to afford the allylic and aliphatic alcohols, respectively. The heterogeneous photocatalytic system is compatible with diverse substrates including pharmaceutical-related molecules(43 examples) with reusability and scale-up experiments, verifying the complementary synthetic utility of this method to the homogeneous catalysts.

Targets and molecular mechanisms of triptolide in cancer therapy

Triptolide（TPL/TL） is a natural drug with novel anticancer effects. Preclinical studies indicated that TPL inhibits cell proliferation, induces cell apoptosis, inhibits tumor metastasis and enhances the effect of other therapeutic methods in various cancer cell lines. Multiple molecules and signaling pathways, such as caspases, heat-shock proteins, NF-κB, and deoxyribonucleic acid（DNA） repair-associated factors, are associated with the anti-cancer effect. TPL also improves chemoradiosensitivity in cancer therapy. Phase I trials indicate the potential clinical value of TPL use. However, further trials with larger sample sizes are needed to confirm these results.

SBRT杀伤肿瘤细胞的机制可能与常规分割放疗不同，可体现在以下两个方面：单次大剂量照射可使肿瘤细胞膜破坏，释放肿瘤特异性抗原，成为“原位肿瘤疫苗”，继而可激发免疫系统杀灭残余肿瘤；当单次分割剂量〉8～10Gy时，照射1～6h后可通过酸性鞘磷脂酶途径介导血管内皮细胞快速凋亡，进而肿瘤血管闭塞，出现二次肿瘤杀伤效应。本文综述原发性肝癌利用SBRT临床研究进展，旨在了解其是否通过上述机制增加了临床疗效。

目的比较老年食管鳞癌患者(年龄≥65岁)单纯放疗(RT)和同步放化疗(CCRT)的有效性和安全性。方法收集2010年1月至2016年12月于本院首程接受根治性放疗的122例老年食管鳞癌患者,根据治疗情况分为RT组81例和CCRT组41例(替吉奥单药23例、奈达铂单药2例、雷替曲塞联合奈达铂7例和多西他赛联合奈达铂5例),采用实体瘤疗效评价标准RECIST 1.1版评价近期疗效,药物毒性反应标准NCI-CTC 4.0版评价药物毒副反应,急性放射反应评分标准(RTOG/EORTC)评价放疗毒副反应,根据随访数据进行预后分析并采用Cox比例风险回归模型进行多因素分析。结果RT组的总有效率和疾病控制率分别为84.0%和98.8%,与CCRT组92.7%和100.0%的差异无统计学意义(P>0.05)。RT组的中位无进展生存期(PFS)和总生存期(OS)分别为16.1个月和23.8个月,与CCRT组19.7个月和27.1个月的差异无统计学意义(P>0.05)。单因素分析显示影响PFS的因素包括ECOG评分、T分期和放疗剂量,影响OS的因素包括ECOG评分、T分期、N分期、临床分期和放疗剂量,多因素分析显示ECOG评分和放疗剂量是影响PFS的独立预后因素,而ECOG评分和N分期是影响OS的独立预后因素。最常见急性不良反应为放射性食管炎和骨髓抑制(多为1~2级),RT组未发生3~4级不良反应,CCRT组3~4级急性放射性食管炎、放射性肺炎和骨髓抑制的发生率升高至7.3%(3/41)、9.6%(4/41)和7.3%(3/41),差异均有统计学意义(P<0.05)。结论老年食管鳞癌行根治性放疗,放疗剂量提高至60~65 Gy,患者耐受性良好,可延长生存并提高生活质量。是否同步放化疗应综合评估后慎重选择。

目的探讨微小RNA-320(miR-320)对食管鳞癌(ESCC)放射敏感性的影响及可能机制。方法采用实时荧光定量PCR(QPCR)检测正常食管上皮细胞株HEEC和ESCC细胞株KYSE-150、TE-13和Eca-109中miR-320和叉头框蛋白M1(FOXM1)的表达水平。采用脂质体法将miR-320模拟物(mimics)及其阴性对照(NC)转染至Eca-109细胞,分为转染组和对照组。QPCR和Western blotting检测miR-320 mimics转染后FOXM1的蛋白和mRNA水平以评价miR-320对FOXM1的调控作用。克隆形成实验评价克隆形成能力;免疫荧光实验检测γH2AX荧光焦点数;AnnexinⅤ-FITC/PI双染法测定细胞凋亡;Western blotting测定XIAP、Bax、Bcl-2和cleaved caspase-3的表达水平;采用双荧光素酶报告基因实验验证miR-320与FOXM1之间的靶向关系。结果 QPCR检测显示,Eca-109细胞中miR-320相对表达量最低,FOXM1相对表达量最高,故选取该细胞株进行后续实验。转染48 h后,转染组miR-320表达量显著高于对照组;QPCR和Western blotting检测发现转染组FOXM1 mRNA和蛋白水平均低于对照组,差异均有统计学意义(P0.05)。结论 miR-320能增强ESCC细胞放射敏感性,可能与靶向抑制FOXM1表达有关。

Background:Concurrent chemoradiotherapy(CCRT)is the standard treatment for locally advanced esophageal squamous cell carcinoma(ESCC).However,the optimal radiotherapy regimen,particularly in terms of total dose and planned range of irradiation field,remains *** phase III clinical trial aimed to compare the survival benefits between different radiation doses and different target ***:This trial compared two aspects of radiation treatment,total dose and field,using a two-by-two factorial *** high-dose(HD)group received 59.4 Gy radiation,and the standard-dose(SD)group received 50.4 *** involved field irradiation(IFI)group and elective nodal irradiation(ENI)group adopted different irradiation *** participants were assigned to one of the four groups(HD+ENI,HD+IFI,SD+ENI and SD+IFI).The primary endpoint was overall survival(OS),and the secondary endpoints included progressionfree survival(PFS).The synergy indexwas used to measure the interaction effect between dose and ***:The interaction analysis did not reveal significant synergistic effects between the dose and irradiation *** comparison to the target field,patients in IFI or ENI showed similar OS(hazard ratio[HR]=0.99,95%CI:0.80-1.23,p=0.930)and PFS(HR=1.02,95%CI:0.82–1.25).TheHDtreatment did not show significantly prolonged OS compared with SD(HR=0.90,95%CI:0.72–1.11,p=0.318),but it suggested improved PFS(25.2 months to 18.0 months).Among the four groups,the HD+IFI group presented the best survival,while the SD+IFI group had the worst *** significant difference in the occurrence of severe adverse events was found in dose or field ***:IFI demonstrated similar treatment efficacy to ENI in CCRT of *** HD demonstrated improved PFS,but did not significantly improve *** dose escalation based on IFI(HD+IFI)showed better therapeutic efficacy than the current recommendation(SD+ENI)and is worth further validation.

目的:基于网络药理学和分子对接探讨苍耳子散化裁方治疗鼻窦炎的作用机制。方法:通过中药系统药理学数据库与分析平台(Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform,TCMSP)筛选出方剂的药物成分及其对应的成分靶标,将UniProt数据库筛选的相关药物靶标和GeneCards、CTD数据库筛选的鼻窦炎疾病靶标取交集;运用STRING平台构建蛋白质-蛋白质相互作用网络;采用Metascape平台对靶标进行基因本体论(GO)功能富集分析、京都基因与基因组百科全书(KEGG)通路富集分析。利用Cytoscape 3.8.2软件构建药物-成分-靶标网络图,最后使用Autodock软件对关键靶标及化合物进行分子对接,选择最佳的结合靶标。结果:筛选出相关活性成分57个,成分靶标与疾病靶标相交得出217个靶标。其主要活性成分为槲皮素(Quercetin)、木犀草素(Luteolin)、β-谷甾醇(Beta-Sitosterol)、金合欢素(Acacetin)、柚皮素(Naringenin)、豆甾醇(Stigmasterol)、芦荟大黄素(Aloe-emodin)等。其核心基因为丝氨酸/苏氨酸蛋白激酶1(Serine/threonine Protein Kinase 1,AKT1)、肿瘤蛋白p53(Tumor Protein p53,TP53)、肿瘤坏死因子(Tumor Necrosis Factor,TNF)、白细胞介素(Interleukin,IL)-6、半胱氨酸天冬氨酸蛋白酶-3(Caspase-3,CASP3)等。在分子对接中,其主要活性成分与关键靶标对接结果的能量均低于-5 kcal/mol。结论:通过网络药理学和分子对接研究了苍耳子散化裁方治疗鼻窦炎的潜在有效成分和作用机制,为今后进一步的机制研究与实验设计提供了依据。
