Platelet transfusion refractoriness after T-cell-replete haploidentical transplantation is associated with inferior clinical outcomes

Haploidentical stem cell transplantation (haplo-SCT) has been an alternative source of bone marrow for patients without human leukocyte antigen (HLA)-matched donors. The aim of this study was to investigate the relationships between platelet transfusion refractoriness (PTR) and clinical outcomes in the setting of haplo-SCT. Between May 2012 and March 2014, 345 patients who underwent unmanipulated haplo-SCT were retrospectively enrolled. PTR occurred in 20.6% of all patients. Patients in the PTR group experienced higher transplant-related mortality (TRM, 43.7% vs. 13.5%, P12 platelet (PLT) transfusions (third quartile of PLT transfusions) were higher than in patients receiving either >6 (second quartile) or >3 (first quartile) PLT transfusions (56.1% vs. 41.6% vs. 28.2%,respectively; P<0.001). The multivariate analysis also showed that PTR was associated with the number of PLT transfusions(P<0.001). PTR could predict poor transplant outcomes in patients who underwent haploidentical SCT.

In this paper,the atomization characteristics of Si3N4 ceramic dry granulation affect the performance of Si3N4 ceramic bearing balls.In order to improve the dry granulation characteristics and the comprehensive performance of Si3N4 ceramic bearing balls,the atomization mechanism of the spinning nozzle used for Si3N4 dry granulation was studied in detail.The interaction between air and binder in the pressure-swirl nozzle is analyzed based on VOF method,the modified realizable k−εturbulence model is used to simulate the flow field inside and outside the pressure-swirl nozzle,the effects of nozzle inlet parameters including the number of tangential inlets and the deflection angle of tangential inlets on the binder volume fraction,velocity distribution and pressure distribution are analyzed.The results show that when the number of tangential inlets increases from 1 to 4,the swirl strength of gas–liquid two-phase in the nozzle increases,the mean diameter of air core increases from 1.51 mm to 2.01 mm,and the spray cone angle increases from 18.5◦to 26.4◦.Besides,when the deflection angle of tangential inlet increases from 0◦to 15◦,the swirl strength of gas–liquid two-phase in the nozzle with the deflection angle of tangential inlet of 10◦is the largest,and the mean diameter of air core and spray cone angle is 3.04 mm and 30.7◦,respectively.Based on the atomization experiment platform of the electric control fuel system,the mean diameter of air core and spray cone angle are measured,the micromorphology of Si3N4 particles is observed,which verifies the correctness of numerical simulation.When the Si3N4 particles are prepared by dry granulation,taking the atomization performance of nozzle into consideration,the pressure-swirl nozzle with 4 tangential inlets and 10◦deflection angle should be selected.

Positron emission tomography(PET)represents molecular imaging for non-invasive phenotyping of physiological and biochemical processes in various oncological diseases.PET imaging with 18F-fluorodeoxyglucose(18F-FDG)for glucose metabolism evaluation is the standard imaging modality for the clinical management of lymphoma.One of the 18F-FDG PET applications is the detection and pre-treatment staging of lymphoma,which is highly sensitive.18F-FDG PET is also applied during treatment to evaluate the individual chemo-sensitivity and accordingly guide the response-adapted therapy.At the end of the therapy regiment,a negative PET scan is indicative of a good prognosis in patients with advanced Hodgkin’s lymphoma and diffuse large B-cell lymphoma.Thus,adjuvant radiotherapy may be alleviated.Future PET studies using non-18F-FDG radiotracers,such as 68Ga-labeled pentixafor(a cyclic pentapeptide that enables sensitive and high-contrast imaging of C-X-C motif chemokine receptor 4),68Ga-labeled fibroblast activation protein inhibitor(FAPI)that reflects the tumor microenvironment,and 89Zr-labeled atezolizumab that targets the programmed cell death-ligand 1(PD-L1),may complement 18F-FDG and offer essential tools to decode lymphoma phenotypes further and identify the mechanisms of lymphoma therapy.

To study the influence of different levels of Bacillus subtilis on growth performance, nutrition metabolism and intestinal microflora of 1 to 42 d Arbor Acres(AA) broilers, a total of 800 one-day-old healthy broilers were randomly divided into 5 groups with 4 replicates per group and 40 broilers per replicate. Broilers were fed a basic diet(group 1) which acted as the control group, and 4 other groups(2 to 5) were fed the basal diet with B. subtilis added at concentrations of 100, 150, 200 and 250 mg/kg, respectively for 42 days. The results showed as follow: the average daily gain(ADG) of group 4 was significantly higher than(P 0.05). The feed to gain ratio(F/G) of all the experimental groups was lower than that of the control and the difference was significant in group 4(P 0.05), crude fat(P > 0.05), dry matter(P > 0.05) and organic matter(P < 0.05). B. subtilis decreased the Escherichia coli and Salmonella populations in the cecum. This shows that adding B. subtilis to the broiler diet can improve the growth performance, increase feed efficiency, regulate serum index and reduce harmful bacteria in the intestinal tract. Based on our study, it could be recommended that addition of B. subtilis at 200 mg/kg could improve the growth performance of broilers.

绿色制造技术是当今制造业的发展潮流,迎合了可持续发展理念的发展要求。金属刀具切削加工也朝着绿色、无污染、高效、低耗、节能的绿色干切削加工方向发展。基于该理念,介观几何特征刀具概念被提出。介观几何特征(刃口、微织构)是介于宏观和微观尺度的微小特征。刀具表面置入微织构可以减小刀-屑接触长度、捕获磨粒和切屑等杂质,从而改善刀具切削性能、提高工件表面质量;刀具刃口处理同样可以提高刀具耐磨性、改善切削状态及已加工表面质量。研究发现,介观几何特征对加工过程中力-热特性、工件表面质量以及刀具寿命等产生重要影响。因此,总结归纳了介观几何特征对刀具各方面切削性能影响的研究现状,探讨介观几何特征对刀具综合切削性能的影响规律,并为后续介观几何特征刀具的优化设计及制备提供参考。

Comparison of efficacy between HLA6/6-and HLA3/6-matched haploidentical hematopoietic stem cell transplant in T-cell-replete transplants between parents and children

To compare the efficacy of HLA6/6-matched haploidentical hematopoietic stem cell transplant(haplo-HSCT) with that of HLA3/6-matched HSCT in T-cell-replete transplants, we recruited 27 consecutive recipients from multiple centers who received HLA6/6-matched haplo-HSCT from a parent or child donor between February 2010 and May 2016. A matched-pair analysis was designed. For each recipient from the study cohort, two recipients were randomly selected from the control cohort and matched(according to patient age, patient sex, disease type, disease status, donor age, donor sex, and recipient-donor relationship). No significant differences were found in hematopoietic recovery. The incidence of grade II–IV and III–IV acute graft versus host disease was similar(18.5% vs. 31.5%, P=0.216; 11.1% vs. 9.3%, P=0.792) in the HLA6/6 and HLA3/6 groups, respectively. The3-year cumulative incidence of relapse was 14.8% and 17.0%(P=0.800). The 3-year cumulative incidence of nonrelapse mortality was 12.1% and 17.6%(P=0.751). The estimated 3-year disease-free survival was 73.1% and 65.5%(P=0.489). The estimated 3-year overall survival was 74.7% and 74.0%(P=0.946). The data suggested the efficacy and safety of the HLA6/6-and the HLA3/6-matched haplo-HSCT between parents and children are comparable. That HLA-mismatch disparity is not correlated with T-cell-replete haplo-HSCT outcome was substantiated.

To establish optimal reference values for recovered immune cell subsets, we prospectively investigated post-transplant immune reconstitution （IR） in 144 patients who received allogeneic stem ceil transplantation （alio- SCT） and without showing any of the following events： poor graft function, grades II-IV acute graft-versus-host disease （GVHD）, serious chronic GVHD, serious bacterial infection, invasive fungal infection, or relapse or death in the first year after transplantation. IR was rapid in monocytes, intermediate in lymphocytes, CD3~ T cells, CD8~ T cells, and CD19~ B cells, and very slow in CD4~ T cells in the entire patient cohort. Immune recovery was generally faster under HLA-matched sibling donor transplantation than under haploidentical transplantation. Results suggest that patients with an IR comparable to the reference values display superior survival, and the levels of recovery in immune ceils need not reach those in healthy donor in the first year after transplantation. We suggest that data from this recipient cohort should be used as reference values for post-transplant immune ceil counts in patients receiving HSCT.
