BACKGROUND The efficacy and safety of proprotein convertase subtilisin/kexin type 9(PCSK-9) inhibitors were confirmed by several clinical trials, but its effectiveness in routine clinical practice in China has not been evaluated. This study aims to describe the real world effectiveness of PCSK-9 inhibitors combined with statins compared with statins-based therapy among patients with very high risk of atherosclerotic cardiovascular disease(ASCVD).METHODS This is a multi-center observational study, enrolled patients from 32 hospitals who underwent percutaneous coronary intervention(PCI) from January to June in 2019. There are 453 patients treated with PCSK-9 inhibitors combined with statins in PCSK-9 inhibitor group and 2,610 patients treated with statins-based lipid lowering therapies in statins-based group. The lipid control rate and incidence of major adverse cardiovascular events(MACE) over six months were compared between two groups.A propensity score-matched(PSM) analysis was used to balance two groups on confounding factors. Survival analysis using Kaplan-Meier methods was applied for *** In a total of 3,063 patients, 89.91% of patients had received moderate or high-intensity statins-based therapy before PCI, but only 9.47% of patients had low-density lipoprotein cholesterol(LDL-C) levels below 1.4 mmol/L at baseline. In the PSM selected patients, LDL-C level was reduced by 42.57% in PCSK-9 inhibitor group and 30.81%(P < 0.001) in statins-based group after six months. The proportion of LDL-C ≤ 1.0 mmol/L increased from 5.29% to 29.26% in PCSK-9 inhibitor group and 0.23% to 6.11% in statins-based group, and the proportion of LDL-C ≤ 1.4 mmol/L increased from 10.36% to 47.69% in PCSK-9 inhibitor group and 2.99% to 18.43% in statins-based group(P < 0.001 for both). CAG-repeat variant in the polymerase γ gene and male infertility in the Chinese population： a meta-analysis

Several studies have reported a relationship between the length of the CAG-repeat in the polymerase y （POLG） gene and male infertility. However, other studies have not reproduced this result. In our study, the POLG-CAG-repeat length was analyzed in 535 healthy individuals from six Chinese Han populations living in different provinces. The frequencies of IO-CAG alleles and genotypes were high （97.38 and 94.13%, respectively）, with no significant difference among the six Chinese Han populations. Furthermore, we determined the distribution of the POLG-CAG-repeat in 150 infertile men and 126 fertile men. Our study suggested that the distributions of POLG-CAG-repeat alleles and genotypes were not significantly different between infertile （95.67 and 92.67%, respectively） and fertile men （97.22 and 94.44%, respectively）. In a subsequent meta-analysis, combining our data with data from previous studies, a comparison of the CAG-repeat alleles in fertile versus infertile men showed no obvious risk for male infertility associated with any particular allele （pooled odds ratio （0R）=0.94; 95% confidence interval （CI）： 0.60-1.48）. The significance level was not attained with any of the following genetic models： homozygote comparison （not lO/not 10 versus 10110： OR= 1.34; 95% Ch 0.66-2.72）, heterozygote comparison （lO/not 10 versus 10/10： OR= 1.04; 95% Ch 0.78-1.38）, dominant model comparison （not lO/not 10＋ 101 not 10 versus 10110. OR= 1.08; 95% Ch 0.79-1.47） and recessive genetic comparison （not lO/not 10 versus lO/not 10＋ 10/10- OR= 1.31; 95% Ch 0.68-2.55）. In conclusion, there is no significant difference of the frequencies of POLG-CAG-repeat variants among six Chinese Han populations, and this polymorphism may not be associated with Chinese male infertility. On the basis of a meta-analysis, there is no obvious association between CAG-repeat variants of the POLG gene and male infertility.

Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in ***: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall ***: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 *** overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. We identify 108 M subdwarfs（sd Ms） out of more than two hundred thousand M type spectra from the second data release（DR2） of the LAMOST regular survey. This sample, among which 58 members are identified for the first time, includes 33 extreme subdwarfs（esd Ms） and 11 ultra subdwarfs（usd Ms）.The selection is based on the usual ratio of absorption depth of Ca H2, Ca H3 and TiO 5 band *** also emphasize the use of the Ca H1 band. We provide estimates of spectral subtype（SPT）, L′epine metallicity index ζ, effective temperature and [Fe/H]. Both ζ–[Fe/H] and SPT–Teff figures show reasonable consistency; compared to PHOENIX model spectra, average rounded values of [Fe/H] for sd Ms, esd Ms and usd Ms are respectively –0.5, –1 and –1.5. The photometric distances are estimated, indicating that most sources are located within 500 pc of the Sun and 350 pc of the Galactic disk. Velocities and 3D Galactic motions are also briefly discussed. Among the 108 subdwarfs, seven stars appear to be active with a significant Hα emission line. The source LAMOST J104521.52＋482823.3 is a white dwarf- M subdwarf binary, while LAMOST J123045.52＋410943.8, also active, exhibits carbon features in red.

Higher proportions of peripheral CD19^＋CD5^＋ B cells predict the effect of corticosteroid in patients with late-onset hemorrhagic cystitis after allogeneic hematopoietic stem cell transplantation

Background The cause of late-onset hemorrhagic cystitis （LOHC） after allogeneic hematopoietic stem cell transplantation （allo-HSCT） remains obscure. In clinical practice, some LOHC patients respond to *** aim of this study was to determine the immune pathogenesis of LOHC post *** With the diagnosis of LOHC, patients were given initial treatment consisting of fluid hydration, alkalization and forced diuresis, and empirical anti-viral therapy for 10-14 days or until a week after the virus became negative. The nonresponders were applied corticosteroid. Seven to ten days later, patients＇ response was evaluated. Along with treatment, CD19^＋ B lymphocyte subsets were measured at various study *** From October 2009 to March 2010, we found 28 cases of LOHC occurred in 25 patients who underwent allo-HSCT in our hospital. Except that three cases were not treated according to the protocol, the other 25 cases were divided into three groups： anti-virus responders （Group A, n=6）, corticosteroid responders （Group B1, n=16）,corticosteroid and anti-virus nonresponders （Group C, n=3） according to their clinical response. Percentages of CD19^＋CD5^＋ B lymphocytes were not significantly different among three groups at onset of LOCH. However, in Group B1after the first anti-virus phase, percentages of CD19^＋CD5^＋ lymphocytes significantly increased comparing with those at onset （P=0.022）, and then significantly decreased at PR （P=0.003） and CR （P=0.002） with corticosteroid treatment. But significant change was not observed in Groups A and *** The immune etiology seems to be involved in the development of LOHC and the proportion of CD19^＋CD5^＋lymphocytes may serve as a cellular biomarker to predict the response to corticosteroid in LOHC

We report an experimental investigation of the influence of surface charges on the emission polarization properties of single CdSe/CdS dot-in-rods(DRs),which is important for their polarization-based practical *** covering the single DRs with N-type semiconductor indium tin oxide(ITO)nanoparticles,the surface of single DRs is charged by ITO through interracial electron *** is confirmed by the experimental observations of the reduced photoluminescence intensities and lifetimes as well as the suppressing *** is found that the full width at half maximum of histogram of polarization degrees of the single DRs is broadened from 0.24(on glass)to 0.41(in ITO).In order to explain the exprimental results,the band-edge exciton fine structure of single DRs is calculated by taking into account the sample parameters,the emission polarization,and the surface *** calculation results show that the level ordering of the emitting states determines the polarization degrees tending to increase or decrease under the influence of surface *** surface electrons can induce an increase in the spacing between the emitting levels to change the populations and thus change the polarization *** addition,different numbers of surface electrons may randomly distribute on the long CdSe/CdS rods,leading to the heterogeneous influences on the single DRs causing the broadening of polarization degrees also.

The multi-physics instrument(MPI)is the first user cooperative instrument at the China Spallation Neutron Source(CSNS).It was designed to explore the structures of complex materials at multiple scales based on the neutron total scattering *** imposes the requirements for the detector,including a high detection efficiency to reduce the measurement time and a large solid angle coverage to cover a wide range of momentum *** satisfy these demands,a large-area array of 3He-filled linear position-sensitive detectors(LPSDs)was constructed,each with a diameter of 1 inch and pressure of 20 *** uses an orbicular layout of the detector and an eight-pack module design for the arrangement of 3He LPSDs,covering a range of scattering angles from 3°to 170°with a total detector area of approximately 7 *** detector works in air,which is separated from the vacuum environment to facilitate installation and *** characteristics of the MPI detector were investigated through Monte Carlo(MC)simulations using Geant4 and experimental *** results suggest that the detectors are highly efficient in the wavelength range of the MPI,and an efficiency over 25%is achievable for above 0.1 A neutrons.A minimal position resolution of 6.4 mm full width at half maximum(FWHM)along the tube length was achieved at a working voltage of 2200 V,and a deviation below 2 mm between the real and measured positions was attained in the beam *** detector module exhibited good consistency and an excellent counting rate capacity of up to 80 kHz,which satisfied the requirements of experiments with a high event *** of its operation over the past year have shown that the detector works steadily in sample experiments,which allows the MPI to serve the user program successfully.

Novel measurements of the neutron energy spectra of the 9Be(d,n)10B reaction with a thick beryllium target are performed using a fast neutron time-of-flight(TOF)spectrometer for the neutron emission angles θ=0°and 45°,and the incident deuteron energies are 250 and 300 keV,*** neutron contributions from the 9Be(d,n)10B reaction are distributed relatively independently for the ground state and the first,second,and third excited states of *** branching ratios of the 9Be(d,n)10B reaction for the different excited states of 10B are obtained for the neutron emission angles θ=0°and 45°,and the incident deuteron energies are 250 and 300 keV,*** branching ratio of the 9Be(d,n)10B reaction for the third excited state decreases with increase in the incident deuteron energy,and the branching ratios for the ground state and the second excited state increase with increase in the neutron emission angle.

Objective:Renal fibrosis is the most common manifestation of chronic kidney disease(CKD).Noting that existing treatments of renal fibrosis only slow disease progression but do not cure it,there is an urgent need to identify novel *** sulfide(H2S)is a newly discovered endogenous small gas signaling molecule exerting a wide range of biologic actions in our *** review illustrates recent experimental findings on the mechanisms underlying the therapeutic effects of H2S against renal fibrosis and highlights its potential in future clinical *** sources:Literature was collected from PubMed until February 2019,using the search terms including"Hydrogen sulfide,""Chronic kidney disease"MRenal interstitial fibrosis,"Kidney disease,""Inflammation factor,""Oxidative stress,""Epithelialto-mesenchymal transition,""H2S donor,""Hypertensive kidney dysfunction,""Myofibroblasts,""Vascular remodeling,""transforming growth factor(TGF)-beta/Smads signaling,"and"Sulfate potassium channels."Study selection:Literature was mainly derived from English articles or articles that could be obtained with English *** type was not *** were also identified from the bibliographies of identified articles and the authors5 ***:The experimental data confirmed that H2S is widely involved in various renal pathologies by suppressing inflammation and oxidative stress,inhibiting the activation of fibrosis-related cells and their cytokine expression,ameliorating vascular remodeling and high blood pressure,stimulating tubular cell regeneration,as well as reducing apoptosis,autophagy,and ***,H2S represents an alternative or additional therapeutic approach for renal ***:We postulate that H2S may delay the occurrence and progress of renal fibrosis,thus protecting renal *** experiments are required to explore the precise role of H2S in renal fibrosis and its application in clinical treatment.
