Curative effect analysis of spiral pedunculated bladder muscle flap in repairing long segment ureteral defects

In the past, the surgeons usually adopted ileal ureter plasty or ureter bladder flap plasty （Boari flap plasty） to restorelong-term ureteral mucosal avulsions and long or entire ureteral segment defects caused mostly by ureteroscope operations. But there are still certain difficulties in the restoration of long segment ureteral defects （〉20 cm） using traditional methods. In order to overcome traditional surgical approaches, we designed a new ureteroplasty operation using spiral pedunculated bladder muscle flap to restore long segment ureteral defects. METHODS Six patients who presented long segment ureteral defects caused in the course of ureteroscopic lithotripsy due to ureteropelvic junction stenosis and stones （length of defects： 21-25 cm, mean length： 22.5 cm）, were given general anesthesia, and made to lay in the horizontal position while indwelling triple lumen catheters. These patients had Gibson incision in the hypogastrium of the injured sides, and we could prolong the surgical incision up to the epigastrium or the flank abdomen moderately if necessary. We exposed the retroperitoneal space, transected the umbilical ligaments, peritoneal adhesions, spermaducts or the round ligaments to dissociate the bladder to the maximum extent possible. We could increase the mobility of the bladder by dissociating the contralateral superior vesical arteries along the anterior trunk of the internal iliac artery. We could also identify the stump of the distal ureter along the path between the ureter and bladder below the iliac vessels, and then we had to ligate or transfix the ureteral stump. We dissociated the ureteropelvic junction carefully, trimmed the stump to the inclined plane in order to facilitate the anastomose between the bladder muscle flap and the trimmed ureteral stump while locating and tracting it by the suture. long-lived lunar volcanism sustained by precession-driven core-mantle friction

Core-mantle friction induced by the precession of the Moon's spin axis is a strong heat source in the deep lunar mantle during the early phase of a satellite's evolution, but its influence on the long-term thermal evolution still remains poorly explored. Using a one-dimensional thermal evolution model, we show that core-mantle friction can sustain global-scale partial melting in the upper lunar mantle until ～3.1 Ga, thus accounting for the intense volcanic activity on the Moon before ～3.0 Ga. Besides, core-mantle friction tends to suppress the secular cooling of the lunar core and is unlikely to be an energy source for the long-lived lunar core dynamo. Our model also favours the transition of the Cassini state before the end of the lunar magma ocean phase(～4.2 Ga), which implies a decreasing lunar obliquity over time after the solidification of the lunar magma ocean. Such a trend of lunar obliquity evolution may allow volcanically released water to be buried in the lunar regolith of the polar regions. As a consequence, local water ice could be more abundant than previously thought when considering only its accumulation caused by solar wind and comet spreading.

Impact of Blood Clot in Oocyte-Corona-Cumulus Complex during in vitro Fertilization and Embryo Transfer

Objective To study the impact of blood clots in the oocyte-corona-cumulus complexes （OCCC） during in vitro fertilization and embryo transfer （IVF-ET）.Methods The OCCCs were harvested from the patients undergoing long protocol ovarian hyperstimulation. The OCCCs with blood clots removed or not, were randomly grouped into A or B. The OCCCs without blood clots were group C （the control）.Results The patient＇s age, infertility duration, the average GN consumption, the average days of superovulation and an average number of harvested oocytes showed no significant difference in the 3 groups. The fertilization rate and 2PN rate in group A were the highest, which were 85.4% and 71.1%, respectively, followed by group C, which were 77.5% and 64.9%, respectively. The lowest fertilization rate and 2PN rate were in group B, 75.8% and 62.2%, respectively. Those in group A were significantly higher than those in groups B and C （P〈0.01）, while there was no significant difference between group B and group C. The implantation rates and pregnancy rates showed no significant difference in the 3 groups after transplantation, even if group A got the highest rate among the 3 groups. Conclusion Removing the blood clots in OCCC can improve the outcome of IVF-ET without increasing the cost and complexity of the operation.

Facilitating prelithiation of silicon carbon anode by localized high-concentration electrolyte for high-rate and long-cycle lithium storage
