AIM:To investigate effects of perirenal space blocking(PSB)on gastrointestinal function in patients with severe acute pancreatitis(SAP).METHODS:Forty patients with SAP were randomly allocated to receive PSB or no PSB(NPSB).All the SAP patients received specialized medical therapy(SMT).Patients in the PSB group received PSB+SMT when hospitalized and after diagnosis,whereas patients in the NPSB group only received SMT.A modifed gastrointestinal failure(GIF)scoring system was used to assess the gastrointestinal function in SAP patients after *** severity(visual analog scale,0 to100)was monitored every 24 h for 72 ***:Modified GIF score decreased in both groups during the 10-d study *** median score decrease was initially significantly greater in the PSB group than in the NPSB group after PSB was *** the 72-h study period,pain intensity decreased in both *** median pain decrease was significantly greater in the PSB group than in the NPSB group at single time *** in the PSB group had significantly lower incidences of hospital mortality,multiple organ dysfunction syndrome,systemic inflammatory response syndrome,and pancreatic infection,and stayed in the intensive care unit for a shorter ***,no difference in terms of operation incidence was found between the two ***:PSB could ameliorate gastrointestinal dysfunction or failure during the early stage of ***,PSB administration could improve prognosis and decrease the mortality of SAP patients.

With global warming,glaciers in the high mountains of China are retreating ***,few data have been reported on whether greenhouse gases from these glaciers are released into the atmosphere or absorbed by glacial *** this study,we collected meltwater and ice samples from Laohugou Glacier No.12 in western China and measured CH_(4)and CO_(2)*** from the glacier terminus was continually sampled between 3 and 5 August 2020 to measure CH_(4)and CO_(2)*** results demonstrated that meltwater is a source of CH_(4)because the average saturations are over 100%.It could be con eluded that CH_(4)in the atmosphere can be released by glacial ***,the CO_(2)saturations are various,and CO_(2)fluxes exhibit positive(released CO_(2))or negative(absorbed CO_(2))values because the water and atmospheric conditions are *** importantly,the CH_(4)and CO_(2)concentrations were higher in meltwater samples from the glacier terminus than in samples from the surface ice(including an ice core)and a surface *** the meltwater effect from the upper part of the glacier cannot be excluded,we speculated that subglacial drainage systems with an anaerobic environment may represent the CH_(4)source,but it needs to be further investigated in the ***,high mountain glaciers are currently ignored in global carbon budgets,and the increased melting of glaciers with global warming may accelerate the absorption of much more CO_(2)and lead to the release of CH_(4).

BACKGROUND Serum protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ) is a promising biomarker for hepatocellular carcinoma(HCC) *** To identify the contributing factors related to the abnormal elevation of PIVKA-Ⅱ level and assess their potential influence on the performance of PIVKA-Ⅱ in detecting *** This study retrospectively enrolled in 784 chronic liver disease(CLD) patients and 267 HCC patients in Mengchao Hepatobiliary Hospital of Fujian Medical University from April 2016 to December 2019. Logistic regression and the area under the receiver operating characteristic curve(AUC) were used to evaluate the influencing factors and diagnostic performance of PIVKA-Ⅱ for HCC, *** Elevated PIVKA-Ⅱ levels were independently positively associated with alcohol-related liver disease, serum alkaline phosphatase(ALP), and total bilirubin(TBIL) for CLD patients and aspartate aminotransferase(AST) and tumor size for HCC patients(all P limit of normal(ULN), TBIL ≤ 1 × ULN, and AST ≤ 1 × ULN than in those with nonviral disease and abnormal ALP, TBIL, or AST(all P ge HCC. For patients with TBIL ≤ 1 × ULN, the AUC of PIVKA-Ⅱ was significantly higher compared to that in patients with TBIL > 1 × ULN(0.817 vs 0.669, P = 0.015), while the difference between ALP ≤ 1 × ULN and ALP > 1 × ULN was not statistically significant(0.783 vs 0.729, P = 0.398). These trends were then more prominently perceived in subgroups of patients with viral etiology and HBV *** Serum PIVKA-Ⅱ has better performance in detecting HCC at an early stage for CLD patients with normal serum TBIL.

BACKGROUND It is controversial whether transjugular intrahepatic portosystemic shunt(TIPS)placement can improve long-term *** To assess whether TIPS placement improves survival in patients with hepaticvenous-pressure-gradient(HVPG)≥16 mmHg,based on HVPG-related risk *** Consecutive variceal bleeding patients treated with endoscopic therapy+nonselectiveβ-blockers(NSBBs)or covered TIPS placement were retrospectively enrolled between January 2013 and December *** measurements were performed before *** primary outcome was transplant-free survival;secondary endpoints were rebleeding and overt hepatic ence-phalopathy(OHE).RESULTS A total of 184 patients were analyzed(mean age,55.27 years±13.86,107 males;102 in the EVL+NSBB group,82 in the covered TIPS group).Based on the HVPG guided risk stratification,70 patients had HVPG<16 mmHg,and 114 patients had HVPG≥16 *** median follow-up time of the cohort was 49.5 *** was no significant difference in transplant-free survival between the two treatment groups overall(hazard ratio[HR],0.61;95%confidence interval[CI]:0.35-1.05;P=0.07).In the high-HVPG tier,transplant-free survival was higher in the TIPS group(HR,0.44;95%CI:0.23-0.85;P=0.004).In the low-HVPG tier,transplantfree survival after the two treatments was similar(HR,0.86;95%CI:0.33-0.23;P=0.74).Covered TIPS placement decreased the rate of rebleeding independent of the HVPG tier(P<0.001).The difference in OHE between the two groups was not statistically significant(P=0.09;P=0.48).CONCLUSION TIPS placement can effectively improve transplant-free survival when the HVPG is greater than 16 mmHg.

The [α/Fe] ratios in stars are good tracers to probe the formation history of stellar populations and the chemical evolution of the Galaxy. The spectroscopic survey of LAMOST provides a good opportunity to determine [α/Fe] of millions of stars in the Galaxy. We present a method of measuring the [α/Fe]ratios from LAMOST spectra using the template-matching technique of the LSP3 pipeline. We use three test samples of stars selected from the ELODIE and MILES libraries, as well as the LEGUE survey to validate our method. Based on the test results, we conclude that our method is valid for measuring [α/Fe]from low-resolution spectra acquired by the LAMOST survey. Within the range of the stellar parameters Teff= [5000, 7500] K, log g = [1.0, 5.0] dex and [Fe/H]= [onsistent with values derived from high-resolution spectra,-1.5, ＋0.5] dex, our [α/Fe] measurements are c and the accuracy of our [α/Fe] measurements from LAMOST spectra is better than 0.1 dex with spectral signal-to-noise higher than 20.
