目的探讨瓣膜植入深度对单纯主动脉瓣反流(aortic regurgitation, AR)患者行经导管主动脉瓣置换术(transcatheter aortic valve replacement, TAVR)后早期心功能变化情况及围术期并发症的影响。方法选择2021年9月-2023年3月本院收治的TAVR治疗的主动脉瓣重度反流的患者30例作为研究对象,根据覆膜支架下缘距无冠瓣距离分为两组,瓣膜植入深度为0~6 mm患者有21例(Ⅰ组),瓣膜植入深度>6 mm患者有9例(Ⅱ组);分别测量俩组患者术前及术后2周超声心动图指标,同时追踪患者围术期并发症情况。通过对比研究,分析TAVR手术瓣膜植入深度对患者术后早期心功能及围术期并发症的影响。结果两组患者术前各项基线临床资料差异均无统计学意义(P>0.05)。与术前比较,两组患者术后2周左房舒张末期内径(LAD)明显减小、左室舒张末期内径(LVEDD)明显减小、室间隔厚度(IVST)明显变薄、左室射血分数(LVEF)明显增高,差异具均有统计学意义(P0.05)。Ⅰ组患者术后二尖瓣反流面积(MR area)明显减小,差异有统计学意义(P0.05)。两组患者术中TEE指标,左室流出道内径、主动脉瓣环前后径、升主动脉内径、左冠状动脉及右冠状动脉开口高度比较差异均无统计学意义(P>0.05);两组患者二尖瓣幕帘宽度比较,差异有统计学意义(P0.05)。结论单纯主动脉瓣反流患者TAVR术后早期心功能已具有明显改善,手术成功率较高,此类患者接受TAVR治疗安全有效。瓣膜植入深度对患者围术期并发症有影响,瓣膜植入过深会增加瓣周漏及传导障碍发生率,可能导致二尖瓣反流程度有加重趋势。

以北京六里屯垃圾填埋场为个案,通过描述统计、多元回归分析等统计方法,对填埋场周边民众的焦虑情绪、风险认知和邻避态度三者状况及其内在关联进行实证分析。研究发现:近3/4的居民属中度以上焦虑,近6成居民邻避态度强烈;焦虑情绪与邻避态度具有显著的正相关关系;在风险认知的4个因素——生活品质、环境污染、身体健康、补偿意愿中,补偿意愿与邻避态度存在显著的负相关关系,生活品质、环境污染、身体健康三者均与邻避态度呈显著的正相关关系。

为了在输出能量为100 kJ的激光装置集束平台上开展激光等离子体不稳定性(LPI)实验研究,建设了基于集束构型的散射光诊断系统。该诊断系统使用漫反射板作为主要拦光、反射、取样元件,利用成像方式将散射光分别成像至iCCD(intensifier Charge Coupled Device)相机等记录部件,采取取样测量方式得到散射光的空间分布、能量大小、光谱及时间波形等。在集束物理实验中,该系统获得了较完备的物理数据,与物理模拟计算程序的计算结果较为吻合,表明在当前条件下散射光的主要机制为子束机制,其作用过程主要集中于等离子体未排空的前期。

Objective: Large volume radiological text data have been accumulated since the incorporation of electronic health record(EHR) systems in clinical practice. We aimed to determine whether deep natural language processing algorithms could aid radiologists in improving thyroid cancer ***: Sonographic EHR data were obtained from the EHR database. Pathological reports were used as the gold standard for diagnosing thyroid cancer. We developed thyroid cancer diagnosis based on natural language processing(THCa Dx NLP) to interpret unstructured sonographic text reports for thyroid cancer diagnosis. We used the area under the receiver operating characteristic curve(AUROC) as the primary metric to measure the performance of the THCa Dx NLP. We compared the performance of thyroid ultrasound radiologists aided with THCa Dx NLP vs. those without THCa Dx NLP using 5 independent test ***: We obtained a total number of 788,129 sonographic radiological reports. The number of thyroid sonographic data points was 132,277, 18,400 of which were thyroid cancer patients. Among the 5 test sets, the numbers of patients per set were 439, 186, 82, 343, and 171. THCa Dx NLP achieved high performance in identifying thyroid cancer patients(the AUROC ranged from 0.857–0.932). Thyroid ultrasound radiologists aided with THCa Dx NLP achieved significantly higher performances than those without THCa Dx NLP in terms of accuracy(93.8% vs. 87.2%; one-sided t-test, adjusted P = 0.003), precision(92.5% vs. 86.0%; P = 0.018), and F1 metric(94.2% vs. 86.4%; P = 0.007).Conclusions: THCa Dx NLP achieved a high AUROC for the identification of thyroid cancer, and improved the accuracy, sensitivity, and precision of thyroid ultrasound radiologists. This warrants further investigation of THCa Dx NLP in prospective clinical trials.

采用原位聚合与水解缩聚法,以四乙氧基硅烷(TEOS)、环氧树脂(E51)、苯乙烯(St)等为主要原料,合成了一种二氧化硅多孔壳微胶囊(Porous silica shell microcapsules,PSSM)。分别采用SEM、FTIR、TGA对PSSM外观形貌、化学组分、核壳比进行表征。通过对掺加PSSM的砂浆试块进行80%抗压强度荷载预压、养护(浸水或干湿循环养护)处理后,运用交流阻抗法与压汞法研究了PSSM对硬化砂浆抗渗性与孔结构的影响规律。结果表明:制备的PSSM粒径约为10～100μm,其含有聚苯乙烯网络支架、环氧树脂和聚硅氧烷囊芯,支架聚合物和多孔壳,核壳质量比为1.54。与未预压-养护处理的试块相比,经预压-养护处理后的空白试块的连通孔溶液电阻RCH和扩散阻抗系数σ均降低,孔隙率升高,表明预压使试块内形成微裂纹缺陷,经养护仍未愈合,因此试块抗渗性降低;而对于掺加8%PSSM的试块,经预压-养护处理后其RCH和σ均增加,孔隙率降低,表明试块抗渗性提高。这是由于PSSM的破壳-固化作用以及长期浸水或干湿循环养护,导致试块中PSSM发生了渗出-固化作用,封堵愈合了试块内的微裂隙,抗渗性得到提高。

Identifying sources of phytopathogen inoculum and determining their contributions to disease outbreaks are essential for predicting disease development and establishing control *** striiformis ***(Pst),the causal agent of wheat stripe rust,is an airborne fungal pathogen with rapid virulence variation that threatens wheat production through its long-distance *** of wide variation in geographic features,climatic conditions,and wheat production systems,Pst sources and related dispersal routes in China are largely *** the present study,we performed genomic analyses of 154 Pst isolates from all major wheat-growing regions in China to determine Pst population structure and *** trajectory tracking,historical migration studies,genetic introgression analyses,and field surveys,we investigated Pst sources and their contributions to wheat stripe rust *** identified Longnan,the Himalayan region,and the Guizhou Plateau,which contain the highest population genetic diversities,as the Pst sources in *** from Longnan disseminates mainly to eastern Liupan Mountain,the Sichuan Basin,and eastern Qinghai;that from the Himalayan region spreads mainly to the Sichuan Basin and eastern Qinghai;and that from the Guizhou Plateau migrates mainly to the Sichuan Basin and the Central *** findings improve our current understanding of wheat stripe rust epidemics in China and emphasize the need for managing stripe rust on a national scale.
