BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

Lignin is a typical biological macromolecule with a three-dimensional network structure and abundant functional groups. It has excellent ionic complexation ability and amphiphilic molecular structure *** this study, the carbon steel anticorrosion performance of sodium lignosulfonate(SLS) in an acid solution was evaluated using the weight loss method, electrochemical measurements, scanning vibration electrode technique(SVET), and surface characterization methods. SLS exhibited excellent corrosion inhibition efficiency for Q235carbon steel in 1 mol·L^(-1) HCl, reaching a maximum value of 98%. A low SLS concentration of 20 mg·L^(-1) resulted in the maximum corrosion inhibition efficiency, which remained nearly constant at higher SLS *** SVET test demonstrated that the formation of an SLS adsorption film can impede corrosion. This study confirms the significance of the application of green biomass resources in the field of metal corrosion protection and green functional materials.

Chemical modification of barite for improving the performance of weighting materials for water-based drilling fluids

With increasing drilling depth and large dosage of weighting materials,drilling fluids with high solid content are characterized by poor stability,high viscosity,large water loss,and thick mud cake,easier leading to reservoir damage and wellbore *** this paper,micronized barite(MB)was modified(mMB)by grafting with hydrophilic polymer onto the surface through the free radical polymerization to displace conventional API barite *** suspension stability of water-based drilling fluids(WBDFs)weighted with API barite:mMB=2:1 in 600 g was significantly enhanced compared with that with API barite/WBDFs,exhibiting the static sag factor within 0.54 and the whole stability index of *** viscosity and yield point reached the minimum,with a reduction of more than 40%compared with API barite only at the same *** multi-stage filling and dense accumulation of weighting materials and clays,filtration loss was decreased,mud cake quality was improved,and simultaneously it had great reservoir protection performance,and the permeability recovery rate reached 87%.In addition,it also effectively improved the lubricity of *** sticking coefficient of mud cake was reduced by 53.4%,and the friction coefficient was ***,mMB can serve as a versatile additive to control the density,rheology,filtration,and stability of WBDFs weighted with API barite,thus regulating comprehensive performance and achieving reservoir protection *** work opened up a new path for the productive drilling of extremely deep and intricate wells by providing an efficient method for managing the performance of high-density WBDFs.

Independent risk factors for disease recurrence after surgery in patients with hepatitis B virus-related hepatocellular carcinoma≤3 cm in diameter

Background:Post-operative recurrence rates are high for hepatitis B virus(HBV)-related hepatocellular carcinoma(HCC).This study aimed to explore the factors associated with post-operative 1-year recurrence rate in patients with HBV-related HCC who had a single small primary tumor(3 cmin diameter).Methods:This was a retrospective study of 203(training cohort)and 64(validation cohort)patients newly diagnosed with HBV-related HCC who had a single small primary *** first year of post-operative follow-up was *** potentially associated with HCC recurrence were identified using Cox regression analyses.A model was constructed based on the factors identified and the prognostic value of the model was evaluated using receiver operating characteristic(ROC)curve analysis and calculation of the area under the ROC curve(AUC).Results:A history of alcoholismand serum levels of a-fetoprotein,total protein and c-glutamyl transpeptidase(GGT)were independently associated with 1-year recurrence rate after surgery.A predictive model based on these four factors had an AUC of 0.711(95%confidence interval,0.643–0.772)in the training cohort and 0.727(95%confidence interval,0.601–0.831)in the validation *** 1-year recurrence rate was significantly lower in the low-risk group than in the high-risk group in both the training cohort(17.0%vs.49.5%,P<0.001)and the validation cohort(43.2%vs.74.1%,P=0.031).Conclusion:A history of alcoholism and serum levels of a-fetoprotein,total protein and c-glutamyl transpeptidase were independently associated with post-operative 1-year recurrence rate in patients with HBV-related HCC who had a single small primary tumor(3 cmin diameter).

Development and Validation of a Prognostic Model for One-year Survival of Cirrhosis Patients with First-ever Spontaneous Bacterial Peritonitis

Background and Aims: Spontaneous bacterial peritonitis(SBP) is one of the leading causes of death in patients withliver cirrhosis. We aimed to establish a prognostic model toevaluate the 1-year survival of cirrhosis patients after thefirst episode of SBP. Methods: A prognostic model was developedbased on a retrospective derivation cohort of 309cirrhosis patients with first-ever SBP and was validated in aseparate validation cohort of 141 patients. We used Uno’sconcordance, calibration curve, and decision curve (DCA)analysis to evaluate the discrimination, calibration, and clinicalnet benefit of the model. Results: A total of 59 (19.1%)patients in the derivation cohort and 42 (29.8%) patientsin the validation cohort died over the course of 1 year. Aprognostic model in nomogram form was developed withpredictors including age [hazard ratio (HR): 1.25;95% confidenceinterval (CI): 0.92–1.71], total serum bilirubin (HR:1.66;95% CI: 1.28–2.14), serum sodium (HR: 0.94;95%CI: 0.90–0.98), history of hypertension (HR: 2.52;95% CI:1.44–4.41) and hepatic encephalopathy (HR: 2.06;95%CI: 1.13–3.73). The nomogram had a higher concordance(0.79) compared with the model end-stage liver disease(0.67) or Child-Turcotte-Pugh (0.71) score. The nomogramalso showed acceptable calibration (calibration slope, 1.12;Bier score, 0.15±0.21) and optimal clinical net benefit in thevalidation cohort. Conclusions: This prediction model developedbased on characteristics of first-ever SBP patientsmay benefit the prediction of patients’ 1-year survival.

Design and high-power testing of offline conditioning cavity for CiADS RFQ high-power coupler

To validate the design rationality of the power coupler for the RFQ cavity and minimize cavity contamination,we designed a low-loss offline conditioning cavity and conducted high-power *** offline cavity features two coupling ports and two tuners,operating at a frequency of 162.5 MHz with a tuning range of 3.2 *** the installation angle of the coupling ring and the insertion depth of the tuner helps minimize cavity *** performed electromagnetic structural and multiphysics simulations,revealing a minimal theoretical power loss of 4.3%.However,when the cavity frequency varied by110 kHz,theoretical power losses increased to10%,necessitating constant tuner adjustments during *** simulations indicated that increased cavity temperature did not affect frequency *** completion of the offline high-power conditioning platform,we measured the transmission performance,revealing a power loss of 6.3%,exceeding the theoretical *** utilized efficient automatic range scanning and standing wave resonant *** fully condition the power coupler,a 15°phase difference between two standing wave points in the condition-ing system was ***,the maximum continuous wave power surpassed 20 kW,exceeding the expected target.

The mechanical behavior of polymer networks is intrinsically correlated with the local chain topology and chain *** this study,we delve into this relationship through the lens of coarse-grained molecular dynamics(CG-MD)*** aim is to illuminate the intricate interplay between local topology and stress distribution within polymer monomers,cross-linkers,and various components with distinct cross-link connections,thereby elucidating their collective impact on the mechanical properties of polymer *** mainly focus on how specific local structures contribute to the overall mechanical response of the *** particular,we employ local stress analysis to unravel the mechanics of these *** findings reveal the diverse responses of individual components,such as junctions,strands,cross-linkers between junctions,and dangling chain ends,when subjected to ***,we observe that these components exhibit varying degrees of deformation tolerance,underscoring the significance of their roles in determining the mechanical characteristics of the *** investigations highlight junctions as primary contributors to stress accumulation,and particles with higher local stress showing a stronger correlation between stress and Voronoi ***,our results indicate that both strands and cross-linkers between junctions exhibit heightened stress levels as strand lengths *** study enhances our understanding of the multifaceted factors governing the mechanical attributes of cross-linked polymer systems at the microstructural level.
