Ultrasound-guided fascia iliaca compartment block combined with general anesthesia for amputation in an acute myocardial infarction patient after percutaneous coronary intervention: A case report

BACKGROUND Fascia iliaca compartment block is a technique that blocks three nerves,similar to a 3-in-1 nerve block.This block provides analgesia for patients undergoing lower limb surgery,and is a simple technique that is easy to implement.Here,we report a case of fascia iliaca compartment block in a patient with myocardial infarction who underwent emergency middle thigh amputation.CASE SUMMARY A 78-year-old female patient weighing 38 kg with gangrene and occlusive peripheral atherosclerosis of the right leg underwent an emergency middle thigh amputation.The patient had a history of hypertension,coronary heart disease,cerebral infarction,anterior wall myocardial infarction,and had recently undergone percutaneous coronary intervention consisting of coronary angiography and right coronary artery stent implantation.Considering the patient's condition,an ultrasound-guided fascia iliaca compartment block combined with general anesthesia was implemented for amputation.The fascia iliaca compartment block provided analgesia for the operation,and reduced the dosage of general anesthetics.It also alleviated adverse cardiovascular effects caused by pain stress,and ensured the safety of the patient during the perioperative period.This block also provided postoperative analgesia.The patient had a good prognosis,and was subsequently discharged from hospital.CONCLUSION Fascia iliaca compartment block provides surgical analgesia.It also alleviates adverse cardiovascular effects,and ensures patient safety during the perioperative period.

Projected changes in Köppen-Trewartha climate zones under 1.5-4℃global warming targets over mid-high latitudes of Northern Asia using an ensemble of RegCM4 simulations

Mid-high latitude Northern Asia is one of the most vulnerable and sensitive areas to global warming,but relatively less studied previously.We used an ensemble of a regional climate model(RegCM4)projections to assess future changes in surface air temperature,precipitation and Köppen-Trewartha(K-T)climate types in Northern Asia under the 1.5-4℃global warming targets.RegCM4 is driven by five CMIP5 global models over an East Asia domain at a grid spacing of 25 km.Validation of the present day(1986-2005)simulations shows that the ensembles of RegCM4(ensR)and driving GCMs(ensG)reproduce the major characters of the observed temperature,precipitation and K-T climate zones reasonably well.Greater and more realistic spatial detail is found in RegCM4 compared to the driving GCMs.A general warming and overall increases in precipitation are projected over the region,with these changes being more pronounced at higher warming levels.The projected warming by ensR shows different spatial patterns,and is in general lower,compared to ensG in most months of the year,while the percentage increases of precipitation are maximum during the cold months.The future changes in K-T climate zones are characterized by a substantial expansion of Dc(temperature oceanic)and retreat of Ec(sub-arctic continental)over the region,reaching∼20%under the 4℃warming level.The most notable change in climate types in ensR is found over Japan(∼60%),followed by Southern Siberia,Mongolia,and the Korean Peninsula(∼40%).The largest change in the K-T climate types is found when increasing from 2 to 3℃.The results will help to better assess the impacts of climate change and in implementation of appropriate adaptation measures over the region.

Observation of gapless corner modes of photonic crystal slabs in synthetic translation dimensions

Second-order topological photonic crystals support localized corner modes that deviate from the conventional bulk-edge correspondence.However,the frequency shift of corner modes spanning the photonic band gap has not been experimentally reported.Here,we observe the gapless corner modes of photonic crystal slabs within a parameter space by considering translation as an additional synthetic dimension.These corner modes,protected by topological pumping in synthetic translation dimensions,are found to exist independently of the specific corner configuration.The gapless corner modes are experimentally imaged via the near-field scanning measurement and validated numerically by full-wave simulations.We propose a topological rainbow with gradient translation,demonstrating the ability to extract and separate specific frequency components of light into different spatial locations.Our work contributes to the advancement of topological photonics and provides valuable insights into the exploration of gapless corner modes in synthetic dimensions.

High-throughput screening system of citrus bacterial cankerassociated transcription factors and its application to the regulation of citrus canker resistance

One of the main diseases that adversely impacts the global citrus industry is citrus bacterial canker(CBC),caused by the bacteria Xanthomonas citri subsp.citri(Xcc).Response to CBC is a complex process,with both proteinDNA as well as protein–protein interactions for the regulatory network.To detect such interactions in CBC resistant regulation,a citrus high-throughput screening system with 203 CBC-inducible transcription factors(TFs),were developed.Screening the upstream regulators of target by yeast-one hybrid(Y1H)methods was also performed.A regulatory module of CBC resistance was identified based on this system.One TF(CsDOF5.8)was explored due to its interactions with the 1-kb promoter fragment of CsPrx25,a resistant gene of CBC involved in reactive oxygen species(ROS)homeostasis regulation.Electrophoretic mobility shift assay(EMSA),dual-LUC assays,as well as transient overexpression of CsDOF5.8,further validated the interactions and transcriptional regulation.The CsDOF5.8–CsPrx25 promoter interaction revealed a complex pathway that governs the regulation of CBC resistance via H2O2homeostasis.The high-throughput Y1H/Y2H screening system could be an efficient tool for studying regulatory pathways or network of CBC resistance regulation.In addition,it could highlight the potential of these candidate genes as targets for efforts to breed CBC-resistant citrus varieties.

Prediction models of burst strength degradation for casing with considerations of both wear and corrosion

Casing wear and casing corrosion are serious problems affecting casing integrity failure in deep and ultra-deep wells.This paper aims to predict the casing burst strength with considerations of both wear and corrosion.Firstly,the crescent wear shape is simplified into three categories according to common mathematical models.Then,based on the mechano-electrochemical(M-E)interaction,the prediction model of corrosion depth is built with worn depth as the initial condition,and the prediction models of burst strength of the worn casing and corroded casing are obtained.Secondly,the accuracy of different prediction models is validated by numerical simulation,and the main influence factors on casing strength are obtained.At last,the theoretical models are applied to an ultra-deep well in Northwest China,and the dangerous well sections caused by wear and corrosion are predicted,and the corrosion rate threshold to ensure the safety of casing is obtained.The results show that the existence of wear defects results in a stress concentration and enhanced M-E interaction on corrosion depth growth.The accuracy of different mathematical models is different:the slot ring model is most accurate for predicting corrosion depth,and the eccentric model is most accurate for predicting the burst strength of corroded casing.The burst strength of the casing will be overestimated by more than one-third if the M-E interaction is neglected,so the coupling effect of wear and corrosion should be sufficiently considered in casing integrity evaluation.

Scattering of Water Waves by Dual Symmetric Inclined Floating Porous Barriers Using the DBEM

The scattering of normally incident water waves by two surface-piercing inclined perforated barriers in water with a uniform finite depth is investigated within the framework of linear water wave theory.Considering that thin barriers are zero-thickness,a novel numerical method involving the the coupling of the dual boundary element method(DBEM)with damping layers is applied.In order to effectively damp out the reflected waves,two damping layers,instead of pseudoboundaries are implemented near the two side boundaries of the computational domain.Thus,the modified linearized free surface boundary conditions are formulated and used for solving both the ordinary boundary integral equation as well as the hypersingular boundary integral equation for degenerate boundaries.The newly developed numerical method is validated against analytical methods using the matched eigenfunction expansion method for the special case of two vertical barriers or the inclined angle to the vertical being zero.The influence of the length of the two damping layers has been discussed.Moreover,these findings are also validated against previous results for several cases.After validation,the numerical results for the reflection coefficient,transmission coefficient and dissipation coefficient are obtained by varying the inclination angle and porosity-effect parameter.The effects of both the inclination angle and the porosity on the amplitudes of wave forces acting on both the front and rear barriers are also investigated.It is found that the effect of the inclination angle mainly shifts the location of the extremal values of the reflection and the transmission coefficients.Additionally,a moderate value of the porosity-parameter is quite effective at dissipating wave energy and mitigating the wave loads on dual barriers.

An Analysis of the Fragmentation of Observing Time At the Muztagh-Ata Site

Cloud cover plays a pivotal role in assessing observational conditions for astronomical site-testing.Except for the fraction of observing time,its fragmentation also wields a significant influence on the quality of nighttime sky clarity.In this article,we introduce the functionΓ∈[0,1],designed to comprehensively capture both the fraction of available observing time and its continuity.Leveraging in situ measurement data gathered at the Muztagh-Ata site between 2017 and 2021,we showcase the effectiveness of our approach.The statistical result illustrates that the Muztagh-Ata site affords approximately 122 nights that were absolutely clear and 205 very good nights annually,corresponding toΓ≥0.9 andΓ≥0.36 respectively.

Rapid report of source parameters of 2023 M6.2 Jishishan,Gansu earthquake sequence

The M6.2 earthquake in Jishishan,Gansu Province,on December 18,2023,caused extraordinary earthquake disasters.It was located in the northern part of the north−south seismic zone,which is a key area for earthquake monitoring in China.The newly built dense strong motion stations in this area provide unprecedented conditions for high-precision earthquake relocation,especially the earthquake focal depth.This paper uses the newly built strong motion and traditional broadband seismic networks to relocate the source locations of the M3.0 and above aftershocks and to invert their focal mechanisms.The horizontal error of earthquake location is estimated to be 0.5−1 km,and the vertical error is 1−2 km.The focal depth range of aftershocks is 9.6−14.6 km,distributed in a 12-km-long strip with SSE direction.Aftershocks in the south are more concentrated horizontally and vertically,while aftershocks in the north are more scattered.The focal mechanisms of the main shock and aftershocks are relatively consistent,and the P-axis orientation is consistent with the regional strain direction.There is a seismic blank area of M3.0 and above,about 3−5 km between the main shock and aftershocks.It is suggested that the energy released by the main shock rupture is concentrated in this area.Based on the earthquake location and focal mechanism of the main shock,it is inferred that the Northern Lajishan fault zone is the seismogenic structure of the main shock,and the main shock did not occur on the main fault,but on a secondary fault.The initial rupture depth and centroid depth of the main shock were 12.8 and 14.0 km,respectively.The source rupture depth may not be the main reason for the severe earthquake disaster.

Monte Carlo Simulation-Based Robust Workflow Scheduling for Spot Instances in Cloud Environments

When deploying workflows in cloud environments,the use of Spot Instances(SIs)is intriguing as they are much cheaper than on-demand ones.However,Sls are volatile and may be revoked at any time,which results in a more challenging scheduling problem involving execution interruption and hence hinders the successful handling of conventional cloud workflow scheduling techniques.Although some scheduling methods for Sls have been proposed,most of them are no more applicable to the latest Sls,as they have evolved by eliminating bidding and simplifying the pricing model.This study focuses on how to minimize the execution cost with a deadline constraint when deploying a workflow on volatile Sls in cloud environments.Based on Monte Carlo simulation and list scheduling,a stochastic scheduling method called MCLS is devised to optimize a utility function introduced for this problem.With the Monte Carlo simulation framework,MCLS employs sampled task execution time to build solutions via deadline distribution and list scheduling,and then returns the most robust solution from all the candidates with a specific evaluation mechanism and selection criteria.Experimental results show that the performance of MCLS is more competitive comparedwithtraditionalalgorithms.

Impact of scandium and terbium on the mechanical properties,corrosion behavior,and biocompatibility of biodegradable Mg-Zn-Zr-Mn alloys

Magnesium(Mg)-based bone implants degrade rapidly in the physiological environment of the human body which affects their structural integrity and biocompatibility before adequate bone repair.Rare earth elements(REEs)have demonstrated their effectiveness in tailoring the corrosion and mechanical behavior of Mg alloys.This study methodically investigated the impacts of scandium(Sc)and terbium(Tb)in tailoring the corrosion resistance,mechanical properties,and biocompatibility of Mg–0.5Zn–0.35Zr–0.15Mn(MZZM)alloys fabricated via casting and hot extrusion.Results indicate that addition of Sc and Tb improved the strength of MZZM alloys via grain size reduction and solid solution strengthening mechanisms.The extruded MZZM–(1–2)Sc–(1–2)Tb(wt.%)alloys exhibit compressive strengths within the range of 336–405 MPa,surpassing the minimum required strength of 200 MPa for bone implants by a significant margin.Potentiodynamic polarization tests revealed low corrosion rates of as–cast MZZM(0.25 mm/y),MZZM–2Tb(0.45 mm/y),MZZM–1Sc–1Tb(0.18 mm/y),and MZZM–1Sc–2Tb(0.64 mm/y),and extruded MZZM(0.17 mm/y),MZZM–1Sc(0.15 mm/y),MZZM-2Sc(0.45 mm/y),MZZM-1Tb(0.17 mm/y),MZZM-2Tb(0.10 mm/y),MZZM–1Sc-1Tb(0.14 mm/y),MZZM-1Sc-2Tb(0.40 mm/y),and MZZM–2Sc–2Tb(0.51 mm/y)alloys,which were found lower compared to corrosion rate of high-purity Mg(~1.0 mm/y)reported in the literature.Furthermore,addition of Sc,or Tb,or Sc and Tb to MZZM alloys did not adversely affect the viability of SaOS2 cells,but enhanced their initial cell attachment,proliferation,and spreading shown via polygonal shapes and filipodia.This study emphasizes the benefits of incorporating Sc and Tb elements in MZZM alloys,as they effectively enhance corrosion resistance,mechanical properties,and biocompatibility simultaneously.
