Impact of COVID-19-related lifestyle changes on diabetic macular edema

AIM:To assess diabetic macular edema(DME)progression during the early phases of the COVID-19 pandemic,when severe societal restrictions raised the concern of possible deterioration of health in patients with systemic conditions,particularly those requiring frequent office ***:This is a multicenter retrospective chart review of 370 patients(724 eyes)with an established diagnosis of DME seen on 3 separate visits between January 2019 and July *** 1 was January 2019 to February 2020(considered pre-COVID-19),period 2 was March 2020 to December 2020(considered the height of the pandemic;highest level of pandemic-related clinical and societal regulations)and period 3 was January 2021 to July 2021(re-adjustment to the new“pandemic norms”).Main outcome measures included visual acuity,body mass index.BMI),blood pressure(BP),hemoglobin A1c(HbA1c),macular thickness,patient adherence to scheduled ophthalmology visits,and DME treatment(s)received at each *** facilitate measurement of macular thickness,each macula was divided into 9 Early Treatment Diabetic Retinopathy Study(ETDRS)-defined macular sectors as measured by OCT ***:There was no change of BMI,systolic BP,and diastolic BP between any of the time ***1c showed a very small increase from period 1(7.6%)to period 2(7.8%,P=0.015)and decreased back to 7.6%at period 3(P=0.12).Macular thickness decreased for 100%of macular *** central macular thickness decreased across all 3 periods from 329.5 to 316.6μm(P=0.0045).After analysis of multiple variables including HbA1c,BMI,adherence to scheduled appointments,different clinic centers,and treatment interventions,there was no easily identifiable subgroup of patients that ex.erienced the increase in ***:DME doesn’t worsen during the COVID-19 pandemic,instead sustaining a very small but statistically significant *** identifying a mechanism behind our findings is beyond the scope of this study,potential ex.lanat

Efficacy and safety of dacomitinib as first-line treatment for advanced non-small cell lung cancer patients with epidermal growth factor receptor 21l858R mutation:A multicenter, case-series study in China

Objective: To provide real-world evidence for the application of first-line dacomitinib treatment for epidermal growth factor receptor(EGFR) 21l858R mutant non-small cell lung cancer(NSClC) patients in China and to ex.lore the factors influencing the efficacy and ***: A longitudinal, consecutive case-series, multicenter study with mix.d prospective and retrospective data was conducted. The primary endpoint was progression-free survival(PFS), and the secondary endpoints included duration of treatment(DOT), overall survival(OS), objective response rate(ORR), disease control rate(DCR) and ***: A total of 155 EGFR 21l858R mutant patients treated with first-line dacomitinib were included. The median follow-up time for these patients was 20.4 months. Among 134 patients with evaluable lesions, the ORR was 70.9% and the DCR was 96.3%. The median PFS was 16.3 [95% confidence interval(95% CI), 13.7-18.9]months. Multivariate Cox.regression analysis suggested that the baseline brain metastasis(BM) status [with *** BM: hazard ratio(HR), 1.331; 95% CI, 0.720-2.458; P=0.361] and initial doses(45 mg vs. 30 mg: HR,0.837; 95% CI, 0.427-1.641; P=0.604) did not significantly affect the median PFS. The median DOT was 21.0(95% CI, 17.5-24.6) months and the median OS was not reached. Genetic tests were performed in 64 patients after progression, among whom 29(45.3%) patients developed the EGFR 20T790M mutation. In addition, among the 46patients who discontinued dacomitinib treatment after progression, 31(67.4%) patients received subsequent thirdgeneration EGFR-tyrosine kinase inhibitors. The most common grade 3-4 adverse events were rash(10.4%),diarrhea(9.1%), stomatitis(7.1%) and paronychia(4.5%). The incidence of grade 3-4 rash was significantly higher in the 45 mg group than that in the 30 mg group(21.9% vs. 7.5%, P=0.042).Conclusions: First-line dacomitinib treatment demonstrated promising efficacy and tolerable adverse events among EGFR 21l858R mutant NSC

Circulating tumor cells(CTCs)are important biomarkers in the development and progression of lung cancer because they can reach other organs through the blood circulation and form distant metastases,ex.cerbating lung cancer *** presence of CTCs is also the main reason for the failure of nanomedicine-based lung cancer ***,magnetic MoSe_(2) nanosheets loaded with programmed death-ligand 1(PD-l1),named PD-l1-MFP NS,were employed here to precisely capture lung cancer CTCs in the blood circulation through the tumor-targeting effect of PD-l1 killing CTCs with highly effective photothermal therapy(PTT).In addition,by increasing the ex.ression of cytomegalovirus Ul16-binding protein(UlBP)ligands on tumor cells,the PD-l1-MFP NS further activated natural killer(NK)cells and triggered NK cell-induced cancer immunotherapy,thereby enhancing the overall tumor-killing *** summary,this material designed to capture CTCs provides a substantial advancement for personalized PTT-triggered immunotherapy and has great clinical translational potential.

Following the recent report by Dasenbrock-Gammon et al.[Nature 615,244–250(2023)]of near-ambient superconductivity in nitrogendoped lutetium trihydride(luH_(3-δ)N_(ε)),significant debate has emerged surrounding the composition and interpretation of the observed sharp resistance ***,we meticulously revisit these claims through comprehensive characterization and *** definitively identify the reported material as lutetium dihydride(luH_(2)),resolving the ambiguity surrounding its *** similar conditions(270–295 K and 1–2 GPa),we replicate the reported sharp decrease in electrical resistance with a 30%success rate,aligning with the observations by Dasenbrock-Gammon et ***,our ex.ensive investigations reveal this phenomenon to be a novel pressure-induced metal-to-metal transition intrinsic to luH_(2),distinct from ***,nitrogen doping ex.rts minimal impact on this *** work not only elucidates the fundamental properties of luH_(2)andluH_(3),but also critically challenges the notion of superconductivity in these lutetium hydride *** findings pave the way for future research on lutetium hydride systems,while emphasizing the crucial importance of rigorous verification in claims of ambient-temperature superconductivity.

SO_(2)-Insertion induced enantioselective ox.sulfonylation to access β-chiral sulfones with quaternary carbon stereocenters

β-Chiral sulfones are structural motifs widely found in natural products and bioactive molecules, while also serving as important intermediates for the synthesis of valuable chiral scaffolds. In contrast to the rapid growth of sulfur diox.de insertion chemistry over the last decade, enantioselective catalytic variants for accessing β-chiral sulfones, especially those bearing a quaternary carbon stereocenter, remain rare. Herein, we report an enantioselective copper/bisox.zoline catalyzed ox.sulfonylation of alkenes to yield isox.zolinyl-containing β-chiral sulfones bearing quaternary carbon stereocenters with moderate to ex.ellent yields and up to 96:4 er. The advantage and irreplaceability of this sulfur diox.de insertion catalytic system have been demonstrated by achieving transformations clearly unavailable with the previously reported sulfonyl chloride system. Additionally,the recoverability of the new-developed bisox.zoline ligand l9 and its reusability without any erosion of the catalytic activity and enantioselectivity further demonstrate the usefulness of the protocol.

A Semi-Mechanistic Model for Predicting the Service life of Composite Coatings on VW63Z Magnesium Alloy

The application of Mg alloys is always accompanied by various coating technology, but a reliable model predicting the service life of coatings on Mg alloys is lacking but urgent. In this work, a semi-mechanistic model was proposed to predict the service life of plasma electrolytic ox.dation (PEO) coating/electrophoretic coatings on a VW63Z Mg alloy;the model was decomposed into three parts: a first part depicting the degradation time of organic coating (l_(1)) and the diffusion time of electrolyte in the inorganic coating (l_(2)), respectively;a second part interpreting the breakdown of coatings due to the corrosion process (l_(3));a final part establishing an algorithm converting the accelerated tests into the real service environment (α);the effect of structural stress and dissimilar metal joints on the service life of coatings was also considered. Based on the ongoing accelerated ex.eriments, the semi-mechanistic model could be able to predict the service life of both PEO coatings and composite coatings on VW63Z Mg alloy with a satisfiable precision.
