How are those “lost to follow-up” patients really doing? A compliance comparison in arthroplasty patients

AIM: To determine whether there is a functional difference between patients who actively follow-up in the office(OFU) and those who are non-compliant with office follow-up visits(NFU). METHODS: We reviewed a consecutive group of 588patients, who had undergone total joint arthroplasty(TJA), for compliance and functional outcomes at one to two years post-operatively. All patients were given verbal instructions by the primary surgeon to return at one year for routine follow-up visits. Patients that were compliant with the instructions at one year were placed in the OFU cohort, while those who were non-compliant were placed in the NFU cohort. Survey mailings and telephone interviews were utilized to obtain complete follow-up for the cohort. A χ2 test and an unpaired t test were used for comparison of baseline characteristics. Analysis of covariance was used to compare the mean clinical outcomes after controlling for confounding ***: Complete follow-up data was collected on 554 of the 588 total patients(93%), with 75.5% of patients assigned to the OFU cohort and 24.5% assigned to the NFU cohort. We found significant differences between the cohorts with the OFU group having a higher mean age(P = 0.026) and a greater proportion of females(P = 0.041). No significant differences were found in either the SF12 or WOMAC scores at baseline or at 12 mo ***: Patients who are compliant to routine follow-up visits at one to two years post-operation do not experience better patient reported outcomes than those that are non-compliant. Additionally, after TJA, older women are more likely to be compliant in following surgeon instructions with regard to follow-up office care.

The potential benefits of generating and using transgenic cattle range from improvements in agriculture to the production of large quantities of pharmaceutically relevant proteins. Previous studies have attempted to produce transgenic cattle and other livestock by pronuclear injection and somatic cell nuclear transfer, but these approaches have been largely ineffective; however, a third approach, lentivirus-mediated transgenesis, has successfully produced transgenic livestock. In this study, we generated transgenic （TG） Korean native cattle using perivitelline space injection of viral vectors, which expressed enhanced green fluorescent protein （EGFP） systemically. Two different types of lentiviral vectors derived from feline immunodeficiency virus （FIV） and human immunodeficiency virus （HIV） carrying EGFP were injected into the perivitelline space of MII oocytes. EGFP expression at 8-cell stage was significantly higher in the FIV group compared to the HIV group （47.5% ± 2.2% v.s. 22.9% 4± 2.9%）. Eight-cell embryos that expressed EGFP were cultured into blastocysts and then transferred into 40 heifers. Ten heifers were successfully impregnated and delivered 10 healthy calves. All of these calves expressed EGFP as detected by in vivo imaging, PCR and Southern blotting. In addition, we established an EGFP-expressing cell line from TG calves, which was followed by nuclear transfer （NT）. Recloned 8-cell embryos also expressed EGFP, and there were no differences in the rates of fusion, cleavage and development between cells derived from TG and non-TG calves, which were subsequently used for NT. These results illustrate that FIV-based lentiviruses are useful for the production of TG cattle. moreover, our established EGFP cell line can be used for additional studies that involve induced pluripotent stem cells.

AIMTo investigate the association between postoperative pain control and oncologic outcomes in resected pancreatic ductal adenocarcinoma (PDAC).METHODSFrom January 2009 to December 2014, 221 patients were diagnosed with PDAC and underwent resection with curative intent. Retrospective review of the patients was performed based on electronic medical records system. One patient without records of numerical rating scale (NRS) pain intensity scores was excluded and eight patients who underwent total pancreatectomy were also excluded. NRS scores during 7 postoperative days following resection of PDAC were reviewed along with clinicopathologic characteristics. Patients were stratified into a good pain control group and a poor pain control group according to the difference in average pain intensity between the early (POD 1, 2, 3) and late (POD 5, 7) postoperative periods. Cox-proportional hazards multivariate analysis was performed to determine association between postoperative pain control and oncologic *** total of 212 patients were dichotomized into good pain control group (n = 162) and poor pain control group (n = 66). Median follow-up period was 17 mo. A negative impact of poor postoperative pain control on overall survival (OS) was observed in the group of patients receiving distal pancreatectomy (DP group; 42.0 mo vs 5.0 mo, P = 0.001). Poor postoperative pain control was also associated with poor disease-free survival (DFS) in the DP group (18.0 mo vs 8.0 mo, P = 0.001). Patients undergoing pancreaticoduodenectomy or pylorus-preserving pancreaticoduodenectomy (PD group) did not show associations between postoperative pain control and oncologic outcomes. Reactive oxygen species （ROS） play an important role in root responses to potassium deprivation by regulating the expression of the high-affinity K＋ transporter gene AtHAK5 and other genes. Activation-tagged lines of Arabidopsis plants containing the AtHAK5 promoter driving luciferase were screened for bioluminescence under potassium- sufficient conditions. A member of the type Ill peroxidase family, RCI3, was isolated and when it was overexpressed by the activation tag, this led to the enhanced expression of luciferase and the endogenous AtHAKS. RCI3 was found to be up- regulated upon potassium deprivation. Plants overexpressing RCI3 （RCI3-ox） showed more ROS production and AtHAK5 expression whereas the ROS production and AtHAK5 expression were reduced in rci3-1 under K＋-deprived conditions. These results suggested that RCI3 is involved in the production of ROS under potassium deprivation and that RCI3- mediated ROS production affects the regulation of AtHAK5 expression. This peroxidase appears to be another component of the low-potassium signal transduction pathway in Arabidopsis roots.

A 68-year-old man underwent laparoscopic low anterior resection for rectal carcinoma in December *** 19 mo after the operation,he developed recurrent rectal cancer with peritoneal metastasis. In September 2008,he subsequently underwent a laparotomy with a peritonectomy,omentectomy, splenectomy,and a Hartmann *** intraperitoneal oxaliplatin 750 mg was administered. The patient was discharged with no postoperative complications and has been well with postoperative FOLFOX chemotherapy.

BACKGROUND A collecting duct carcinoma is a very rare, malignant renal epithelial *** metastases are present in one third of cases at the time of diagnosis. It is known to have a poor *** SUMMARY A 42-year-old male was sent to our surgery clinic for removal of a 119.2 mm ×108.3 mm encapsulated cystic mass, which was localized in the 8th segment of the right liver lobe. The lesion was first identified on ultrasonography. A computed tomography scan confirmed the presence of a Bosniak type Ⅲ cystic lesion,which affected the liver and convexity of the right kidney. Surgical intervention involved a right nephrectomy, with removal of the cystic mass. The patient was mobilized on the first postoperative day and was discharged after 7 d. The histological and immunohistochemical examination revealed a low-grade collecting duct renal carcinoma, which is a rare variant of papillary carcinoma,with low malignant potential. The patient did not receive chemotherapy and after 21 mo of follow-up, a radiological examination and laboratory analyses showed normal aspects.
