Biliary strictures are characterized by the narrowing of the bile duct lumen,usually caused by surgical biliary injury,cancer,inflammation,and scarring from *** stent placement is a well-established method for the management of biliary ***,maintaining optimal mechanical properties of stents and designing surfaces that can prevent stent-induced tissue hyperplasia and biofilm formation are challenges in the fabrication of biodegradable biliary stents(BBSs)for customized *** study proposes a novel approach to fabricating functionalized polymer BBSs with nanoengineered surfaces using 3D *** 3D printed stents,fabricated from bioactive silica poly(ε-carprolactone)(PCL)via a sol-gel method,exhibited tunable mechanical properties suitable for supporting the bile duct while ensuring ***,a nanoengineered surface layer was successfully created on a sirolimus(SRL)-coated functionalized PCL(fPCL)stent using Zn ion sputtering-based plasma immersion ion implantation(S-PIII)treatment to enhance the performance of the *** nanoengineered surface of the SRL-coated fPCL stent effectively reduced bacterial responses and remarkably inhibited fibroblast proliferation and initial burst release of SRL in vitro *** physicochemical properties and biological behaviors,including in vitro biocompatibility and in vivo therapeutic efficacy in the rabbit bile duct,of the Zn-SRL@fPCL stent demonstrated its potential as a versatile platform for clinical applications in bile duct tissue engineering.

BACKGROUND Breast cancer in young women has been shown to have an aggressive behavior and poor *** To evaluate the outcomes of young hormone receptor(HR)-positive patients with breast cancer treated with neoadjuvant chemotherapy(NAC),and the oncologic efficacy of gonadotropin-releasing hormone(GnRH)*** This retrospective study involved a prospectively enrolled *** included patients diagnosed with invasive breast cancer who were treated with NAC followed by curative surgery at the Samsung Medical Center and Samsung Changwon Hospital between January 2006 and December *** patients with HR-positive and human epidermal grow factor 2(HER2)-negative breast cancer,we analyzed the characteristics and oncology outcomes between the patients equal to or younger than 35 years and the patients older than 35 *** Among 431 patients with NAC and HR-positive/HER2-negative breast cancer,78 were 35 years old or younger,and 353 patients were older than 35 *** median follow-up was 71.0 *** was no statistically significant difference in disease free survival(DFS,P=0.565)and overall survival(P=0.820)between the patients equal to or younger than 35 years and the patients older than 35 *** two groups differed in that the GnRH agonist was used more frequently in the group of patients equal to or younger than 35 years than in the other group(52.4%vs 11.2%,Phan 35 years,patients treated with the GnRH agonist had better DFS(P=0.037).CONCLUSION Administration of GnRH agonists might improve the DFS rate of HR-positive/HER2-negative breast cancer in the equal to or younger than 35 years group of patients with NAC.

High-and medium-entropy alloys(HEAs and MEAs)possess high solid-solution *** investigations have been conducted on its impact on yield strength,however,there are limited reports regarding the relation between solid-solution strengthening and strain-hardening *** addition,no attempt has been made to account for the dislocation-mediated plasticity;most works focused on twinning-or transformation-induced plasticity(TWIP or TRIP).In this work we reveal the role of solidsolution strengthening on the strain-hardening rate via systematically investigating evolutions of deformation structures by controlling the Cr/V ratio in prototypical V_(1-x)Cr_(x)CoNi *** the TWIP of CrCoNi with the dislocation slip of V_(0.4)Cr_(0.6)CoNi,the hardening rate of CrCoNi was superior to slip-band refinements of V_(0.4)Cr_(0.6)CoNi due to the dynamic Hall-Petch ***,as V content increased further to V_(0.7)Cr_(0.3)CoNi and VCoNi,their rate of slip-band refinement in V_(0.7)Cr_(0.3)CoNi and VCoNi with high solid-solution strength surpassed that of *** it is generally accepted in conventional alloys that deformation twinning results in a higher strain-hardening rate than dislocation-mediated plasticity,we observed that the latter can be predominant in the former under an activated huge solid-solution strengthening *** high solid-solution strength lowered the cross-slip activation and consequently retarded the dislocation rearrangement rate,i.e.,the dynamic *** delay in the hardening rate decrease,therefore,increased the strain-hardening rate,results in an overall higher strain-hardening rate of V-rich alloys.

Development of tenofovir disoproxil fumarate resistance after complete viral suppression in a patient with treatment-na?ve chronic hepatitis B: A case report and review of the literature

Tenofovir disoproxil fumarate(TDF) is a potent nucleo-tide analogue that is recommended as first-line therapy for patients with chronic hepatitis B. The results of a longitudinal study of TDF treatment demonstrated no development of resistance. We observed one treatment-na?ve chronic hepatitis B(CHB) patient who developed TDF resistance after complete viral suppression during long-term TDF treatment. A 37-year-old HBe Ag-positive man received TDF 300 mg/d for 43 mo. The hepatitis B virus(HBV) DNA titer was 8 log_(10) copies/m L at baseline and became undetectable at 16 mo after treatment. However, the HBV DNA titer rebounded to 7.5 log_(10) copies/m L at 43 mo after treatment. We performed full sequencing to find mutation sites associated with virologic breakthrough. The results showed 9 mutation sites, most of which had not been well-known as mutation sites. We changed the therapy from tenofovir to entecavir with a regimen of 0.5 mg once daily. After 4 mo, the HBV DNA titer decreased to 267 copies/m L, and the liver enzyme levels were normalized.

Liver intravoxel incoherent motion diffusion-weighted imaging for the assessment of hepatic steatosis and fibrosis in children

AIM To evaluate the correlation between intravoxel incoherent motion(IVIM) diffusion-weighted imaging(DWI) parameters and the degree of hepatic steatosis and fibrosis in *** This retrospective study was approved by the institutional review board. The children(≤ 18 years) who underwent liver IVIM DWI with 8 b-values under the suspicion of hepatic steatosis or fibrosis from February 2013 to November 2016 were included. Subjects were divided into normal, fatty liver(FAT), and fibrotic liver(FIB) groups. The slow diffusion coefficient(D), fast diffusion coefficient(D*), perfusion fraction(f), and apparent diffusion coefficient(ADC) were measured. MR proton density fat fraction(PDFF), MR elastography(MRE), and IVIM values were *** A total of 123 children(median age of 12 years old, range: 6-18 years) were included, with 8 in the normal group, 93 in the FAT group, and 22 in the FIB group. The D* values were lower in the FIB group compared with those of the normal(P = 0.015) and FAT(P = 0.003) groups. The f values were lower in the FIB group compared with the FAT group(P = 0.001). In multivariate analyses, PDFF value was positively correlated with f value(β = 3.194, P < 0.001), and MRE value was negatively correlated with D* value(β =-7.031, P = 0.032). The D and ADC values were not influenced by PDFF or MRE *** In liver IVIM DWI with multiple b-values in children, there was a positive correlation between hepatic fat and blood volume, and a negative correlation between hepatic stiffness and endovascular blood flow velocity, while diffusion-related parameters were not affected.
