Objective: To investigate the uptake rate of prostate specific antigen(pSA) testing among Hong Kong Chinese males aged 50 or above, and identify factors associated with the likelihood of undergoing a pSA ***: A population-based telephone survey was conducted in Hong Kong in 2007. The survey covered demographic information, perceived health status, use of complementary therapy, cancer screening behavior, perceived susceptibility to cancer and family history of cancer. Descriptive statistics, percentages and logistic regression analysis were used for data ***: A total of 1,002 men aged 50 or above took part in the study(response rate =67%), and the uptake rate of pSA testing was found to be 10%. Employment status, use of complementary therapy, perceiving regular visits to a doctor as good for health and the recommendations of health professionals were significant factors associated with pSA ***: The uptake rate of pSA testing in the study population was very low. Among all the factors identified, recommendations from health professionals had the strongest association with the uptake of pSA testing, and they should therefore take an active role in educating this population about cancer prevention and detection.

目的:研究向基层机构倾斜的门诊报销制度(即覆盖门诊服务且县、乡、村门诊报销比例由低到高的制度设计)对高血压患者门诊服务利用的影响。方法:从宁夏回族自治区"创新支付制度项目"获取2009年和2011年入户调查的面板数据,采用自然试验的设计,针对干预组和对照组,利用双重差分法(difference-in-differences)进行分析,其结果指标为自报高血压患者样本被调查前3个月内是否就诊及其主要就诊地点的选择。结果:相较于对照组,倾向于基层的新农合门诊覆盖使得干预组高血压患者被调查前3个月内从未就诊的概率下降10.2%,并使患者将村卫生室作为主要就诊地点的概率提高了15.7%。结论:门诊报销比例向基层机构倾斜,可以引导高血压患者在疾病风险较小时就近利用门诊服务。

This study aimed to investigate the association between atrophy in the prefrontal cortex with executive function and verbal fluency in elderly male and female patients poststroke. Thirty elderly female patients with non-aphasic ischemic stroke aged -〉 60 years and 30 age-matched non-aphasic male patients with ischemic stroke were recruited. Automatic magnetic resonance imaging segmentation was used to assess the volume of the whole prefrontal cortex, along with its subdivisions： anterior cingulate cortex, orbitofrontal cortex and dorsolateral prefrontal cortex. The Semantic Verbal Fluency Test was administered at 3 and 15 months poststroke. At 3 months poststroke, left dorsolateral prefrontal cortex volume was significantly correlated with Verbal Fluency Test score in female patients only （partial coefficient = 0.453, p = 0.045）, after controlling for age, education, diabetes, neurological deficit, white matter lesions volume, as well as the location and volume of infarcts. At 15 months poststroke, there remained a significant association between the left dorsolateral prefrontal cortex volume and Verbal Fluency Test （partial coefficient = 0.661, p = 0.001） and between the left prefrontal cortex volume and Verbal Fluency Test （partial coefficient = 0.573, p = 0.004） in female patients after the same adjustments. These findings indicate that atrophy of the left dorsolateral prefrontal cortex contributes to the impairment of verbal fluency in elderly female patients with stroke. Sex differences may be present in the neuropsychological mechanisms of verbal fluency impairment in patients with stroke.

Exercise is proven to be effective, safe and the preferred intervention for improving the range of shoulder motion of breast cancer patients who have undergone surgery for axillary lymph-node dissection. The application of evidence-based guidelines to clinical practice can help healthcare professionals to provide good quality care to patients and, in turn, produce better patient outcomes. The purpose of the present paper is to describe the development process of an evidenced-based guideline.
